Obesity is a medical condition of excess body fat accumulated overtime, while overweight is a condition of excess body weight relatively to the height. According to the Body Mass Index(BMI), a BMI between 25 to 29.9 is considered over weight, while a BMI of over 30 is an indication of obesity. According to the statistic, 68% of American population are either overweight or obese.

You can calculate your BMI index BMI= weight (kg)/ height (m2)

Conjugated linoleic acids (CLA) are a family of over 28 isomers of linoleic acid found abundantly in the meat and dairy products.
In this randomized, double-blind, placebo-controlled trial, subjects with a body mass index (BMI) of 24 to 35 kg/m(2) randomly received 1.7 g of cis-9,trans-11 and trans-10,cis-12 CLA (n = 30) or placebo (salad oil; n = 33) in 200 mL of sterilized milk twice daily for 12 wk, conducted by Kaohsiung Veterans General Hospital Pingtung Branch(1). In the sixty-three subjects completed the study (CLA, n = 30). After 12 wk, compared with the baseline, the BW, BMI, total fat mass, fat percentage, subcutaneous fat mass, and waist-to-hip ratio decreased in the CLA group (P < 0.05). The CLA group was stratified by BMI and gender. The BW, BMI, subcutaneous fat mass, and waist-to-hip ratio decreased in 27 subjects with a BMI ≥ 27, and these indices, except subcutaneous fat mass, were lower in female subjects. The levels of total cholesterol, triacylglycerol, low-density lipoprotein, and plasma fasting glucose increased, whereas those of high-density lipoprotein decreased after 3 mo of CLA treatment.

For a healthy person weight loss is simple: burn more calories than you consume. The healthiest weight is determine by height to weight ratio or Body Mass Index (BMI) and the amount of fat in your body. Unfortunately over 60% of the North American population is considered to be overweight, according to the BMI index. There must be something wrong with our diet. In faraway places such as Southern Italy and Greek, most people live longer than us because their diet contains lots of fruits and vegetables. The People in Southern Italy and Greek only eat fish and poultry a few times a week and meat a few times a month. Their diet is high in carbohydrates and only containing 8% of saturated fat.
You can find some very good traditional Greek foods in you local restaurants.
Here are some meals that I found very different than our diet:

1. Savor souvlaki
Also known as shish kebob, it contains a large amount of vegetables and a small amount of meat. This is a meal with an average of about 300 calories and only 10 grams of fat with 1/5 of fat being saturated.

2. Pita Bread sandwich
Pita bread sandwich includes hefty raw vegetable, some meat, feta cheese and cucumber sauce. It contains roughly 800 calories with 45 grams of total fat and half of that are saturated.

Unfortunately, under the influence of western diet, some of the meals that we found is no longer really Greek foods. They are high in meat and saturated fat, therefore please do not assume that all Greek restaurant are original Mediterranean diet.

Here are 2 recipes of original Mediterranean diet that not only helps you to lose weight but also reduces your high blood pressure and bad cholesterol level in result of lowering risk of heart diseases and strokes.

1.Sauce of yoghurt with dill ( Sources from Ellaia)

Sauce of yoghurt with dill
300 gm. yoghurt with low water content
1 spoonful dry dill
1 clove garlic, salt, pepper
2 spoonfuls olive oil ELLAIA
1 spoonful lemon juice.
We pulp the garlic in a mortar with little salt.
In a deep bowl we strike yogurt with garlic, the dill and the pepper and we continue with the olive oil and the lemon until they are mixed well. We maintain the mixture in the refrigerator. Accompanies vegetables and green salad.

2. Meat with plums ( Sources from Ellaia)

1,5 kg meat (pork, lamb, beef or even chicken)
1 onion
salt, pepper, nutmeg
1 cinnamon stick
5-6 clovesjuice of 2 lemons
2-3 spoonfuls olive oil ELLAIA
Half kg plums
1 cup almonds
2 spoonfuls sugar or honey
Wash the meat and cut it in portions. We put it in a basin and throw on the onion in big particles, salt, pepper, the cinnamon, cloves, the nutmeg and the juice of lemon. We leave the meat to be marinated for 2-3 hours and meanwhile we wet the plums in water in order to absorb water and also the almonds in order to be peeled easily. We put the olive oil in a saucepan and sauté the meat. We douse with the marinade liquid, we add 1-2 glasses hot water and we boil the meat in low temperature, until it gets soft. We put inside also the plums, the almonds and the honey or the sugar and we leave them to boil for a few minutes.

