Respiratory Disease
Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing, etc,.
Restrictive lung diseases or restrictive ventilatory defects
Restrictive lung disease is a condition marked most obviously by a reduction in total lung capacity. A restrictive ventilatory defect may be caused by a pulmonary deficit, such as pulmonary fibrosis (abnormally stiff, non-compliant lungs), or by non-pulmonary deficits, including respiratory muscle weakness, paralysis, and deformity or rigidity of the chest wall(1).
The Symptoms
1. Dyspnea (shortness of breath)
Patients with obstructive and restrictive ventilatory abnormalities suffer from exercise intolerance and dyspnea. Breathing pattern components (volume, flow, and timing) during incremental exercise may provide further insight in the role played by dynamic hyperinflation in the genesis of dyspnea parameters(2).
2. Wheezing
Wheezing occurs during breathing, as a result of some parts of the respiratory tree must be narrowed or obstructed due to inflammation of the airways expiration causes of a blockage in the airways or lungs.
3. Chronic coughing
Chronic cought is a insistently repetitive or continuous cough last longer than eight weeks.
According to the study of 10 patients with ulcerative colitis, all of whom were non-smokers, presented with a productive cough. In six, the chest radiograph was normal and cough was the only symptom; three of these patients had a minor obstructive ventilatory defect on testing. Four patients complained
of exertional dyspnoea and had both an abnormal chest radiograph with bilateral pulmonary shadows and a mixed obstructive and restrictive ventilatory defect(3).
4. Coughing up blood
It may be a result due to bleeding of the upper Respiratory tract or lung tumor.
5. Chest pain
Half of patients with respiratory Diseases are experience symptoms of chest pain(4).
6. Etc.

Musculoskeletal disorders (MSDs) are medical condition mostly caused by work related occupations and working environment, affecting patients’ muscles, joints, tendons, ligaments and nerves and developing over time. A community sample of 73 females and 32 males aged 85 and over underwent a standardised examination at home. Musculoskeletal pain was reported by 57% of those interviewed. A major restriction of joint movement range was frequent in the shoulder but uncommon in other joints(1).
Types of Musculo-Skeletal disorders in elder(2)
1. Osteoarthritis
2. Gout
3. Rheumatoid Arthritis
4. Polymalagia Arthritis
5. Cervical myleopathy and spinal canal stenosis
6. Osteoporosis
7. Low back pain

Rheumatoid Arthritis
Rheumatoid Arthritis is defined a chronic disorder as a result of inflammation, affecting mostly the flexible (synovial) joints and tissues and organs in the body. The disease affects more women than men and generally occurs after the ages of 40, causing diminished quality of life of many elders(1). According to CDC, Musculoskeletal disorders (MSDs) affects over 52 millions of adults in the US alone, including 294,000 children under age 18 with some form of arthritis or rheumatic conditions(2). Rheumatoid Arthritis can induced bone loss through elevating bone resorption without increasing bone formation(4). A cross-sectional population-based study of 1042 patients with rheumatoid arthritis showed that RA patients had an increased risk of death from various causes(4a).

B.4. The Important supplements for rheumatoid arthritis
1. Glucosamine
Glucosamine, a compound of the simple sugar glucose and the amino acid glutamine, is a precursor for glycosaminoglycans(194,289), a major component of joint cartilage(195,290). In a randomized placebo-controlled study of 51 rheumatoid arthritis (RA) patients: 25 patients in the glucosamine group and 26 patients in the placebo group, Glucosamine hydrochloride at a daily dose of 1,500 mg showed to improve some symptomatic effects on RA(291).
The orally administered combination of a glucosamine-chondroitin-quercetin glucoside (GCQG) supplement also exerted a special effect on improving the synovial fluid properties in OA patients, according to the Matsuno Clinic for Rheumatic Diseases(292). Furthermore, a novel glucosamine analogue also showed to inhibit the development of RA through exerting antioxidative and immunomodulatory effects(293).
2. Omega 3 fatty acids
Omega-3 fatty acids found abundantly in cold water fish, algae and seeds and nuts of plant has shown to exhibit the prevention of different pathologies, such as cardiovascular, psychiatric, neurological, dermatological and rheumatological disorders(297), including RA. Dr. Shafaghat A said," The hexane extract from the flower, leaf, stem, and seed contained 39.1%, 43.2%, 29.0%, and 37.6% of omega-3 fatty acids, ... possess considerable antioxidant activity(298) of that may be helpful in attenuate the oxidative stress in induction of RA.Dr. Jeromson S. and the research team at the University of Stirling suggested that Omega 3 fatty acid also plays an essential role in Skeletal Muscle Health(299) in inhibition of pro inflammatory cytokines(300) which has a direct influence to the onset of rheumatoid arthritis. In Rheumatoid Arthritis, omega-3 fatty acid supplementation improved physical functioning, physical role, bodily pain, general health, vitality, social functioning, grip strength, morning stiffness,....in comparison to the effectiveness of non-steroidal anti-inflammatory drugs (NSAIDs) in patient of rheumatoid arthritis(301).

