Respiratory Disease is defined as medical conditions which affect the breathing organ and tissues including Inflammatory lung disease, Obstructive lung diseases, Restrictive lung diseases, Respiratory tract infections, trachea, bronchi, bronchioles, alveoli, the nerves and muscles breathing , etc,.

                                  Bronchiectasis

Bronchiectasis is defined as a condition chracterized by the damage of the localized, irreversible dilation of part of the bronchial tree and the walls of the large airways of the lung as a result of the destruction of the lung muscles and elastic tissues. Bronchiectasis can be present alone, but in most cases, it is a disease coexisted with patient of chronic obstructive pulmonary disease (COPD).

The Antioxidants
Evidences of antioxidants enhance immune function in fighting against free radicals and foreign invasion are well know, specially in aging population. In the study by Institute of Human Nutrition, University of Southampton, showed that while antioxidant defenses interact when a component is compromised, the nature and extent of the defenses are influenced by dietary intake of sulfur amino acids, for glutathione synthesis, and vitamins E and C. In animal studies, in vivo and in vitro responses to inflammatory stimuli are influenced by dietary intake of copper, zinc, selenium, N-acetylcysteine, cysteine, methionine, taurine, and vitamin E. Information from animal studies has yet to be fully translated into a clinical context. However, N-acetylcysteine, vitamin E, and a cocktail of antioxidant nutrients have reduced inflammatory symptoms in inflammatory joint disease, acute and chronic pancreatitis, and adult respiratory distress syndrome. Impaired antioxidant defenses may contribute to disease progression after infection with human immunodeficiency virus. Powerful arguments have been advanced for treatment with antioxidants to slow progression of acquired immunodeficiency syndrome(41).
Also according to the study by Universidad Complutense de Madrid, several laboratories, including our own, have shown that antioxidants preserve an adequate function of immune cells against homeostatic disturbances caused by oxidative stress, such as that involved with age. Therefore, since the immune system is an indicator of health and a longevity predictor, the protection of this system afforded by dietary antioxidant supplementation may play an important role in order to achieve a healthy ageing(42).

Bronchiectasis can be present alone, but in most cases, it is a disease coexisted with patient of chronic obstructive pulmonary disease (COPD).

                                  The Prevention
The do and do not’s list
1. Quit smoke and avoid air polution
As the disease developed as late complications of a smoke inhalation injury. If you are constantly working in dusty or air polution environment, prevent inhaling with proper air mask(14, 14a).

2. Enhance immune function to prevent inflections caused by bacteria and virus
Bronchiectasis is a chronic, progressive lung disease where there is irreversible, abnormal dilatation of one or more bronchi, with chronic airway inflammation, associated chronic cough and sputum production, recurrent chest infections, and airflow obstruction. According to the study by Imperial College London, National Heart & Lung Institute, abnormalities in innate and adaptive immunity may predispose to bronchiectasis at both stages. Recent immunogenetic evidence suggests that there may be a link between the level of natural killer (NK) cell activation and disease susceptibility, implicating a predisposing role for innate immune mechanisms(38).

3. Proper treatment of lung infection
As the progression of the infection and inflammation can lead to chronic cough and sputm and recurrent chest infections, airflow obstruction

4. Prevent lung damage causes of broncholithiasis
In the study to to determine the relationships between ventilation distribution outcomes and the presence and extent of structural damage as assessed by chest CT in infants and young children with cystic fibrosis (CF), found that chest CT and pathology outcomes were available in 49 infants (31 male) with bronchiectasis and air trapping present in 13 (27%) and 24 (49%) infants, respectively. The presence of bronchiectasis or air trapping was associated with increased M(2)/M(0) but not LCI or M(1)/M(0). There was a weak, but statistically significant association between the extent of air trapping and all ventilation distribution outcomes(39).

5. Avoid children to inhale small objects of which can to stick to the small air way and increase the risk of broncholithiasis.

6. Eat your fruit and vegetables
Fruits and vegetables contain high amount of antioxidants which can promote immune function in fighting against the foreign invasion cause of infection, such as bacteria and virus. There is
the evidence on the association between fruit and vegetable (F&V) consumption and risk of chronic disease(40).

Common cold is a condition of viral infection of the upper respiratory system of that lead to symptoms of runny nose, nasal congestion, sneezing, sore throat, cough, headache, etc.
According to the statistic conducted by the University of Aberdeen, cranberry, one of most comon use of herb in complementary and alternative medicine (CAM) amongst the general population.for treatment of common cold and enhanced immunity(1).

Dr. Bennett J and Dr. Brown CM. said, "Almong 40% of patients used herbal remedies, cranberry.... is one the most use to treat or prevent a health condition (e.g., common cold)(3)" in the study by the Methodist Hospital, Omaha.

