Dietary Patterns in the Risk of Metabolic Syndrome

A dietary pattern is the measurement of the quantity, variety, or combination of different foods and beverage in a diet as well as the frequency of the foods are habitually consumed.

Most common dietary patterns are

* The traditional diet, Wikipedia wrote, "Traditional foods are foods and dishes that are passed through generations or which have been consumed many generations. Traditional foods and dishes are traditional in nature, and may have a historic precedent in a national dish, regional cuisine or local cuisine".

* The Western diet is a standard American diet (SAD) associated with high intakes of red meat, processed meat, pre-packaged and refined foods, fried foods, high-fat dairy products, high salt, and sugar.foods.

Metabolic syndrome is a cluster of conditions associated with the increased risk of cardiovascular disease and diabetes.

The health problems found in the group include too much fat around the waist, elevated blood pressure, high triglycerides, elevated blood sugar, and low HDL cholesterol.

In other words, if you have 3 of the conditions in the group, your risk of CVD and diabetes are increased substantially compared to those who have less.

According to the statistics, approximately 30% of people in the US are living with metabolic syndrome.

Believe it or not, among US adults aged 18 years or older, the prevalence of metabolic syndrome rose by more than 35% from 1988–1994 to 2007–2012, increasing from 25.3% to 34.2%.

Besides the aforementioned conditions, risks of metabolic syndrome may include age, family history, physical, inactivity, and women with polycystic ovary syndrome, epidemiologically.

As of today, treatment of metabolic syndrome commonly is prescribed medications to reduce blood pressure, cholesterol, and/or blood sugar with an aim to reduce the risk of developing further health complications and improve quality of life.

On finding a potent ingredient for the treatment of metabolic syndrome, researchers investigated the association between major dietary patterns, including several eating habits.

The cross-sectional study included a total of 425 subjects 35 to 55 y of age, using a food-frequency questionnaire in 5 dietary patterns.

Five major dietary patterns were examined by the Isfahan University of Medical Sciences, including

* A western pattern (high in sweets, butter, soda, mayonnaise, sugar, cookies, the tail of a lamb, hydrogenated fat, and eggs),

* A prudent pattern (high in fish, peas, honey, nuts, juice, dry fruits, vegetable oil, liver and organic meat, and coconuts and low in hydrogenated fat and non-leafy vegetables),

* A vegetarian pattern (high in potatoes, legumes, fruits rich in vitamin C, rice, green leafy vegetables, and fruits rich in vitamin A),

* A high-fat dairy pattern (high in high-fat yogurt and high-fat milk and low in low-fat yogurt, peas, and bread), and

* A chicken and plant pattern (high in chicken, fruits rich in vitamin A, green leafy vegetables, and mayonnaise and low in the beef, liver, and organic meat).

Based on the tested differentiation and adjusting to other confounders,

* The western pattern has a strong and negative impact in increasing triacylglycerol and blood pressure.

* The prudent pattern was positively associated a low high-density lipoprotein cholesterol levels comapred to other dietary patterns.

* However, the vegetarian dietary pattern was inversely associated with a risk of an abnormal fasting blood glucose level.

Collectively, all dietary patterns are associated with positive or negative in increasing or decreasing the risk of metabolic syndrome.

Chaste Tree Berry, the Functional Food for Treatment of Type I and II Diabetes

Chaste tree berry may have a potential in improved pancreatic islet regeneration in reduced risk of development and treatment of Type I and II diabetes, the renowned institute study postulated.

Diabetes is condition caused by insufficient insulin entering the bloodstream in regulation of glucose. The disease is either caused by cells in the pancreas dying off or receptor sites clogged up by fat and cholesterol. In some cases, diabetes is also caused by an allergic reaction of cells in the immune system.

Chaste tree berry is a species of Vitex agnus-castus, genus Vitex, belonging to the family Lamiaceae, native to the Mediterranean region, used in herbal medicine for thousands of years as an aphrodisiac herb and considered as Queen herb in treating menstrual problems and discomforts.

According to the study, diabetes rats induced by streptozotocin treated with extract of chaste tree berry displayed a significant effect in increased volume density of pancreatic islet as well as percentage of β-cells and size of the islet.

