                           Respiratory Disease

Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing, etc,.

Restrictive lung diseases or restrictive ventilatory defects

Restrictive lung disease is a condition marked most obviously by a reduction in total lung capacity. A restrictive ventilatory defect may be caused by a pulmonary deficit, such as pulmonary fibrosis (abnormally stiff, non-compliant lungs), or by non-pulmonary deficits, including respiratory muscle weakness, paralysis, and deformity or rigidity of the chest wall(1).

                              The Treatment

 In herbal medicine perspective
The aim od herbal medicine is to prevent lung infections and relieve its symptoms. Herbs support the lungs, strengthen the immune system and repair tissues, include
1. Herbs support the lungs
In the study of the effect of astragalus membranceus(fisch) bge, codonpsis pilosula and glycyrrhiza uralensis fisch on airway responsiveness, found that after 28 asthmatic patients received the treatment of those herbs, the results showed that FVC value increased obviously (t = 2.217, P < 0.05) and the value of FEV1 and PEF was all higher compared with before treatment of the herbs (P < 0.01)(33).
2. Herbs enhance immunity
In the study of the effect of Chinese herbs (Astragalus radix and Ganoderma lucidum) on immune response of carp in fish fed diets containing Astragalus (0.5%), Ganoderma (0.5%) and combination of two herbs (Astragalus 0.5% and Ganoderma 0.5%) for 5 weeks, showed that feeding non-vaccinated and vaccinated carp with combination of Astragalus and Ganoderma stimulated respiratory burst activity, phagocytosis of phagocytic cells in blood and lysozyme and circulatory antibody titres in plasma in vaccinated carp. Fish challenged with A. hydrophila had variable survival. The best survival (60%) was in vaccinated group fed with both herbs, while almost 90% of control fish (negative control) and 60% of fish vaccinated only (positive control) died(34).
3. Herbs repair tissues
According to the study by Ohio State University’s Heart and Lung Research Institute, Grape-seed extractseemed to aid wound healing in two ways: It helped the body make more of a compound used to regenerate damaged blood vessels, and it also increased the amount of free radicals in the wound site. Free radicals help clear potentially pathogenic bacteria from a wound(35).

Musculoskeletal disorders (MSDs) are medical condition mostly caused by work related occupations and working environment, affecting patients’ muscles, joints, tendons, ligaments and nerves and developing over time. A community sample of 73 females and 32 males aged 85 and over underwent a standardised examination at home. Musculoskeletal pain was reported by 57% of those interviewed. A major restriction of joint movement range was frequent in the shoulder but uncommon in other joints(1).

                                                      Rheumatoid Arthritis

Rheumatoid Arthritis is defined a chronic disorder as a result of inflammation, affecting mostly the flexible (synovial) joints and tissues and organs in the body. The disease affects more women than men and generally occurs after the ages of 40, causing diminished quality of life of many elders(1). According to CDC, Musculoskeletal disorders (MSDs) affects over 52 millions of adults in the US alone, including 294,000 children under age 18 with some form of arthritis or rheumatic conditions(2). Rheumatoid Arthritis can induced bone loss through elevating bone resorption without increasing bone formation(4). A cross-sectional population-based study of 1042 patients with rheumatoid arthritis showed that RA patients had an increased risk of death from various causes(4a).

6. Tai Chi
Tai chi, is an ancient form of mind-body technique used in Chinese with meditative movements that promote balance and healing of the mind and body(406, 331). But to use the technique for treatment of RA has produced conflict result.
According to the review of 4 trials including 206 participants, tai chi showed a statistically significant and clinically important improvements in ankle plantar flexion, benefits on lower extremity range of motion, in particular ankle range of motion, for people with RA but had no alternation of outcomes of disease activity(332). Contract to the study, Dr Lee MS and the research team at the Universities of Exeter & Plymouth, said, " Tai chi reported some positive findings for tai chi on disability index, quality of life, depression and mood for RA patients, (but) did not demonstrate effectiveness on pain reduction(333).
Other study in the assessment of patient with RA suggested that tai chi should be sued for treatment of people with a chronic debilitating disease such as RA due to its potential for physical and psychological improvements(334).

Focal segmental glomerulosclerosis is scar tissues in the filtering unit of the kidney due to primary glomerular diseases in children and adolescents inducing edema and the loss of protein from the plasma into the urine. The syndrome may result of kidney failure when affected children and adolescents  grow into adulthood. According to Dr. Fogo AB, Focal segmental glomerulosclerosis (FSGS) has a common lesion in progressive kidney disease, and a disease characterized by marked proteinuria and podocyte injury(*).