Cranberry, a species of Vaccinium oxycoccos, belongings to the family Ericaceae, found in the cooler part of Northern America. Daily consumption of a cranberry beverage exhibited immune modulating effect by inducing the cells first line of defense in the epithelium, for decreased risk of common cold and flu(2), the study by the University of Florida insisted.
High molecular weight non-dialyzable material (NDM) found in Cranberry juice, may consist a therapeutic potential against both A and B influenza virus infections as the juice exhibited anti viral effects through attenuate the invasion of influenzavirus (IV) as well as to neutralize the cytotoxicity of IV in cell cultures(5).

Raspberries, used as a herbal remedy in common cold, fever and flu-like infections(6), are perennial, the edible fruit of a multitude of plant species in the genus Rubus belongings to the family Rosaceae, originated from Europe. Because of their commercial values and health benefits, the fruits have been grown for processing of quick frozen (IQF) fruit, purée, juice, or as dried fruit used in a variety of grocery products.
According to the Medical University of Gdańs, with a rich sources of phytochmicals, sanguiin H-6 and ellagic acid, raspberries process a valuable and biological properties including antioxidative, antimicrobial and cytotoxic, including treament of common cold(6).
Its ethanol-water extracts with sanguiin H-6 and ellagic acid found effectively against in actively against eight and nine bacterial strains(7), through enhancing the immune system by regulating NADPH oxidase activity against bacteria(8).

Green tea has been a precious drink in traditional Chinese culture and used exceptional in socialization for more than 4000 thousand years. Because of their health benefits, they have been cultivated for commercial purposes all over the world.
L-Theanin, antioxidant found in green tea, has been found epidemiologically to consist various positive effects particular for treatment of common cold(9). According to Dr. Bukowski JF and Dr. Percival SS. the antioxidant also enhance the function of gammadeltaT cells against the proliferate to a wide variety of microbes and tumor cells, as well as decreasing the incidence of cold and flu symptoms(10).

Taking altogether people who are at increased risk of Common cold due to weaken immune system or aging .....should drink at least one serving daily and people with common cold should drink the smoothie as much as they can, depending to digestive toleration.
Change life style and diet pattern are also recommended.

The smoothie for reduced risk and treatment of Anemia(β-thalassemia and sickle cell disease)
Yield: 2 serving (about 8 ounce each)
1 1/2 cups papaya
1 cup of rice milk

1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed. Add more green tea drink if needed
3. Serve immediately

The finding the natural ingredients for treatment of anemia(β-thalassemia and sickle cell disease) is considered as a dream of many scientist to replace the long usage adverse effect of conventional medicine to other organs in the body. Unfortunately, many compounds found effective in initial studying failed to confirm the potential in large sample size and multi center.

Most people understand that anemia is caused by iron deficiency in the bloodstream, but in reality anemia is characterized by deficiency in the hemoglobin of the red blood cells diminishing the ability of the blood to transport oxygen to our cells and to remove carbon dioxide.

Antioxidant plays an important role in the influences of the pharmacologic induction of fetal hemoglobin (HbF) for treatment of β-thalassemia and sickle cell disease through expression in the fetal liver, spleen and bone marrow(1).

Recent studies suggested that chronic consumption of papaya(2) may be beneficiary to reduce symptoms and treatment for patients with anemia such as β-thalassemia and sickle cell disease.

Papaya is a species of Carica Papaya, belongs to the family Caricaceae and native to the tropics of the Americas, and was first cultivated in Mexico and several centuries. Vitamin C found abundantly in papaya out of all fruits, according to the Chinese Academy of Medical Science, improved the levels of hemoglobin-based oxygen carriers (HBOCs) through the indication of the oxygen binding curves and the molecular weights(2).

In patients with sickle cell hemoglobin from both parents and are associated with unusually large numbers of immature blood cells, C. papaya seed oil significantly increased HbSSM and HbSSF human sickle cell blood peroxidase activity and hemoglobin concentration significantly, through exhibited particular antisickling properties and reduced oxidative stress in sickle cell patient(3).

In patients with beta-thalassemia major and intermedia and E-beta-thalassemia due to defects and mutations in the beta-globin gene, fermented papaya preparation (FPP) improve the status of anemia through increased the content of reduced glutathione (GSH) in red blood cells (RBC), and decreased free radicals generation, membrane lipid peroxidation, and externalization of phosphatidylserine (PS) in beta-thal, major and intermedia, (in Israel) and E-beta-thal patients(4).Dr. Olalla-Saad ST said, "... reduction of oxidative status in red blood cells of patients with β- and E-β-thalassemia (β-thal) treated with fermented papaya preparation"(5) of that reducing the severity of the symptoms of anemia(6).