Dr. Oche O, the lead researcher said, "plant extracts, (exhibited) regeneration of β-cells along with β-cells repairs, in compared to non-treated diabetic control which showed complete degeneration of the islet cells".

Furthermore, the study also found that the levels of total bilirubin, direct bilirubin, and unconjugated bilirubin which are associated with diabetes were significantly lower in the treated group in comparison to control.

More important, in D-galactose induces pancreatic disorder along in the aging mouse model, treatment of Vitex agnus-castus (VAC) demonstrated potential pancreatic protective and hypoglycemic effects through improved the size of pancreatic islets decreased in aged and D-galactose groups.

In a total of 72 adult female Naval Medical Research Institute (NMRI) mice (weighing 30-35 g) divided into 6 groups of control, VAC hydroalcoholic extract, D-galactose, D-galactose + VAC hydroalcoholic extract, aged, aged + VAC hydroalcoholic extract through subcutaneous injection of D-galactose for 45 days and, VAC hydroalcoholic extract gavaged twice a day in the last 7 days. 24 h after the last drug and extract administrations, group treated with Chaste tree berry extract or combinations of the extract showed increased insulin and decreased serum glucose and in D-galactose level compared to other treatment groups.

The study also found, "Despite the size of pancreatic islets decreased in aged and D-galactose groups, ........... VAC hydroalcoholic extract has hypoglycemic and pancreatic protective effects in natural aged and aging model mice".

In fact, there is also a report indicated that oral administration of once-daily to diabetic rats for the period of 30 days of iridoid glucoside isolated from chaste tree berry may be as effective as glibenclamide, a known hypoglycemic drug, at a concentration of 50 mg/kg b.w.
Promisingly, iridoid glucoside reversed hyperglycemia-induced biochemical changes to near-normal levels.

Collectively, Chaste tree berry may be used as a functional food for the treatment of Type I and Type II diabetes through the hypoglycemic and pancreatic protective effects.

Herbal Cinnamon, The Natural Alternative for Treatment of Infertility, Scientists Say

Scientists may have found a kitchen spice which processes some underline mechanisms in enhancing the rate of fertility, some studies suggested.

Infertility is a condition characterized by the inability of a couple to conceive after 12 months of unprotected sexual intercourse or the cannot carry the pregnancy full term.

The syndrome affects over 5 million couples alone in the US. Because of the unawareness of treatments, only 10% seek help from professionals.

According to the statistic, about 35% of infertility is caused by the male's inability to fertilize. 35% is caused by the female's inability to conceive, 10% attributes to both, and 10 % is considered a failure with an unknown cause.

Dr, Romero Ramos R, in the study [Risk factors associated to female infertility], wrote, "There were 6 factors with statistical significance: advanced age (p < 0.001), elevated body mass index (p < 0.001), age of onset of sexual activity (p < 0.001), prior pelvic surgeries (p < 0.001), and presence of stress (p < 0.001). Other risk factors such as smoking, chemical, and radiological treatments, pelvic inflammatory disease, exercise, contraceptive use, alcohol intake, drugs, coffee, solvents, glue, and insecticides, were not significant".

In male infertility, according to a case-control study conducted by Dr. Buiatti E and colleagues, about coffee and alcohol consumption, smoking habits, x-ray exposure, usual sitting posture, drug consumption, other nonoccupational risk factors, socioeconomic status, education level, and occupational history are some of the prevalent risk factors associated to male infertility.

And, the high relative risk was associated with (a) occupation in the radioelectric industry, (b) non-sedentary clerical workers, (c) clerical work in the typographic industry, and (d) occupation in the textile industry,

Cinnamon, a kitchen spice in many cultures, may be best known for its effectiveness in reducing the levels of blood glucose in diabetics and lipid peroxidation through its antioxidant activity(4).

In a total of 45 women with Polycystic Ovary Syndrome, randomized, 26 women completed 3 months of the study, and 17 women completed the entire 6 months of the study, researchers showed,
* Application of cinnamon exerts a positive effect in the improvement of menstrual cyclicity associated with luteal phase progesterone levels and ovulatory menses without altering insulin resistance or serum androgen levels

* Cinnamaldehyde, a major chemical compound of cinnamon enhances fertility through selectively induced progesterone production and inhibits the production of testosterone and dehydroepiandrosterone in human adrenal cells.

