                         Thyroid disease

Thyroid disease is defined as a condition of malfunction of thyroid. Hyperthyroidism is a condition in which the thyroid gland is over active and produces too much thyroid hormones.

Euthyroid sick syndrome as a result of Sepsis

Sepsis is defined as a condition caused by chemicals released into the bloodstream to fight the infection trigger inflammation throughout the body as a result of severe infection(a)(b). according to the study by the University of Utah, sepsis is the commonest cause of admission to medical ICUs across the world. Mortality from sepsis continues to be high. Besides shock and multi-organ dysfunction occurring following the intense inflammatory reaction to sepsis, complications arising from sepsis-related immunoparalysis contribute to the morbidity and mortality from sepsis(c).

                                              The  Treatments

Treatments in conventional medicine perspective
1. Antibiotics
Most case of sepsis are treated with combination of two or three antibiotics given at the same time.
High doses of vancomycin were administered in order to rescue patients from septic shock. Plasma drug concentration dropped while clinical condition of patients worsened. Conversely, drug levels increased spontaneously once the infection was reverted. The theoretical model provided greater insight into pharmacokinetic features related with the use of vancomycin in septic patients(58). Other study indicated that patients with sepsis do not seem to have the same level of impairment of tissue distribution as described for patients with septic shock. A 25% lower dose of piperacillin administered by continuous infusion seems to maintain higher trough concentrations compared with standard bolus dosing. It is likely that the clinical advantages of continuous infusion are most likely to be evident when treating pathogens with high minimum inhibitory concentration, although without therapeutic drug monitoring and subsequent dose adjustment, infusions may never achieve target concentrations of organisms with very high minimum inhibitory concentrations in a small number of patients(59).

2. IV Fluids
In the study to determine the clinical and pharmacokinetic differences between continuous and intermittent dosing in patients with severe sepsis, found that continuous administration of beta-lactam antibiotics achieved higher plasma antibiotic concentrations than intermittent administration with improvement in clinical cure. This study provides a strong rationale for further multicenter trials with sufficient power to identify differences in patient-centered endpoints(60). Other study suggested that routine use of prolonged infusion of time-dependentantibiotics for the empiric treatment of gram-negative bacterial infections offers no advantage over intermittent infusion antibiotic therapy with regard to treatment success, mortality, or hospital length of stay. These results were confirmed after controlling for potential confounders in a multivariate analysis(61).

Obesity is a medical condition of excess body fat accumulated overtime, while overweight is a condition of excess body weight relatively to the height. According to the Body Mass Index(BMI), a BMI between 25 to 29.9 is considered over weight, while a BMI of over 30 is an indication of obesity. According to the statistic, 68% of American population are either overweight or obese.

You can calculate your BMI index BMI= weight (kg)/ height (m2)

Polycystic Ovarian Syndrome is defined as endocrinologic diseases caused by undeveloped follicles clumping on the ovaries that interferes with the function of the normal ovaries as resulting of enlarged ovaries, leading to hormone imbalance( excessive androgen), resulting in male pattern hair development, acne, irregular period or absence of period, weight gain and effecting fertility. It effects over 5% of women population or 1 in 20 women.

The Studies of Atherogenic changes in low-density lipoprotein of non-obese women with polycystic ovary syndrome

In a case-control study of the evaluating complete lipid and lipoprotein profiles in 64 Non PCOS patients and 64 age- and BMI-matched controls. All women with PCOS in our study population were not obese and to determine the differences in the LDL particle profiles between PCOS phenotypes, the patients with PCOS were divided into two subgroups according to the presence of clinical or biochemical hyperandrogenism, posted in PubMed, indicated that non-obese women with PCOS have no significant quantitative or qualitative changes in LDL-C profile, data on obese Korean women with PCOS could offer complementary findings about the possible relationship between the magnitude of obesity and LDL phenotype.

           Attention Deficit Disorder (ADHD/ADD)

Attention Deficit Disorder (ADHD/ADD) also known as attention deficit hyperactivity disorder is a chronic disease occurs in certain age and years of a children, defined as a condition with characteristics of co-existence of attentional problems and hyperactivity. It effects about 5% of the all children in the world with boys have 3-4 times higher risk than girl. Over one third of children with ADHD will have continued symptoms existed into their adult life.

