                            Respiratory Disease

Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing , etc,.

                                      Upper respiratory tract infection

Upper respiratory tract infections are considered to be the infection of the airway above the glottis or vocal cords. This includes the nose, sinuses, pharynx, and larynx, including the infection of tonsillitis, pharyngitis, laryngitis, sinusitis, otitis media, etc,.

                                               The Symptoms

The symptoms of include nasal congestion, a runny nose, sneezing, pharyngitis/tonsillopharyngitis, headache, muscle aches (usually severe) sore throat, watery eyes, etc,. Influenza infections may result in different clinical presentations. In the study to determine the clinical differences between circulating influenza strains in a young healthy adult population in the tropics, found that there were no statistical differences between H3N2 and influenza B (p = 0.58). Those with nasal congestion, rash, eye symptoms, injected pharynx or fever were more likely to have H3N2; and those with sore throat, fever, injected pharynx or rhinorrhoea were more likely to have influenza B than H1N1-2009(1). Others in the study of the differing symptom patterns in early pandemic vs seasonal influenza infections, conducted by Department of Laboratory Medicine, National University Hospital, Singapore, found that from the week of June 16 to June 23, 2009, this pandemic influenza A(H1N1/2009) displaced and then replaced the seasonal influenzas (H3N2, H1N1, and B). Of 2683 samples tested during this 12-week surveillance period, 742 (27.6%) were positive for any influenza virus using this assay, with 547 cases of A(H1N1/2009) (20.4%), 167 cases of A(H3N2) (6.2%), 14 cases of A(H1N1) (0.5%), and 12 cases of influenza B (0.4%). Results of multivariate analysis showed that age (P < .001), fever (P < .001), cough (P < .001), sore throat (P = .002), rhinorrhea (P = .001), and dyspnea (P < .001) were significantly different among these groups.(2). According to the study of the symptoms of the common cold and influenza conducted by Cardiff School of Biosciences, Cardiff University, indicated that the effects of cytokines in human beings now helps to explain some of the symptoms of colds and flu that were previously in the realm of folklore rather than medicine-eg, fever, anorexia, malaise, chilliness, headache, and muscle aches and pains. The mechanisms of symptoms of sore throat, rhinorrhoea, sneezing, nasal congestion, cough, watery eyes, and sinus pain(3).

The smoothie for reduced risk and treatment of Cough

Yield: 2 servings (about 8 ounces each)
1/4  cup wolf berry
1/8 cup honey
1 3/4 cups orange juice

1. Place tall ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately

The finding of a natural source for treatment of Cough without induced side effects has encountered many obstacles, many ingredients showed initially the promising result in animal studies have not produced same potentials in either large sample size and mutli centers human trials.
Orange(1) and Wolfberry(4) Juice and Honey(6) in recent studies by renowned institution suggested that the combined ingredients may consist a potential effect for production of natural alternation in treating  respiratory disease, including cough.

Cough is a continued reflection and natural reaction of lung to clear the large breathing passages from secretions, irritants, foreign particles, etc. or as a result of infection such as acute (less than 3 weeks) or chronic (over 5 weeks) respiratory diseases.Vitamin C, also known as L-ascorbic acid, a water-soluble vitamin, found abundantly in orange juice, is best known for its free radical scavengers activity and regenerating oxidized vitamin E for immune support in reduced risk of "morning cough", "chronic cough", "wheeze" and "wheeze ever(1).
According to the National Institute of Public Health and the Environment (RIVM), intake of vitamin C exhibited protective effects against respiratory symptoms in comparison to those with lower intake(2).
Dr.Goldman RD at the Canadian Paediatric Society, Drug Therapy and Hazardous Substances Committee, suggested, use of over counter medicine for treatment of cough in adult populations when cough frequency and severity, may cause medication errors and adverse events(3). Therefore, cough and cold therapies should consider intake of vitamin C and other forms of natural sources instead(3).

Wolfberry, also known as  lycium fruits is the common name for the fruit of two very closely related species, the genus of Lycium, belongings to family Solanaceae, native to native to southeastern Europe and Asia. has been found effective in treatment of various human infectious diseases, including cough, according to the Hazara University Mansehra, Pakistan(4).