* Further differentiation, suggested that cinnamon extract not only reduces insulin resistance in vitro and in vivo by increasing phosphatidylinositol 3-kinase activity with insulin stimulation in the insulin signaling pathway but also improves menstrual cyclicity in female induced PCOs.

Moreover, in the infertile male rat study conducted by Fırat University, long-term cinnamon bark oil(CPO) improved the effect on testicular oxidant-antioxidant balance and sperm quality with significantly decreasing the abnormal sperm rate and apoptotic germ cell count.

Interestingly, in the study to investigate the likelihood of detrimental effects of carbon tetrachloride (CCl4 ) on the male reproductive system through oxidative stress mechanism and also protective effects of cinnamon bark oil (CBO), researchers at the Firat University conducted an experiment including 28 healthy male Wistar rats.

The tested subjects were divided into four groups, seven rats in each. Group 1 received only olive oil daily; group 2 was treated with 100 mg kg(-1) CBO daily; group 3 was treated with only 0.25 ml kg(-1) CCl4 weekly, and group 4 received weekly CCl4 + daily CBO.

All administrations were made by the intragastric catheter and maintained for 10 weeks.

At the final report, researchers suggested that
* CCl4 administration caused significant decreases in body and reproductive organ weights, testicular catalase (CAT) activity, sperm motility and concentration, and significant increases in lipid peroxidation (LPO) level, abnormal sperm rate and apoptotic index along with some histopathological damages compared with the control group.

* Pretreatment of CBO following the administration of CCl4 significant improvements in absolute weights of testis and epididymis, all sperm quality parameters, LPO level, apoptotic index, and testicular histopathological structure Cl4, compared to the group given CCl4 only.

Taken altogether, Cinnamon may be effective in promoting fertility in both sexes through enhancing the menstrual cycle in women and improving sperm performance in men.

Cranberry Protects Your Gum Against Periodontitis

Periodontitis is a serious case of gingivitis associated with gum infection that damages the soft tissue and destroys the bone that supports your teeth as a result of long-term poor oral hygiene

Periodontitis is preventable if the progression of gingivitis stages is stopped

Gingivitis is a common and mild form of periodontal disease caused by inflammation of the gingiva, leading to symptoms of irritation, redness, and swelling of the gum that lines the base of the teeth.

Gingivitis dental plaques formed around the base of the teeth are sticky biofilm of bacteria associated with the interaction of the food intake and bacteria in the mouth. Over time, if untreated, the condition can lead to periodontitis and advanced periodontitis.

At this early stage, the bone and connective tissue that hold the teeth in place are not yet affected. The problems can be reversed by more frequent and proper brushing and flossing.

Epidemiologically, risk factors induce the onset of gingivitis are smoking or chewing tobacco. diabetes and long-term use of certain medication including oral contraceptives, steroids, anticonvulsants, calcium channel blockers.

Some people under the treatment of chemotherapy may also experience symptoms of gingivitis.

The Colgate Professional in the examined the symptoms advised that if you have experienced some of the symptoms that do not heal themselves in a short time, such as puffy or swollen, or tender, bleeding during brushing or flossing,... and constant bad breath or a bad taste in your mouth, you may have gingivitis.

Cranberry is an evergreen dwarf shrub, genus Vaccinium, belongings to the family Ericaceae, native to Northern America and Southern Asia. Because of its health benefits, cranberry has been cultivated in some parts of the world for commercial profit and used in traditional and herbal medicine to treat wounds, urinary disorders, diarrhea, diabetes, stomach ailments, and liver problems.

On finding a potential compound for the treatment of periodontitis, researchers examined the effect of cranberry and rice extracts as a potent gingipain inhibitor.

Based on the tested analysis, cranberry and rice extracts interfere with gingipain activity that causes the slow down the progression of adult periodontitis.

Furthermore, the extracts prevented the growth and biofilm formation of periodontal pathogens induced the onset of periodontitis.