Hypertensive nephropathy is health condition of a progressive kidney disease caused by chronic hypertension. According to the Transplantologii i Chorób Wewnetrznych Akademii Medycznej w Gdańsku, treatment of hypertension according to international standards would allow to reduce the still increasing number of patients with hypertensive nephropathy(*).
The smoothie for reduced risk and treatment of Hypertensive nephropathy
Yield: 2 servings (about 8 ounces each)
3/4 cup celery
3/4 cup of apple with peel
1 cup green tea drink 1 cup green tea drink (Make from 4 grams(2 tea bags) of green tea, a slice of ginger and a cup of hot water lipped for 5 minutes, and set aside for cooling to room temperature)
1. Place the celery, apple with peels, green tea, in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately
Blood pressure is the force of blood pushing against the walls of the arteries as the heart pumps out blood. high blood pressure means raising pressure in your heart. If your blood pressure stays high over time it can damage the body in many ways.
Blood pressure medications have undoubtedly prevented many deaths from heart disease in the past 30 years, but also induced many irreversible adverse effects, including kidney damage.
Diet rich in fruits and vegetable and reduced intake of red meat, saturated fat and trans fat has shown to attenuated risk of hypertension(1). Recent study showed that celery(2) intake used conjunction with berries(3) and green tea(4) may be the potential drink for prevention and treatment of hypertension.
Celery is a species of Apium graveolens, belongings to the family Apiaceae, cultivated all around the globe as a vegetable. According to Chronic administration of hexanic, methanolic, and aqueous-ethanolic extracts of celery seed effectively exhibited an antihypertensive activity in chronic treatment of elevated BP in tested animals(5).
Dr. Dianat M and the research team at the Ahvaz Jundishapur University of Medical Sciences said," hydro-alcoholic celery leaf extract on systolic blood pressure (SBP), heart rate (HR) and lipid profile in animals' model of hypertension induced by fructose"(6)
3‑n‑butylphthalide (NBP), an phytochemical found in celery seed used in traditional Chinese medicine exhibited significantly antihypertensive and attenuated progression of hypertensive effects through decreased oxidative stress and the expression of pro‑inflammatory cytokines(7).
Apple is the pomaceous fruit of the apple tree, a species of the rose family Rosaceae. It is one of the most widely cultivated tree fruits. With phytochemical flavonoids, apple and its peel inhibited angiotensin converting enzyme (ACE) in promoted the development of high blood pressure, Dr Balasuriya N and Dr. Rupasinghe HP. Suggested(8)According to the Mount Sinai School of Medicine, "Flavonoid (found abundantly in fruits and vegetable)(10) intake has been inversely associated with mortality from coronary heart disease and stroke,..... Lowering blood pressure through increased dietary consumption of dietary antioxidants(such as flavonoid) may decrease the rate of end-organ damage that is secondary to hypertension"(9).
Green tea has been a precious drink in traditional Chinese culture and used exceptional in socialization for more than 4000 thousand years. Due to its commercial values and health effects, green tea now has been cultivated all over the world in suitable climate.
Epigallocatechin-3-O-gallate (EGCG), the main polyphenol presence in green tea is best known for its antioxidant and anti inflammatory effect. In animal model, the phytochemical exhibited anti hypertensive and progressive activity through reduction of oxidative stress in paraventricular nucleus of hypothalamus (PVN), the Xi'an Jiaotong University Health Science Center study showed(11).
Dr. Yarmolinsky J and the research team at the Federal University of Rio Grande do Sul, suggested that, intake of tea are shown to reduce blood pressure in individuals within prehypertensive and hypertensive ranges in the review of all relevant studies published from 1946 to September 27, 2013.(12).
Furthermore, green tea extract (GTE) inhibited the oxidative stress and ingiotensin (Ang) II induces endothelial dysfunction (ED) causes of hypertension through scavenging of superoxide anion generation, the University of Padova, insisted(13).
People who are at increased risk of Hypertensive nephropathy due to chronic hypertension, family history .....should drink at least one cup daily and people with Hypertensive nephropathy should drink the juices at least 3 times a day, depending to digestive toleration.
Change of life style and diet pattern are also recommended.