Dr. Cassileth B, professor at the Memorial Sloan-Kettering Cancer Center, said, " The berries of Lycium barbarum, a perennial plant native to Asia and southeastern Europe, have been used for centuries in traditional Chinese medicine to treat poor vision, anemia, inflammation, and cough"(6).
The fruit water extract also exhibited anti-inflammatory effect for treatment of cough through suppression of pro inflammatory cytokines(5).
Honey, the rich golden liquid is the miraculous product made by bees using nectar from flowers, is considered as one of healthy sweet food for replacing the use of white sugar and artificial sweetener by many people. Recently, it is considered as effective treatment of childhood cough, persistent post-infectious cough, according to the joint study lead by the Maastricht University Medical Center (MUMC)(7).
Dr. Ran D. Goldman, at the the College of Family Physicians of Canada said, "There is increasing evidence that a single dose of honey might reduce mucus secretion and reduce cough in children."(8).
In the study of honey for acute cough in children, Dr. Oduwole O and the research team at the University of Calabar Teaching Hospital (ITDR/P), insisted (without consideration of the adverse effect of conventional medicine of dextromethorphan) that honey may be better than 'no treatment' and diphenhydramine in the symptomatic relief of cough but not better than dextromethorphan(9).

Orange juice, containing the highest amount of vitamin C in comparison to other fruits may mimic the similar effects when use conjunction with wolf berry and honey for reduced risk and treatment of cough with no adverse effects.

                            Respiratory Disease

Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing , etc,.
Restrictive lung diseases or restrictive ventilatory defects
Restrictive lung disease is a condition marked most obviously by a reduction in total lung capacity. A restrictive ventilatory defect may be caused by a pulmonary deficit, such as pulmonary fibrosis (abnormally stiff, non-compliant lungs), or by non-pulmonary deficits, including respiratory muscle weakness, paralysis, and deformity or rigidity of the chest wall(1).

                                                  The Treatment

In herbal medicine perspective
The aim od herbal medicine is to prevent lung infections and relieve its symptoms. Herbs support the lungs, strengthen the immune system and repair tissues, include
1. Herbs support the lungs
In the study of the effect of astragalus membranceus(fisch) bge, codonpsis pilosula and glycyrrhiza uralensis fisch on airway responsiveness, found that after 28 asthmatic patients received the treatment of those herbs, the results showed that FVC value increased obviously (t = 2.217, P < 0.05) and the value of FEV1 and PEF was all higher compared with before treatment of the herbs (P < 0.01)(33).
2. Herbs enhance immunity
In the study of the effect of Chinese herbs (Astragalus radix and Ganoderma lucidum) on immune response of carp in fish fed diets containing Astragalus (0.5%), Ganoderma (0.5%) and combination of two herbs (Astragalus 0.5% and Ganoderma 0.5%) for 5 weeks, showed that feeding non-vaccinated and vaccinated carp with combination of Astragalus and Ganoderma stimulated respiratory burst activity, phagocytosis of phagocytic cells in blood and lysozyme and circulatory antibody titres in plasma in vaccinated carp. Fish challenged with A. hydrophila had variable survival. The best survival (60%) was in vaccinated group fed with both herbs, while almost 90% of control fish (negative control) and 60% of fish vaccinated only (positive control) died(34).
3. Herbs repair tissues
According to the study by Ohio State University’s Heart and Lung Research Institute, Grape-seed extractseemed to aid wound healing in two ways: It helped the body make more of a compound used to regenerate damaged blood vessels, and it also increased the amount of free radicals in the wound site. Free radicals help clear potentially pathogenic bacteria from a wound(35).

The smoothie for reduced risk and treatment of Cold Sores

Yield: 2 serving (about 8 ounce each)
1 1/2 cups orange
1 cup green tea drink (Make from 4 grams of green tea, a slice of ginger and a cup of hot water lipped for 5 minutes, and let cool to room temperature)

1. Place tall ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately

The finding of a natural source for reduced risk and treatment of Cold Sores with no adverse effects has encountered many obstacles, many ingredients showed initially the promising result in animal studies have not produced same potentials in either large sample size and mutli centers human trials.
Recent studies conducted by respectable institution suggested that green tea(1), Orange juice(3) may consist a potential response for reduced risk and treatment of cold sore with little or no adverse effects

Cold sores (Herpes labialis) is an infection of the lip as a result of the outbreak of by herpes simplex virus (HSV-1). The blister is healed within 2 - 3 weeks, but the virus remains dormant in the facial nerve and the rate of the recurrence of the diseases is very high to rare episodes to 12 or more.