In fact, on sickle cell trait (HbAS) and sickle cell disease (HbSS) blood samples, long term consumption of tropical fruit extract, Carica papaya produced significant less numbers of sickle cells, the Covenant University study suggested(7).

People who are at increased risk of anemia(β-thalassemia and sickle cell disease) due to family history, genetic mutation .....should drink at least 2 serving daily and people with chronic anemia(β-thalassemia and sickle cell disease) should drink the juices as much as they can, depending to digestive toleration.
Change of life style and diet pattern are also recommended.

The smoothie for reduced risk and treatment of Chocolate Cysts
Yield: 3 serving (about 8 ounce each)
1/2  cup lavender herbal tea
1/2 cup chamomile herbal tea
1/2 cup St. John's wort herbal tea
3/4 cup flax seed

1. Place all ingredients in a blender and puree about 1 minute
2. Blend on high speed about 1 minute or until the mixture is thick and the ice is well crushed.
3. Serve immediately

The finding of a natural source for reduced risk and treatment of pospartum depression  has encountered many obstacles, many ingredients showed initially the promising result in animal studies have not produced same potentials in either large sample size and mutli centers human trials.

Scientists in some well known institutions may have found the therapeutic and potential ingredients from natural sources(Green tea and grape) for reduced risk and treatment of pospartum depression.

Lavender is a flower plant of the genus Lavandula, belongings to the family Lamiaceae, native to Asia. The herb has been used in traditional medicine to treat painful bruises and aches, to relieve various neuralgic pains, sprains, rheumatism, etc.

Pospartum depression (postnatal depression), is a type of clinical depression, affecting women, after childbirth. Normally this of nervous tension occurs in women in the first new months of pregnancy, but in some women, it can last after child birth.

According to the joint study lead by the National Cheng Kung University, lavender tea relieved sleep quality, fatigue, and depression; and in improving maternal-infant attachment during the early postpartum period(1).
Dr. Keshavarz Afshar M. and the research team at the joint study lead by the Tehran University of Medical Sciences. suggested, use of Lavender essential oil as aromatherapy improved the quality of sleep in postpartum women(3).

Chamomile is also known as camomile, common name of many species daisy-like plants in the family Asteraceae. The herb has been used in traditional medicine as antispasmodic and anti-inflammatory constituents and to treat menstrual cramps and sleep disorders, reduce cramping and spastic pain in the bowels, relieve excessive gas and bloating in the intestine, etc.
According to the study of a total of 80 Taiwanese postnatal women with poor sleep quality (Postpartum Sleep) recruited from November 2012-August 2013, intake of chamomile tea, as a supplementary may be recommended to postpartum women for alleviating depression and sleep quality problems(2).

St John's wort, a yellow flower plant in the genus Hypericum belongings to the family Hypericaceae, native to the regions of North America, Europe, Turkey, Russia, India, China and Brazil, has been used for treatment of depression(5) over thousand of years.
According to the report by University of Vienna, St. John's wort may be effective for treatment of postnatal depression, but women who breastfeed, long-term studies of outcome in infants are needed(4).

Omega 3 fatty acids found abundantly in flax seed are associated with postpartum depression,, according to dr. Levant B, low tissue levels of Omega 3 fatty acods, particularly docosahexaenoic acid (DHA), are reported in patients with either postpartum or nonpuerperal depression(6). In animal study, Omega 3 fatty acids, expressed its anti depressant-like and postpartum depression effects through decreased the biomarkers related to depression such as corticosterone and pro-inflammatory cytokines(7).
In support of above claim, the randomized dose-ranging pilot trial of omega-3 fatty acids for postpartum depression found that post treatment of Postnatal Depression Scale (EPDS) and Hamilton Rating Scale for Depression (HRSD) is much low than the pre treatment scale, after 8 weeks application of Omega 3 fatty acids(8).

The combination of smoothie with ingredients of  Lavender, Chamomile, St. John' Wort and Flax Seedmay hold a key in further studies in production of an effective natural ingredients for prevention and treatment of postpartum depression without induced adverse effects

Women who are at increased risk of postpartum depression due to family history,... should drink at least one cup daily. Women with postpartum depression should drink no more than 4 serving daily depending to the digestive toleration.

Green tea has been a precious drink in traditional Chinese culture and used exceptional in socialization for more than 4000 thousand years. Because of their health benefits, they have been cultivated for commercial purposes all over the world. In animal model, polyphenolic compounds present in green tea, particularly catechins, are well known to process a strong anti-influenza activity in in a dose-dependent effect, especially in the early phase of lungs infection(1).
According to the Yonsei University, Catechin derivatives found in green tea also exhibited a broad spectrum activity against various subtypes of influenza viruses, through its antioxidant effect in inhibition of circulating human influenza viruses (A/H1N1, A/H3N2 and B type), H2N2 and H9N2 avian influenza virus, the viruses on red blood cell (RBC)(2).
Dr. Song JM and colleagues at the Yonsei University, polyphenolic compound catechins ((-)-epigallocatechin gallate (EGCG), (-)-epicatechin gallate (ECG) and (-)-epigallocatechin (EGC)) all showed to exhibited antiviral effect on influenza virus through  altering the physical properties of viral membrane(3).