The smoothie for prevention and treatment of proteinuria(albuminuria)
Yield: 2 servings (about 8 ounces each)
1 1/4 cups tomato
1/2 cup broccoli
1 cup organic orange juice

1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately

The finding of a natural source for treatment of Albuminuria  has been running into many obstacles, many ingredients showed initially with promising result in animal studies have not produced same potentials in large sample size and mutli centers human trials.

Recent studies back by well known institutions proposed, Cooked Tomato, broccoli and Orange Juice may be the next generation of natural ingredients for prevention and treatment of Albuminuria due to kidney decline

Albuminuria is a medical condition with too much protein in the urine, found
typically inpatients with kidney diseases. According to the Brigham and Women's Hwayospital, higher derivative vitamin A, beta-carotene, intake may reduce risk for Glomerular Filtration Rate (GFR) decline(1).

Lycopene, a chemically a carotene, also exhibited anti nephrotoxicity through anti proinflammatory pathways(3),

Coenzyme Q10, antioxidant found abundantly in broccoli may prevent or minimize cyclosporine nephrotoxicity through its antioxidant effect(4).
Dr. Ishikawa A and Dr. Homma Y. professors a the the University of Tokyo said, "Ubiquinol, the reduced form of coenzyme Q10 ......an antioxidants, may have potential for preventing CyA nephrotoxicity"(5).
In type 2 diabetes model, intake of supplement of coenzyme Q10 exhibited anti renal mitochondrial dysfunction caused by diabetic induced nephropathy(6).

Free radical scavengers vitamin C (Orange)  intake, increased plasma and renal cortical vitamin C or E content in prevention of renal injury(7) and development of diabetic nephropathy(9).
According to the Yonsei University Wonju College of Medicine, vitamin C exhibited anti oxidative stress in ameliorated albuminuria and renal sclerosis in experimental diabetic rats(8).

The effectiveness of Cooked Tomato, broccoli and Orange Juice may serve as cornerstones of pharmaceutical target for further studies in production of a potential medication for reduced risk, complications and treatment of Albuminuria with little or no adverse effects.

People who are at high risk of Albuminuria due to aging, weaken reno function... should drink at least one serving daily and women with Albuminuria should drink no more than 4 servings daily, depending to digestive toleration.
Life style and diet pattern change are necessary as overweight and obesity are well-established risk factors for cardiovascular disease and decline in kidney function(2)

                           Respiratory Disease

Respiratory Disease is defined as medical conditions, affecting the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing, etc,.

                                         Upper respiratory tract infection

Upper respiratory tract infections are considered to be the infection of the airway above the glottis or vocal cords. This includes the nose, sinuses, pharynx, and larynx, including the infection of tonsillitis, pharyngitis, laryngitis, sinusitis, otitis media, etc,.

                             The Symptoms

The symptoms of include nasal congestion, a runny nose, sneezing, pharyngitis/tonsillopharyngitis, headache, muscle aches (usually severe) sore throat, watery eyes, etc,. Influenza infections may result in different clinical presentations. In the study to determine the clinical differences between circulating influenza strains in a young healthy adult population in the tropics, found that there were no statistical differences between H3N2 and influenza B (p = 0.58). Those with nasal congestion, rash, eye symptoms, injected pharynx or fever were more likely to have H3N2; and those with sore throat, fever, injected pharynx or rhinorrhoea were more likely to have influenza B than H1N1-2009(1). Others in the study of the differing symptom patterns in early pandemic vs seasonal influenza infections, conducted by Department of Laboratory Medicine, National University Hospital, Singapore, found that from the week of June 16 to June 23, 2009, this pandemic influenza A(H1N1/2009) displaced and then replaced the seasonal influenzas (H3N2, H1N1, and B). Of 2683 samples tested during this 12-week surveillance period, 742 (27.6%) were positive for any influenza virus using this assay, with 547 cases of A(H1N1/2009) (20.4%), 167 cases of A(H3N2) (6.2%), 14 cases of A(H1N1) (0.5%), and 12 cases of influenza B (0.4%). Results of multivariate analysis showed that age (P < .001), fever (P < .001), cough (P < .001), sore throat (P = .002), rhinorrhea (P = .001), and dyspnea (P < .001) were significantly different among these groups.(2). According to the study of the symptoms of the common cold and influenza conducted by Cardiff School of Biosciences, Cardiff University, indicated that the effects of cytokines in human beings now helps to explain some of the symptoms of colds and flu that were previously in the realm of folklore rather than medicine-eg, fever, anorexia, malaise, chilliness, headache, and muscle aches and pains. The mechanisms of symptoms of sore throat, rhinorrhoea, sneezing, nasal congestion, cough, watery eyes, and sinus pain(3).