Finally we will provide you with a Mediterranean diet food list. It contains cheese, eggs, fish and sea food, fruits, grains, legumes, nuts, olive oil, red wine, vegetables and yogurt.
On the top of the pyramid contains meat, poultry and on the lower broader base, there are dairy products, oils, fruits and vegetables.

                Polycystic Ovarian Syndrome

Polycystic Ovarian Syndrome is defined as endocrinologic diseases among reproductive-age women caused by undeveloped follicles clumping on the ovaries that interferes with the function of the normal ovaries associated with a high risk for metabolic disorder(1) as resulting of enlarged ovaries(2), leading to hormone imbalance(excessive androgen and anti-müllerian hormone (AMH) )(1)(3)(4)(5), induced Hirsutism(6)(7), reproductive disorder(10(12)), risks of type 2 diabetes(9)(10)(11), metabolic syndrome(10)(12) and early cardiovascular disease(8)(13), acne(10)(14), endometrial cancer(18)(19),weight gain and obesity(15)(16)(17). The syndrome effects over 5% of women population or 1 in 20 women.
Unfortunately, according to studies, women with PCOs after the reproductive age, are associated to continuously increase risk of type II diabetes, with no increasingaltered glucose tolerance(20), CVD and hypertension(21).

                         The Treatments

In Weight Loss Perspective

Weight loss(733)(734) for obese patients is one of most vital factor to reduce PCOs symptoms(735)(737) and risk of PCOs complications(736)(737). Some researchers found that 5-10% of weight low will improve PCOs features(737) by 25%, including pregnancy(738). How can you lose weight and maintain healthybody for a healthy conception?

              The Common Weight Loss Foods

Epidemiological studies, linking certain foods in reduced weight loss for obese individual have produced certain positive results(759)(760)(761), but researchers emphasized that weight loss can not be in the expenses of intake of nutrients(762).

1. Oats
Oat is a species of cereal grain grown for its seed and was first cultivated in Europe but is now grown all over the world, especially in Scotland and England. It is one of good crop to plant in the poor soil after crops are harvested in the Fall(763.
Oat, rich in beta-glucan, may hold a magic power in regulation of metabolism and liver-protecting effect. According to Chung-Shan Medical University, consumption of oat reduced obesity, abdominal fat, and improved lipid profiles and liver functions with no adverse effects(764). Whole-grain ready-to-eat oat cereal, in a randomized study of 144 Free-living, overweight and obese adults had favorable effects on fasting lipid levels and waist circumference(765). Other study suggested that at-derived beta-glucan, combined with moderate exercise also improved lipid profile and caused a decrease in weight(766).

2. Salmon
Salmon is the common name for Salmonidae. They are anadromous, born in fresh water, migrate to the ocean, then travel thousands of miles in the deep sea cold water throughout their life cycle and within to five years returning to the exacted location where they were born to reproduce and die(767).
Dietary salmon oil, in rat study showed to reduced weight loss in mice model, but induced weight gain in recombinant inbred strains. This findings may suggested that body weight response to salmon oil feeding is controlled by multiple genes(768). A novel oral form of salmon calcitonin (sCT), according to the study at1Nordic Bioscience, also showed that salmon oil improves fasting and postprandial glycemic control and insulin sensitivity and may be considered as a promising agent for the treatment of obesity-related insulin resistance and type 2 diabetes patients(769)(770).

3. Brown rice
Brown rice or unpolished rice is a kind of whole, natural grain with very brown color of which only the outermost layer of the rice kernel are removed, thus preventing the lost of nutrients(771).
The study of the weigh effect of brown rice on metabolic parameters in type 2 diabetes, showed a positive effect of consumption of the brown rice (LB) in association with a decreased waist circumference in type 2 diabetic patients(772). Other randomized study of forty women between 20 and 35 years of age in the comparison of white rice and mixed rice of weight control, including brown rice, indicated that meal replacement with mixed rice was superior to replacement with white rice in weight control, improving antioxidant enzyme activity(773).

4. Chicken
Chicken are domesticated fowl belonging to the subspecies of Gallus domesticus and is raised all over the world for its delicious meats and eggs. It can be roasted, broiled, grilled or poached into a very delicious, tasteful and nutritious meal(774).
In the comparison of weight loss and total lipid profile changes in overweight women consuming beef or chicken, researchers at 1Rippe Lifestyle Institute, Shrewsbury showed a interest result of weight loss and improved lipid profile can be accomplished through diet and exercise, regardless to dietary protein source including lean beef or chicken(775). But the study at University of Navarra, showed a opposite result in the study of frequent consumption of selenium-enrichedchicken meat(776), it may be the result of the presence of antioxidant selenium in the meat.