Moreover, the extracts also inhibited a broad spectrum of host proteases in the induction of undesirable side effects

As of today, the ideal gingipain inhibitor has yet to be discovered, however, cranberry and rice extracts may be a promising gingipain inhibitor comapred to gingipain N-terminal pro domains, synthetic compounds, antibiotics, antiseptics, antibodies, and bacteria which may induce undesirable side effects

The results strongly suggested that cranberry promotes oral health by inhibiting the onset and progression of periodontitis through its anti-bacterial activity

Taken altogether, cranberry may be considered a remedy for the prevention and treatment of periodontitis, pending to the confirmation of the larger sample size and multicenter human study.

Bromelain Speeds Up Healing of Ecchymosis

Ecchymosis is a discoloration of the skin caused by the bleeding underneath the skin, typically in bruising.

Most cases of ecchymosis are results in which the blood vessels near the surface of the skin burst, particularly in the impact of an injury.

In other words, ecchymosis is associated with blood vessel leaks, and the blood is accumulated under the skin.

The discoloring of the skin is also involved in the clotting factors, that stop the bleeding so that the tissue starts healing.

The progression of ecchymosis is gone through a certain stage from red or purple to black or blue
to brown and to yellow.

Certain medications have been found to induce ecchymosis including:
* Blood thinners, such as aspirin or warfarin

*Antibiotics such as cephalexin, cefaclor, and cefuroxime-axetil can cause ecchymosis by inducing low platelet count

* Corticosteroids

According to the study from the University College Hospital, easy bruising tended to be older (61 vs 52 yrs), on higher daily dosages (1,388 vs 1,067 micrograms), particularly in the long term inhaled corticosteroids users.

Most cases of ecchymosis usually heal on their own within two to three weeks. Most common medications used for the treatment of ecchymosis are pain relievers.

Bromelain, a proteolytic enzyme found in pineapples (Ananas comosus) has been used in traditional medicine as an inflammatory agent and to treat pains, strains, and muscle aches and pains and ease back pain and chronic joint pain, skin diseases, etc.

With an aim to find a potential compound for the treatment of ecchymosis, researchers examined the effects of bromelain on ecchymosis postprocedure unwanted effects.

In the reconfirm the effect of bromelain used internationally by physicians on limit ecchymosis secondary to cosmetic, laser, and surgical skin procedures, researchers reviewed the published literature and provide evidence-based recommendations on bromelain for prevention and treatment of postprocedure ecchymosis.

The search on the computerized bibliographic databases Medline, EMBASE, Scopus, and CINAHL on March 23, 2015, identified 7 clinical trials that study the effect of oral bromelain in the prevention and/or treatment of postprocedure ecchymosis compared to no studies that use topical bromelain for the treatment of ecchymosis.

Based on the results, 5 out of 7 randomized controlled trials reported improvement postprocedure ecchymosis in the bromelain-treated group.

Taken altogether, bromelain may be considered a remedy for the treatment of postprocedure ecchymosis, pending to the confirmation of the larger sample size and multicenter human study.

Intake of bromelain in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.

All participants were randomly divided into two groups of 34 students each (the two groups used combination of fennel extract/vitamin E and ibuprofen cross-over form in the 2 months).

Application of the combination displayed a mean of peak pain intensity in the first, second, third, sixth, and forty-eight hours in the group was significantly lower than the group that had used ibuprofen.

Also, a combination of fennel extract/vitamin E was more effective than ibuprofen in the first hour (P < 0.03) and second hour (P < 0.04), according to the Visual Analogue Scale (VAS) tool for ranking the intensity of the pain.

These results suggested that fennel and vitamin E combination was more efficacy in blocking the neuron in transmitting pain caused by overexpression prostaglandin hormone in the induction of abdominal muscle constriction in comparison to conventional medicine ibuprofen.

Dr. Nasehi M, the lead author, after taking into account other co and confounders said, "Combination of fennel extract/vitamin E is effective on decreasing the intensity of pain of primary dysmenorrhea, and it is advised to those who cannot use chemical drugs".

In a separate study to investigate the effect of fennel used alone on primary dysmenorrhea symptoms and menorrhea duration, 80 female students satisfied the criteria and guidelines were randomly divided into two groups of intervention (n = 40) and control (n = 40).