Green tea contains more amount of antioxidants than any drinks or food with the same volume, and is the leaves of Camellia sinensis, undergone minimal oxidation during processing, originated from China. According to the Zhejiang University,AverTeaX formulation containg high amount of catechins effectively inhibited herpes simplex labialis infection with clinical significance(1).
Dr Zhao M and colleagues in the study of inhibitory effects on herpessimplex virus suggested topical formulation containing EGCG-stearate in 100% glycerin USP effectively prevent and treat HSV-induced symptoms(2).

Orange is a species of Citrus Sinensis, belong to the family Rutaceae and native to the Southeast Asia. Orange is round citrus fruits with finely-textured skins, orange in color and one of most popular fruits in the world. Its juice contains highest amount of vitamin C in comparison of other juice. According to the Universidad Autonoma del Estado de Mexico, oral administration of 1 mg of lignin-ascorbic acid tablet or solution three times daily for a month inhibited HSV-1 infection(3).
In a randomized double-bind, placebo-controlled clinical trial, topical ascorbic acid-containing pharmaceutical formulation, Ascoxal, exerted anti antiviral effects herpes simplex virus with a statistically significant clinical result(4).
Dr. Terezhalmy GT, and colleagues in the study of treatment of recurrent herpes labialis suggested, application of different doses of water-soluble bioflavonoid-ascorbic acid inhibited the initial symptoms associated with recurrent herpes labialis(5).

The identified ingredients of catechins and vitamin C found abundantly in green tea and orange should provide us with strong evidences in support the use of smoothie in reduced risk and treatment of cold sore.

People who are at increased risk of Cold Sores due to family history, nutrient deficiency.. .....should drink at least one serving daily and people with Cold Sores should drink the juices as much as they can, depending to digestive toleration.
Change of life style and diet pattern are also recommended.

The smoothie for prevention and treatment of Diarrhea
Yield: 2 serving (about 8 ounce each)
1 cup banana
1/2 cup yogurt
1 cup ginger tea
(Make from 2 bags of herbal ginger tea, and a cup of hot water lipped for 5 minutes. Set aside for cooling to room temperature)

1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed. Add more green tea drink if needed
3. Serve immediately

The finding the natural ingredients for improvement of diarrhea without side effect is considered as a dream of many scientists. Unfortunately, many compounds found effective in initial studying failed to confirm the potential in large sample size and multi center. Some even produce conflicted results in second and further studies.

Diarrhea is  a condition of passing of loose or watery stools as a result of viruses (rotavirus, norovirus, etc.), bacteria (food poisoning, E coli), parasites effects, intestinal disorders, fat mal-absorption, laxative abuse ..., etc. alkaloids, tannins, flavonoids and terpenes.

Recent studies back by respectable institutions suggested that ginger, may have a therapeutic values used for reduced risk and treatment of diarrhea, particular in underdevelopment countries where sanitation is a problem. According to Dr. Gao F.,  diarrhea-predominant irritable bowel syndrome can be treated by ginger-partitioned moxibustion and acupoint application(1).

Ginger (Zingiber officinale) or ginger root is the genus Zingiber, belonging to the family Zingiberaceae, native to Tamil. It has been used in traditional and Chinese medicine to treat dyspepsia, gastroparesis, constipation, edema, difficult urination, colic, etc.

According to the United Graduate School of Veterinary Sciences, zingerone can inhibit colonic motility via direct action on smooth muscles of that may might exert beneficial therapeutic effects on diarrhea by excessive gastrointestinal motility(2).
In Enterotoxigenic Escherichia coli heat-labile enterotoxin (LT)-induced diarrhea animal model, chemically synthesized zingerone derivatives, compound 31 (2-[(4-methoxybenzyl)oxy]benzoic acid) significantly suppressed LT-induced diarrhea in mice through phase one cell cycle arrest(3).