5. Water
According to Virginia Tech, consuming 500 ml water prior to each main meal may lead to greater weight loss than a hypocaloric diet alone in middle-aged and older adults, because of it acutely reduces meal energy intake(EI)(777) but not in younger subjects(778). The Choose Healthy Options Consciously Everyday (CHOICE) randomized clinical trial, suggested that replacing caloric beverages with water or diet beverages induced 2% to 2.5% weight loss(779).

6. Nuts
Dietary consumption of nuts with rich sources of multiple nutrients is associated with health benefits and weight loss, epidemiologically. but in well-controlled nut-feeding trials, no changes in body weight were observed(780). Pistachio nuts, a member of the cashew family, can be consumed as a portion-controlled snack for individuals restricting calories to lose weight without concern of weight gain(781)(782).

7. Green tea
Green tea contains more amount of antioxidants than any drinks or food with the same volume, and is the leaves of Camellia sinensis, undergone minimal oxidation during processing, originated from China. Green tea has been a precious drink in traditional Chinese culture and used exceptional in socialization for more than 4000 thousand years. Because of their health benefits, they have been cultivated for commercial purposes all over the world.
According to Maastricht University, caffeine intake and green tea supplementation are associated to improved weight maintenance, partly through thermogenesis and fat oxidation, in a randomized placebo-controlled double blind parallel trial in 76 overweight and moderately obese subjects(783). Catechins the antioxiands found abundantly in green tea, significantly decreased body weight and significantly maintained body weight after a period of weight loss, depending to the moderators of habitual caffeine intake and ethnicity(784). In a high-protein diet, green tea-caffeine mixture, failed to exert the same effect on body weight maintenance(785).

8. Coffee
Coffee made from the roasted seeds of the genus Coffee, belonging to the family Rubiaceae native to southern Arabia. Coffee may consist certain substances.
In a randomized, double-blind, placebo-controlled, linear dose, crossover study, green coffee extract, consistent with human and animal studies and a meta-analysis may be an effective nutraceutical in reducing weight in preobese adults(786). Mannooligosaccharides (MOS), extracted from coffee, exhibited a potential functional use for weight management and improvement in adipose tissue distribution, but varying in gender(787)
Unfortunately, green coffee extract (GCE), according to University of Exeter, in reviews of the effect against weight loss of existed data, showed a significant poor methodological quality and suggested that more rigorous trials are needed to assess the usefulness of GCE as a weight loss tool(788). Other study, reported a case of Brazil Potent Slimming Coffee and possibly also other weight-lossproducts may be contaminated with sibutramine which can cause severe adverse reactions, mentally(789).

                 Cardiovascular Disease

Cardiovascular disease is defined as medical conditions affecting the cardiovascular system, including heart, blood vessels(arteries and veins).
Beside cancer, heart disease kills more than 2,000 Americans everyday. Approximately 60 million Americans have heart disease.
There are many causes of heart diseases. Most of heart diseases are caused by high blood pressure contributes to hardening of the arteries. High levels of bad cholesterol (LDL) build up in the arteries as a result of uncontrolled diet with high levels of saturated fat and trans fat. All these add to the formation of atherosclerosis lesions and eventually arterial blockage or anything that serves to damage the inner lining of blood vessels and impedes the transportation of oxygen and nutrition to the heart can be defined as a risk of heart disease.

                      Ischemic heart disease
Ischemic heart disease is defined as a condition of reduced blood supply of the heart muscle, as a result of coronary artery disease caused by plague building up in the arteries.

                 The Causes and Risk Factors

The Causes
Cause of Ischemic heart disease is a result of reduced blood supply of the heart muscle, due to coronary artery disease caused by plague building up in the arteries.

The Risk Factors
1. Diabetes
Dr. Wentworth JM and the research team at the Royal Melbourne Hospital Department of Diabetes and Endocrinology, indicated that people with long-standing type 2 diabetes who attend a tertiary hospital outpatient clinic, ischemic heart disease, in contrast to other vascular complications, correlates robustly with BMI. These findings indicate that clinical trials of weight loss in type 2 diabetesshould use cardiac endpoints as their primary outcomes(3).