The intervention group was given one soft capsule Fennel (30 mg) every 4 hours, 3 days before menstruation till the 5th day, and continued for 3 months. The control group received no medication.

According to the result of the experiments, the fennel treated group exerted a significant decrease of the mean of nausea intensity and weakness from 1.93, and 2.88 after 3 months, in comparison to the control group from 2.37, 6.65 in the control group.

Also in compared to the control group, the fennel group reduced the duration of the menstrual period after two and three months of use.

Moreover, Fennel treated group demonstrated a strong improvement of quality and feelings of well-being after 1 and 3 months.

Taken together, fennel may be considered as a functional food for the treatment of Primary Dysmenorrhea without inducing adverse effects.

The Science Base of Herbs in Induced Weight Loss and Weight Maintenance

Overweight and obesity have reached epidemic proportions in many countries, causing economic burden in increased absentees in the workplaces and funding for the health care system in dealing with obesity-related disorders such as diabetes, hypertension, and cardiovascular diseases(1).

One of six Malaysian are either extremely overweight or obese, according to Datin Paduka Santha Kumari, chairman of the Selangor branch of the Malaysian Diabetes Association and the global health observatory, at least 2.8 million people die each year as a result of being overweight or obese, and an estimated 35.8 million (2.3%) of the global burden of disease is caused by overweight or obesity(2).

Epidemiological studies, linking herbal medicine, foods, and vitamins in preventing and treating these diseases have been inconclusive(a)

Some researchers suggested that the intake of certain herbal medicine may enhance appetite suppression.

According to the Georgetown University Medical Center, the appetite of average daily food intake was decreased only with the herbal formulation, regardless number of single-ingredient in comparison to the effectiveness among herbal formulation and a commonly available(3).

Other researchers insisted that using herbs and supplements to induce weight loss should be taken with care, as a considerable number of reports have been published on hepatotoxicity associated to intake of herbal products(4)(5)(6)(7).

The College of Medicine, The Ohio State University insisted that various dietary, lifestyle and psychological factors are involved in the etiology of Prameha, in initiated disturbances of fat and carbohydrate metabolism in overweight and obese(8), without effective management, obtaining a workable weight loss plan may be extremely difficult.

1. Ginseng
Ginseng is a slow-growing perennial plant with fleshy roots, the genus Panax, belongings to the family Araliaceae. Depending on the climate where it grows, ginseng can be classified mainly into Panax ginseng Asian ginseng (root), Red ginseng, wild ginseng, American ginseng (root)(14).

In 10 obese middle-aged Korean women who took Panax ginseng extracts for 8 wk, showed effectively in influence on gut microbiota in promoting weight loss but depending to the composition of gut microbiota prior to ginseng intake(15).

In mice fed with a high-fat diet, ginseng saponin and ginsenosides exerted anti-obesity effects via the modulation of physiological lipid metabolism in vivo or intracellular signaling in cell culture systems and abnormal physiology of leptin, insulin, and adiponectin in energy and lipid metabolism(16).

2. Zingiber Officinale(Ginger
Ginger (Zingiber officinale) or ginger root is the genus Zingiber, belongings to the family Zingiberaceae, native to Tamil, used in traditional and Chinese medicine to treat dyspepsia, gastroparesis, constipation, edema, difficult urination, colic, etc.(17).

According to the Tehran University of Medical Sciences, Slimax (extract of several plants including Zingiber officinale and Bofutsushosan) showed a significant decrease in body weight or inhibited weight gain(18).

Application of composition of a multi-ingredients supplement containing primarily raspberry ketone, caffeine, capsaicin, garlic, ginger, and Citrus aurantium, also improved body composition, waist and hip girth, in an eight-week diet and exercise weight loss program(19).

In a comparison of the weight loss effect of Orlistat, a pharmacological agent promoting weight loss, injection of ginger (Zingiber officinale Roscoe, Zingiberacae) in male albino rats induced a great ability to reduce body weight without inhibiting pancreatic lipase level, or affected bilirubin concentration, and increased peroxisomal catalase level and HDL-cholesterol(20).

3. Cissus quadrangularis (CQ)
Cissus quadrangularis is an ancient herb used to treat a variety of indications, belongings to the grape family, probably native to India or Sri Lanka.