The smoothie at least prolong and prevention of Contrast-induced nephropathy
Yield: 2 servings (about 8 ounces each)
1 1/2 cups cooked sweet potato
1 1/2 grams baking soda
1 cup of organic orange juice

1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately

The finding of a natural source for treatment of Contrast-induced nephropathy has been running into many obstacles, many ingredients showed initially with promising result in animal studies have not produced same potentials in large sample size and mutli centers human trials.

Recent studies back by well known institutions proposed, Sodium bicarbonate and Juar tea may be the next generation of natural ingredients for prevention and treatment of Skeletal Abnormalities in Conorenal syndrome.

Contrast-induced nephropathy is  meducal condition of reno dysfunction with either a greater than 25% increase of serum creatinine or an absolute increase in serum creatinine of 0.5 mg/dL increase in absolute value, within 48-72 hours of intravenous contrast administration, according to Medscape(1) caused by acute kidney injure.

Sodium bicarbonate or baking soda, bread soda, cooking soda may be used for prevention and treatment of patients acute kidney injure induced Contrast-induced nephropathy(CIN)(2). According to the National University Health System, Singapore, in a multi-centre, randomised, controlled trial, suggested that 12-hour sustained sodium chloride pre-hydration regimen was more protective than the 1-hour abbreviated SOB regimen in patients with Contrast-induced nephropathy(CIN)(3).
Dr. Navaneethan SD and colleagues at the Cleveland Clinic said, "Hydration with sodium bicarbonate decreases the incidence of contrast-induced nephropathy in comparison to hydration with normal saline without a significant difference in need for renal replacement therapy and in-hospital mortality"(4).

Antioxidant including vitamin C and E plays important role in reduced risk and treatment of patients of contrast-induced nephropathy caused by acute kidney injure or chronic kidney disease, according to the Shahid Sadoughi University of Medical Sciences(6).
Vitamin C, a water soluble vitamin found abundantly in orange in a study in comparison of
N-acetylcysteine and/or ascorbic acid versus placebo, showed that high doses of NAC plus hydration provide better protection against CIN than combination therapy of NAC and ascorbic acid plus hydration, or hydration alone(7).
Dr. Sadat U and colleagues at the Cambridge University Hospitals NHS Foundation Trust, in the review of 9 randomized, controlled trials said,"Ascorbic acid provides effective nephroprotection against CI-AKI and may form a part of effective prophylactic pharmacological regimens"(8).

Vitamin E, is a fat soluble antioxidant found abundantly in sweet potato may be effective in combination with 0.9% saline against CI-AKI in CKD patients(5).
In animal model, alpha tocopherol showed protective effect on renal damage induced CIN,through significant increase of renal SOD, reduction in MDA and an increase of TAC in serum(9).
Furthermore, Dr, Tasanarong A and professors at the Thammasat University said, "Prophylactic oral administration of alpha tocopherol is capable of protecting against CIN in patients with chronic kidney disease undergoing elective coronary procedures"(10).

The effectiveness of Baking Soda, Sweet Potato and Orange Juice may serve as cornerstones of pharmaceutical target for further studies in production of a potential medication for reduced risk, complications and treatment of Contrast-induced nephropathy with little or no adverse effects.

People who are at high risk of Contrast-induced nephropathy due to kidney disease should drink no more than 4 servings daily, depending to digestive toleration.
Life style and diet pattern change are necessary.

In traditional Chinese medicine perspective
Pulmonary fibrosis (PF) is a restrictive lung disease that may occur idiopathically or as a complication of many diseases. In the study to test a new Chinese medicine formula DSQRL for the treatment of experimental PF in comparison with prednisone, conducted by Hong Kong Baptist University, Hong Kong, showed that at the end of 30 days treatment, the DSQRL treatment achieved a better outcome (p<0.05) than prednisone in terms of histological examination, bronchoalveolar lavage fluid analysis, hydroxyproline assay and complications(36). In other study by the same university to compare the Chinese medicine formula DSQRL from glucocorticoids, indicated that at the end of 60 days treatment, the DSQRL treatment achieved a significantly better outcome than prednisolone in terms of general behavior, histological examination, hydroxyproline content of the lung and inflammatory cell counts in bronchoalveolar lavage fluid(37)