Respiratory Disease
Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing, etc,.
Pulmonary vascular disease: Pulmonary veno-occlusive disease
Pulmonary vascular disease is defined as a condition of blood flow to the lung’s artery is blocked suddenly due to a blood clot somewhere in the body, including pulmonary embolism, chronic thromboembolic disease, pulmonary arterial hypertension, pulmonary veno-occlusive disease, pulmonary arteriovenous malformations, pulmonary edema, etc.
Pulmonary veno-occlusive disease (PVOD) is an extremely rare form of pulmonary hypertension, affecting mostly in children and young adults as a result of a progressive obstruction of small pulmonary veins that leads to elevation in pulmonary vascular resistance and right ventricular failure.
Treatments In conventional medicine perspective
A. Non surgical therapy
1. Oxygen therapy
Total correction was performed in a case of complete transposition of the great arteries (TGA) with severe pulmonary vascular obstructive disease (PVOD). Although severe pulmonary hypertension remained after surgery, oxygenation was continued for 15 months, which included a shift to at-home oxygen inhalation therapy (HOT). Cardiac catheterization 15 months after surgery demonstrated that pulmonary hypertension was greatly improved, According to the study by the Nagoya University Graduate School of Medicine(49).
2. Warfarin therapy
In PVOD, one could suggest that caution should be taken in the application of anticoagulation, because of the frequent association with occult alveolar hemorrhage. However, we have reported the same proportion of hemoptysis in idiopathic PAH and PVOD, even with the use of anticoagulation, according to the study by Alice Huertas, MD; Barbara Girerd, MD; Peter Dorfmuller, MD, PhD; Dermot O’Callaghan, MD; Marc Humbert, MD, PhD; David Montani, MD, PhD(50).
3. Immunosuppressive agents
In the study of 14 subjects with PAH, including seven patients with the idiopathic form and seven patients with atrial septal defects, but no other congenital heart abnormalities, indicated that chronic administration of sildenafil improves the physical capacity of PAH patients and may be beneficial in selected cases of veno-occlusive disease(51).
B. Lung Transplantation
According to the study by the, Single living-donor lobar lung transplantation is a unique method of treatment, mostly for small children, and size matching is the most important factor to conduct single living-donor lobar lung transplantationsafely. We report a successful single living-donor lobar lung transplantation for a 6-year-old girl with pulmonary venoocclusive disease who received the graft from her mother. Preoperatively, the recipient was intubated under deep sedation because of repeated episodes of pulmonary edema due to rapidly deteriorating pulmonaryvenoocclusive disease(52).
As the prognosis in PVOD patients is worse than in idiopathic PAH, and due to limited medical therapy, lung transplantation should be considered and discussed early in the course of PVOD or suspected PVOD (HRCT findings, worsening symptoms on treatment or refractoriness to medical therapy, development of worsening hypoxemia or chronic heart failure). In these patients, PAH-specific therapies may serve as a bridge to lung transplantation(50). Other study indicated that patients with PVOD present with symptoms of right-sided heart failure. Radiologic examination shows prominent pulmonary arteries with Kerley B lines, pleural effusion, and mediastinal adenopathy. The definite diagnosis is made by histologic examination. Eccentric intimal fibrosis and recanalized thrombi inpulmonary veins and venules, arterialized veins, alveolar edema, and medial hypertrophy of arteries are seen on lung biopsy. No effective treatment is available; lung transplantation has been tried. The prognosis associated with PVOD is poor(53).
General recommendations include limiting of physical activity to tolerance and avoidance of concomitant medications that can potentially aggravate pulmonary hypertension (such as β-adrenergic receptor blockers) or interfere with the metabolism of vitamin K antagonist anticoagulation therapy. Vaccinations to prevent pneumococcal pneumonia and influenza are advised. Given a possible etiological link between tobacco exposure and PVOD, smoking cessation advice and appropriate pharmacotherapy is particularly important and should routinely be offered to cigarette smokers. Diuretics offer symptomatic benefit in those with right ventricular volume overload that is not controlled by dietary measures alone. The effect of diuretics on mortality in PVOD has not been systemically examined(50).