In a randomized, double-blind, placebo-controlled design study involving 123 overweight and obese persons (47.2% male; 52.8% female; ages 19-50). conducted by the University of Yaoundé I, Cissus quadrangularis, application of cissus quadrangularis displayed a statistically significant net reduction in weight and central obesity, as well as normalized metabolic syndrome(21).

In a comparison of the efficacy of Cissus quadrangularis-only and Cissusquadrangularis/Irvingia gabonensis combination, on weight loss in overweight and obese human subjects, cissus quadrangularis-only group showed significant reductions on all variables in compared to the placebo group, but lower effects in compared to the cissus quadrangularis/Irvingia gabonensis combination group(22)

Intake of CQR-300, a proprietary extract of Cissusquadrangularis, also demonstrated a significant deceased weight and blood glucose levels, serum lipids, thus improving cardiovascular risk factors, according to the University of Yaoundé I,(23).

4. Sambucus nigra
Elder also was known as Common Elder, is a shrub or small tree of the genus Sambucus of 30 specie, belongings to the family Adoxaceae, native to the temperate-to-subtropical regions of both the Northern Hemisphere and the Southern Hemisphere, used in traditional and herbal medicine to treat constipation, colic, diarrhea, colds, flu, bronchial and upper-respiratory, irritation of the skin, rheumatism, etc.

The composition of supplement with Sambucus nigra and Asparagus officinalis study expressed effectiveness in reduced weight, blood pressure, enhanced physical and emotional well-being, and quality of life(24).

The Tehran University of Medical Sciences, Sambucus nigra study, also demonstrated a significant decrease in body weight in numbers of animal studies(25).

5. Asparagus officinalis
Asparagus is a flowering plant belonging to species the genus Asparagus, native to the western coasts of northern Spain, north to Ireland, Great Britain, and northwest Germany, northern Africa, and western Asia, used from early times as a vegetable and medicine, because of its delicate flavor and diuretic properties(26).

Asparagus officinalis L. powder tablets provided 19 mg saponins per day in overweight and obese subjects reduced mean weight, blood pressure, and enhanced physical and emotional well-being and the quality of life with the rate of very good or good in most of the completers(27).

In 12 weeks supplementation of a botanical extract-based weight loss formula, including Asparagus, researchers at the Poznan University of Medical Science indicated that the formula exerts a significant change of the body composition improvement index (BCI), induces greater weight loss than placebo, probably through a concurrently performed exercise program recommended strategy for lifestyle modification(28).

6. Slimax
Slimax (extract of several plants including officinale and Bofutsushosan), in the reviews of the database of studies published, researchers showed that intake of Slimax indices a significant decrease in body weight(29).

FDA banned Ephera (Ma Huang) weight loss products in 2004
[Ma Huang is also known as ephedra. The acrid, slightly bitter, and warm herb has been used in TCM to promote urination and sweating and rid of cold, soothe wheezing asthma, ...., by enhancing the functions of lung and bladder channels(9).

According to Dongguk University, certain chemicals found in Ephera showed an association with changes of BW and BMI through alteration of gut microbiota varied by indigenous microbiota of tested subject(10).

The combined study of a low-calorie diet and Ephera, in 125 otherwise healthy obese women, showed effectively in reducing BMI. RMR change not compensated for by the herbal medicines tried.

Resting metabolic rate (RMR) change seemed to be affected by constitution and body composition rather than by medicine(11). The Dongeui University study of composition GGEx18( Laminaria japonica, Rheum palmatum, and Ephedra sinica) in high fat diet-induced obese mice suggested that GGEx18 improves obesity through skeletal muscle AMPK and AMPK-stimulated expression of PPARα and its target enzymes for fatty acid oxidation, through inhibited lipid accumulation, and similar activation of genes(12).

Please note that due to its adverse reactions of palpitations, stress, headache and insomnia and stroke, heart attacks, cardiac arrhythmias, seizures, and psychotic effect, the FDA banned the product on 2004, the Dutch Inspectorate for Health Care and the Ministry of Health in Canada have recently requested a market recall of some ephedrine-containing herbal products in response to above adverse reactions. In the Netherlands, the status of Ephedra-containing products is currently reconsidered(13)]