Grape is a woody vines of the genus Vitis, belong to the family Vitaceae, native to southern Turkey.
Its antioxidant, resveratrol  has shown to process strong anti influenza virus, including, inhibitory effect on the NA activity of the influenza virus strain A/PR/8/34 (H1N1)(4)(5), epidemiological studies suggest.
Resveratrol (RV) in the Univ. of Rome Sapienza study also exhibits anti-inflammatory, and anti-proliferative properties against a wide variety of viruses including members of the Herpesviridae family, retroviruses, influenza A virus and polyomavirus by altering certain cellular pathways(6).

Dr. Thapa M and colleagues at the Kansas State University said, " polyphenols, such as resveratrol and epigallocatechin gallate (EGCG), showed significant anti-influenza activity in vitro and/or in vivo", especially against influenza virus (porcine H1N1 strain)(7).

Taking altogether people who are at increased risk of  influenza  due to weaken immunity and aging  .....should drink at least one serving daily and people with  influenza  should drink the smoothie at 4 serving a day, depending to digestive toleration.

                                               The Diagnosis

If you are experience the above symptoms, you doctor may suspect that you have developed broncholithiasis. After recording your family history and completing the physical exam, the test which your doctor orders include
1. Blood test
The aim of the blood test is to check for underline causes of broncholithiasis as well as any infection of
the lungs or breathing passages.
2. A sputum culture
A sputum culture is to identify bacteria or fungi that infect the lungs or breathing passages.
3. Bronchoscopy
Bronchoscopy is considered the most important diagnostic test for broncholithiasis. In retrospective review of the clinical data of patients with broncholithiasis who also underwent bronchoscopy at Mayo Clinic, Forty-eight percent (23 of 48) of the partly eroding broncholiths were successfully removed bronchoscopically, with a greater percentage removed with the rigid bronchoscope (67%) than with the flexible bronchoscope (30%). All free broncholiths were completely extracted regardless of the type of bronchoscope used. Complications occurred in only two patients (4% of the bronchoscopic broncholithectomy group), both with partially eroded broncholiths, and consisted of hemorrhage in one patient requiring thoracotomy and acute dyspnea in another patient, caused by a loose broncholith lodged in the trachea. We conclude that flexible and/or rigid bronchoscopic extraction of partly eroded or free broncholiths in the tracheobronchial tree can be considered safe and effective(33).
4. Chest CT scan
CT can suggest the diagnosis of broncholithiasis and is useful when bronchoscopy does not show a broncholith, according to the study byBroncholithiasis: CT features in 15 patients(34).
5. Chest X ray
The aim of the test is to observe any abnormality in the lung causes of broncholithiasis.
6. High resolution computed tomography (HRCT), laminograms and Friberoptic bronchoscopy ( FOB )
According to the study to evaluate high resolution computed tomography (HRCT) in the diagnosis of broncholithiasis, researchers at the Service central de Radiologie et Imagerie Médicale, indicated that
in 9 cases, chest X rays were abnormal but the diagnosis of broncholithiasis can’t never be affirmed. In 9 cases, FOB was abnormal: broncholith were identified in only 2 cases; the other diagnosis were tumor like stenosis (n = 3), inflammatory stenosis (n = 3), extrinsic compression (n = 1). On conventional CT scan, broncholithiasis was suspected in 8 patients but because of volume averaging the relationship between calcified lymph nodes and bronchial tree was difficult to determine exactly. Only HRCT sections, sometimes tilted in the axis of the middle lobar bronchus, can affirm the endobronchial or peribronchial location of calcified lymph nodes in all patients(35).
In other study to assess the validity and reliability of the Korean version of chronic obstructive pulmonary disease assessment test (CAT) and Dyspnea-12 Questionnaire for patients with bronchiectasis or tuberculous destroyed lung, showed that Korean version of CAT and Dyspnea-12 are valid and reliable in patients with tuberculous destroyed lung and bronchiectasis(36).
Another study indicated that in 8 of the 19 patients, laminograms or chest CT scans helped establish the diagnosis; FOB was performed on 18 patients and was abnormal in each case, with 8 intrabronchial calcifications identified. However, fiberoptic bronchoscopy ( FOB ) has limited therapeutic indications in this disorder(37).