8. Fibromyalgia
The Treatment
B. Treatment in herbal and traditional Chinese medicine
B.3. The Preventive Minerals and Vitamins
1. Minerals for Immunity
Strengthen immunity plays an important role in regulating the free radical and other form of irregular cell growth causes of inflammation and auto immune diseases as well as attenuating the foreigner invasion. According to the study maintaining normal levels of Copper, Selenium and Zinc have found to improve immune function against morbidity, decrease antibody production, infection, impaired natural killing cells, etc......(252) in animal study. Also according to Dr. Kremer JM and Bigaouette J., deficiency of pyridoxine, zinc, copper, and magnesium may induce rheumatoid arthritis expression(253).
1.1. Copper
Epidemiological studies suggested that levels of copper may be influence to the risk of rheumatoid arthritis has produced inconclusive results, it probably due to the disease orgination(254)(255). According to the relevant published data were retrieved through PubMed, Chinese National Knowledge Infrastructure (CNKI), and Chinese Biomedical Database (CBM) before September 20, 2014 of 1444 RA cases and 1241 healthy controls, above normal serum levels of copper (Cu) has found in patients with rheumatoid arthritis(254). Dr Li J and the research team at the joint study lead by Sichuan University, suggested patient with rheumatoid arthritis with B-lymphocyte dysfunction are associated to lower levels of copper, selenium and zinc levels(255). Other study insisted that the alternation of copper concentration may be a result of the changes of the immunoregulatory cytokines(256). Dr Percival SS said,"The immune system requires copper to perform several functions,.....interleukin 2 is reduced and T cell proliferation are reduced in copper deficiency"(257).
1.2. Magnesium
Dr. Minton K. in the study of mutation of Mutation of the magnesium transporter 1 (MAGT1) gene has been found to associate to immunodeficiency due to reduced levels of magnesium(258), inducing severe chronic viral infections and inflammations(259). Deficiency of magnesium is found in patient with RA(253), routine dietary supplementation with multivitamins and trace elements is appropriate in this population(253), Dr. Kremer JM1, Bigaouette J. suggested. The Ankara University study expressed only slight and statistically insignificant reductions of plasma and erythrocyte Mg levels of the RA patients showed when compared to controls, with a positive correlations between erythrocyte SOD activity and Mg level(260).
1.3. Selenium
Selenium, is found to be one of most powerful antioxidant in enhancing immune system in fighting against inflammatory and immune-related diseases, including anti-viral immunity, autoimmunity, sepsis, allergic asthma, and chronic inflammatory disorders mainly through its incorporation into selenoproteins(261). Low serum Se concentration is found to associate in patient with RA(262). But according to the Free University of Brussels, in a in a double blind multi-centric placebo-controlled study showed no clinical benefit selenium treatment on RA but a significant improvement in arm movements and health feeling(263).
1.4. Zinc
Recent study showed that lower than normal serum levels of Zinc (Cu) has found in patients with rheumatoid arthritis(254).
Deficiency of zinc is found to increase immune response through activation of increasing immune cell and inducing pro inflammatory ccytokines IL6(264), especially in aging population(265). who are vulnerable for the aging degenerative chronic diseases of rheumatoid arthritis(266).
1.5. Pyridoxine
Dietary pyridoxine showed to improve immune responses(267) and reduced the expression of pro inflammatory cytokines of that may relate to the early onset of rheomatoid arthritis(268) but the deficiency was not severe enough to affect the immune system, according to the Instituto Nacional de Ciencias Médicas y Nutricion(269). Dr Chiang EP. and the research team of the National Chung-Hsing University said, " Suboptimal vitamin B6 status seen in rheumatoid arthritis can be corrected by 50 mg pyridoxine supplementation for 30 days,.... but (The pyridoxine supplementation) did not suppress pro-inflammatory cytokine production in patients with rheumatoid arthritis"(270).