2. High lever of blood cholesterol
In the study of the male population of Kaunas, aged 45-59 years, who were examined in 1972-1974, Among 2,034 men, who at the first examination were without signs of ischemic heart disease (IHD), there appeared after 7.4 years 75 cases of IHD as a result of increased high levels of blood cholesterol(4)

3. Hypertension
"Epidemiological surveys show the clear association of hypertension with an increased risk of developing ischemic heart disease. One method of quantifying atherosclerosis is to measure, at necropsy, the percentage of the intimal surface of the coronary arteries or aorta which is occupied by raised plaques" Dr. Davies MJ. and the team of researchers at St George's Hospital Medical School, University of Londo, said(5)

4. High blood triglycerides
High blood triglycerides is associated to increased risk of Ischemic heart disease. Dr. Chien KL, and the team at the National Taiwan University, indicated that low HDL-C was significantly associated with acute coronary events, and triglyceride levels as well as renal function were inversely related to all-cause deaths after thecoronary event(6).

5. Obesity
In the document of Concomitant ischemic heart disease (IHD), in 350 normotensives (4%) and in 119 hypertensives (13.8%). The prevalence of IHD was not significantly different in lean, overweight, moderate and severe obese hypertensives, also when sex and smoking habits were considered. Our data indicated a strong association between obesity and hypertension. In addition they may be consistent with the suggestion that obese hypertensives were not characterized by a lower risk of ischaemic heart disease (IHD), than lean hypertensives(7).

6. Lack of physical activity
In the study to investigate the relative impact of physical fitness, physicaldemands at work, and physical activity during leisure time on ischemic heart disease (IHD) and all-cause mortality among employed men with pre-existing cardiovascular disease (CVD), showed that among gainfully employed men with pre-existing CVD, a high physical fitness was associated with a substantially reduced risk for IHD and all-cause mortality(8).

7. Smoking and other tobacco use
Some researchers suggested that there is a stronger association between smokingand peripheral arterial disease than ischemic heart disease does not appear to be influenced by the other risk factors(9)

8. Family history of heart disease
Literature suggests that inherited factors are important in the development of premature ischemic heart disease, but decline in importance with age(10)

9. Excessive drinking
In the study to investigate the relationship between alcohol consumption and the prevalence of the metabolic syndrome (MetS), type 2 diabetes mellitus (DM),coronary heart disease (CHD), stroke, peripheral arterial disease (PAD), and overall cardiovascular disease (CVD) in a Mediterranean cohort, showed that Heavy drinking was associated with an increase in the prevalence of all of thesedisease states(11)

10. Unhealthy diet
Diet high in saturated fat and trans fat can lead to cholesterol building up in the arterial wall of that can decrease blood flow and oxygen to the heart muscle.

11. Stress, coping, and social support as psychosocial factors
Data on stress (Stress Appraisal Scale), coping strategies (Ways of Coping Questionnaire), and social network/social support (Norbeck Social Support Questionnaire) were collected from 100 persons with the primary diagnosis of IHD prior to discharge from hospital, in the study ofStress, coping, and social support as psychosocial factors in readmissions for ischaemic heart disease, indicated that greater use of the coping strategy 'seeking social support' was associated with the readmission of persons who had had their first admission for IHD. Less use of the coping strategy 'accepting responsibility' was associated with the readmission of persons who had a history of prior admission for IHD(12).

12. Sickle-cell disease
Sickle cell disease (SCD) is an inherited chronic haemolytic anaemia whose clinical manifestations arise from the tendency of the haemoglobin to polymerize and deform red blood cells into the characteristic sickle shape due to a single nucleotide change in the β-globin. Sickle cell anaemia (SCA) is associated with recurrent multi-organ ischemia and infarction. Myocardial ischemia (MI) and infarction are increasingly recognised as features of SCA(13)

13. Shift work and metabolic syndrome
Other researchers suggested that Shift work and metabolic syndrome also associated to increased risk of Ischemic heart disease(14).
I would like to summarize this section by quoting the study by Dr. Fowkes FG and the research team at University of Edinburgh, "Multiple regression of risk factors on measures of peripheral arterial disease showed associations with diabetes mellitus (but not impaired glucose tolerance), systolic blood pressure, andserum cholesterol; inverse association with high-density lipoprotein cholesterol; and only univariate association with triglycerides. In multiple logistic regressions of risk factors on six separate indicators of cardiovascular disease, the only consistent difference was that smoking increased the risk of peripheral arterial disease (range of odds ratios, 1.8-5.6) more than heart disease (range of odds ratios, 1.1-1.6)"(15).