Yogurt, a foods food produced by bacterial fermentation of milk may also be used for treatment of diarrhea(4).According to the China Medical University, yogart as probiotic effectively treat diarrhea in patients with diarrhea within 15 days(4).
In acute diarrhea in 6-24-month-old hospitalized infants, Yogurt treatment significantly improved mean hospitalization days, reduced diarrhea frequency and weight gain, according to Dr. Pashapour N and Dr. Iou SG.(5).
Furthermore, according to the joint study lead by the  University of Tasmania, yogurt, containing containing Lactobacillus rhamnosus GG (LGG), Bifidobacterium lactis (Bb-12) and Lactobacillus acidophilus (La-5) may be effective for reducing the incidence of antibiotic-associated diarrhoea in children(6).

Banana is the common name of a genus of tropical herbaceous plants, in recent studies also expressed its anti diarrhea effects up to 57% in test subjects(7). In the comparison of Warfarin and Banana Flakes Supplement on diarrhea, researchers at the showed that oral administration of dried banana flakes treat diarrhea effectively and subsequent decrease in the rate and extent of diarrhea(8), according to the Thomas Jefferson University.
Dr. Rabbani GH and colleagues at the Centre Health and Population Research, Bangladesh in the investigation of the effects of green banana and pectin suggested, green banana mediated the stimulation of colonic small bowel absorption through their antidiarrheal and colonotrophic effects(9).

The discovery of simple ingredients which can be found easily in the market of third world countries may act as a cornerstone for the development of potential and functional foods for reduced risk and treatment of  Diarrhea.
People with higher risk of diarrhea due to digestive problem and family history should drink at least one serving a day and people with diarrhea should drink 4 servings daily, depending to the digestive toleration.

In conventional medicine perspective
Treatments are depending to the underline causes in diagnosis
F.1.1. Non medication Therapies
1. Pulmonary rehabilitation
Pulmonary rehabilitation programs improve exercise tolerance, muscle strength, and dyspnea in patients with COPD. In the study to assess prospectively the effectiveness and feasibility of pulmonary rehabilitation in patients with restrictive lung diseases by Department of Respiratory Medicine, Ghent University Hospital, showed that patients with RLD respond well after 12 weeks of pulmonary rehabilitation, and even better results were seen after 24 weeks. Clinically significant improvements were obtained in the majority of the patients after 24 weeks(26).
2. Oxygen therapy
Collective experience with pulmonary rehabilitation and disease management has shown that patients with lung diseases including COPD and restrictive lung diseases live a longer and more productive quality of life if they can remain active(27).
3. Continuous positive airway pressure (CPAP)
The aim of CPAP is to provide continuous positive pressure to maintain a continuous level of positive airway pressure of that can improve oxygenation in a patient whose hypoxemia is refractory to oxygen therapy. Evidence suggests that use of CPAP for longer than 6 hours decreases sleepiness, improves daily functioning, and restores memory to normal levels(28).
4. Mechanical ventilation support
The aim of Mechanical ventilation is to mechanically assist or replace spontaneous breathing, if the patient is on his/her way to respiratory failure. High levels of prolonged pressure support ventilation promote diaphragmatic atrophy and contractile dysfunction. Furthermore, similar to controlled mechanical ventilation, pressure support ventilation-induced diaphragmatic atrophy and weakness are associated with both diaphragmatic oxidative stress and protease activation(29).
F.1.2. Medication therapy
1. Inhaled corticosteroids
Inhaled corticosteroids act locally in the lungs to inhibit the inflammatory process and support the function to relieve the symptoms of Restrictive lung disease. In the study of nine pregnant women with interstitial and restrictive lung disease between 1981 and 1994, showed that hree patients had severe disease, characterized by vital capacity < or = 1.5 L (50% predicted) or diffusing capacity < or = 50% predicted. Five patients had exercise-induced oxygen desaturation, and four required supplemental oxygen. Five patients required corticosteroids. One patient had an adverse outcome; she was delivered at 31 weeks and required mechanical ventilation for 72 hours.