High anion gap metabolic acidosis is a form of metabolic acidosis characterized by a high anion gap caused producing too much acid or not enough bicarbonate from the body.
Recent study led by the Texas A&M College of Medicine, suggested that dietary alkali, including fruits and vegetables or oral bicarbonate, an intermediate form in the deprotonation of carbonic acid(baking soda)may be effective for treatment of metabolic acidosis in CKD(1). According to Health ABC Study, low serum bicarbonate associated with higher mortality independent of systemic pH and potential confounders and may also contribute tometabolic acidosis
or respiratory alkalosis(2) and the progression of CKD(3).
Dr. Goraya N and colleagues at the Texas A&M College of Medicine, said,"One year of fruits and vegetables or NaHCO3 in individuals with stage 4 CKD,...., improve metabolic acidosis and reduce kidney injury in stage 4 CKD without producing hyperkalemia"(4).
Thiamine or vitamin B1, a vitamin of the B complex found abundantly in yogurt may benefit patients with Metabolic acidosis due to thiamine deficiency(5). According ot he Tokai University Oiso Hospital, thiamine treatment at intravenous doses of 100 mg every 12 h resolved lactic acidosis and improved the clinical condition of 3 patients with metabolic acidosis(6).
Dr. Ozawa H and colleagues at the Tokai University Oiso Hospital, said, " Thiamine deficiency should be included in the differential diagnosis when encountering cases of heart failure with severe metabolic acidosis, even in developed countries"(7).
In the searching for the effective diet for treatment of metabolic acidosis Dr. Adeva MM and Souto G. suggested that dietary with more in fruits and vegetables less in animal meat is associated to recued risk and treatment of metabolic acidosis, including High anion gap Metabolic acidosis(8)(9) such as Apples and Apricots(10).
The combination of Apple, Apricot, Yogurt and Alkaline bicarbonate-rich water smoothie may hold a key in further studies in production of an effective natural ingredients for prevention and treatment of Metabolic acidosis without inducing adverse effects. People who are at increased risk of Metabolic acidosis due to family history, weaken kidney may drink at least 1 serving daily and people with Metabolic acidosis should drink as much as they can, depending to the digestive toleration.
Dietary pattern and life style change are recommended.

The smoothie for reduced symptoms and treatment of metabolic acidosis
Yield: 2 serving (about 8 ounce each)
3/4 cup Apples
3/4 cup Apricots
1/4 cup yogurt
1 cup alkaline bicarbonate-rich water
1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed. Add more green tea drink if needed
3. Serve immediately
The finding the natural ingredients for treatment of vasomotor symptoms is considered as a dream of many scientist to replace the long term usage adverse effect of conventional medicine.
Unfortunately, many compounds found effective in initial studying failed to confirm the potential in large sample size and multi center.

The Prevention
Since patients with are at high risk of hemoptysis, using the below preventive diet and phytochemicals and antioxidant should be taken with care and only with the related field specialists
1. Dietary nitrate
According to the study by the Queen Mary University of London, dietary nitrate (vegetables, fruit, and processed meats) and to a lesser extent dietary nitrite, elicit pulmonary dilatation, prevent pulmonary vascular remodeling, and reduce the right ventricular hypertrophy characteristic of PH. This favorable pharmacodynamic profile depends on endothelial NO synthase and xanthine oxidoreductase -catalyzed reduction of nitrite to NO. Exploitation of this mechanism (ie, dietary nitrate/nitrite supplementation) represents a viable, orally active therapy for PH(40).
2. Fish oil
In the study of the effects of dietary polyunsaturated fats on chronic hypoxicpulmonary hypertension were assessed in rats fed fish oil, corn oil, or a lower fat, “high-carbohydrate” diet (regular) beginning 1 mo before the start of hypoxia (0.4 atm, n = 30 for each), showed that fish oil diet increased lung eicosapentaenoic acid 50-fold and depleted lung arachidonic acid 60% (P less than 0.0001 for each).Lung thromboxane B2 and 6-ketoprostaglandin F1 alpha levels were lower, and platelet aggregation, in response to collagen, was reduced in rats fed fish oil. Chronically hypoxic rats fed fish oil had lower mortality rates than the other hypoxic rats. They also had lower blood viscosity, as well as less right ventricular hypertrophy and less peripheral extension of vascular smooth muscle to intra-acinar pulmonary arteries (P less than 0.05 for each). The mechanism by which dietary fish oil decreases pulmonary hypertension and vascular remodeling during chronic hypoxia remains uncertain(41).
3. Dietary phytoestrogens
In the study totest the hypothesis that phytoestrogenic compounds in the dietcontributed to the female cardioprotectionfour groups of female rats were studied: sham-operated (Sham) and fistula (Fist) rats fed a diet with [P(+)] or without [P(-)] phytoestrogens. Eight weeks postfistula, systolic and diastolic cardiac function was assessed by using a blood-perfused, isolated heart preparation. High-phytoestrogen diet had no effect on body, heart, and lung weights, or cardiac function in Sham rats., showed that hypothesized that phytoestrogenic compounds in the diet contributed to the female cardioprotection(42).