The Herbal medicine  - Panax ginseng

Herbal medicine believe ADHD is a result of coordination failure in the reticular activating system of the brain. Therefore, herbs used to treat ADHD are those improves concentration, attention, motivation and performance, promote the stay on task and normal energy levels and particular enhance the flow of oxygen and nutrients to the brain and supports brain functions in information processing and memory, etc.
a. Antioxidant activity
In the evaluation of the extraction conditions of polysaccharides from the rhizomes of Panax japonicus C.A. Meyer and its antioxidant effect found that antioxidant activity exhibited Panax japonicus polysaccharides (PJP) had a good potential for antioxidant, according to "Optimization of polysaccharides from Panaxjaponicus C.A. Meyer by RSM and its anti-oxidant activity" by Wang R, Chen P, Jia F, Tang J, Ma F.(70)

b. Antipsychotic effect
In the investigation of Panax quinquefolium (PQ) and its significant neuroactive properties for its antipsychotic potential found that PQ blocked ketamine induced memory impairment in the passive avoidance paradigm. In the chronic studies, PQ reduced the ketamine induced enhanced immobility in the forced swim test and did not show extra-pyramidal side effects in bar test and wood block test of catalepsy. These behavioural effects were compared with standard drugs haloperidol and clozapine. Further PQ reduced DA and 5-HT content after chronic treatment, but not after acute administration, according to "Evaluation of the Antipsychotic Potential of Panax quinquefolium in Ketamine Induced Experimental Psychosis Model in Mice" by Chatterjee M, Singh S, Kumari R, Verma AK, Palit G.(71)

c. Neuroprotective effect
In the analyzing Panax ginseng C.A. Meyer and its beneficial effects in cerebral ischemia and inhibition of the inflammatory cascade in sepsis found that Ginsenoside Rb1 (GRb1) partially inhibited the activation of nuclear factor-κB (NF-κB) pathway from 6 h to 72 h after ischemia and reperfusion onset, as determined by the expression of total and phosphorylated NF-κB/p65, inhibitor protein of κB (IκB)-α, and IκB-kinase complex (IKK)-α. All these results indicate that suppression of local inflammation after cerebral ischemia might be one mechanism that contributes to the neuroprotection of GRb1, according to "Suppression of local inflammation contributes to the neuroprotective effect of ginsenoside Rb1 in rats with cerebral ischemia" by Zhu J, Jiang Y, Wu L, Lu T, Xu G, Liu X.(72)

d. Immunological activities
In the study of Water-soluble ginseng oligosaccharides (designated as WGOS) with a degree of polymerization ranging from 2 to 10 were obtained from warm-water extract of Panax ginseng roots, found that WGOS were potent B and T-cell stimulators and WGOS-1 has the highest immunostimulating effect on lymphocyte proliferation among those purified fractions. It is hoped that the WGOS will be developed into functional food or medicine, according to "Structural characterization and immunological activities of the water-soluble oligosaccharides isolated from the Panax ginseng roots" by Wan D, Jiao L, Yang H, Liu S.(73)

Biochemical Analysisof  Male Infertility In Conventional medicine Perspective

I. Definition
The study of the chemical substances and vital processes occurring in male testicle such as the concentration of white blood cells, the level of fructose in the semen, and the volume, pH, and liquefaction time of the ejaculate.

II. Procedure
The semen sample of sperm is taken at the clinic laboratory through masturbation, the the semen is sent for analysis for chemical substances and vital processes as mentioned in the definition.

III. Diagnosis
This test a semen analysis typically for
1. Volume
The semen sample required for this test is no less than 2.0 ml or more
It measures the number of sperm per millilitre ( ml ) of ejaculate
2. pH
pH is a measure of the acidity or basicity of a solution ( PH of 07 is considered as neutral). In this test, it is requires in the range of 7.2-8.0 to be considered as normal.

3. Sperm concentrationIt measures the number of sperm per millilitre of ejaculate, for a normal sperm concentration it requires 20,000,000 per ml or more.

4. Motility
Motility is a measure of sperm for movement, the normal sperm motility is 50% or more with forward progression. It can be classified in 4 grade
a) Sperm with progressive motility:
These are the strongest and swim fast in a straight line
b) Non-linear motility: These also move forward but tend to travel in a curved or crooked motion.
c) Non-progressive motility:
Sperm not move forward despite the fact that they move their tails.
d) Non motility:
Fail to move at all.

5. Rapid forward progressive motility
Sample sperm requires to have at least 25% or more of this types of sperm to be considered as normal.

6. Morphology
It is the study of normal and abnormal form of the sperm. It considered normal if it isa) 30% or more normal forms (WHO criteria)
b) 11% or more normal forms (Tygerberg strict criteria developed by Dr. Roelof Menkveld, Tygerberg Hospital, South Africa, and disseminated by Dr. Thinus Kruger )

7. Vitality The sample should have least 75% live sperm in a semen sample
8. White blood cells
The sample must have a white blood cells of less than 1,000,000 per ml. Otherwise, it may be an indication of immunity causes of infertility and more testes are required.

Sometime the semen sample may also be tested for vesicular function, citrate or acid phosphatase for the prostate gland and free carnitine as an index of epididymal function, if it is requested by your fertilization specialist.
