Diels, Eclipta prostrata L., and Tagetes erecta L. tested against malarial vector, found that all plant extracts showed moderate adulticidal activity and EI effects after 24 h of exposure at 1,000 ppm; however, the highest adulticidal activity was observed in ethyl acetate extract of A.lineata, chloroform extract of A. paniculata, acetone extract of C. hirsutus, and methanol extract of T. erecta (LD(50) = 126.92, 95.82, 109.40, and 89.83 ppm, according to “Efficacy of medicinal plant extracts against malarial vector, Anopheles subpictus Grassi” by Elango G, Rahuman AA, Kamaraj C, Bagavan A, Zahir AA.(1).

In the investigation of the antiparasitic activities in determination of the efficacies of leaf hexane, chloroform, ethyl acetate, acetone and methanol extracts of Aegle marmelos (Linn.) Diels, Eclipta prostrata L., and Tagetes erecta L. against the adult cattle tick Haemaphysalis bispinosa Neumann 1897 (Acarina: Ixodidae), indicated that the highest parasitic activity was found in leaf ethyl acetate extract of A. lineata, methanol extract of A. marmelos, A. paniculata, and C. hirsutus against H. bispinosa (LC(50) = 395.27, 358.45, 327.21 and 420.50 ppm); ethyl acetate extract of A. paniculata, C. hirsutus, methanol extracts of A. marmelos, A. lineata, and E. prostrata against the larvae of R. microplus (LC(50) = 207.70, 258.61, 134.09, 206.00, and 274.33 ppm); hexane extract of A. lineata, ethyl acetate extract of A. paniculata, E. prostrata, acetone extracts of T. erecta, methanol extracts of A. marmelos and C. hirsutus against P. cervi (LC(50) = 254.23, 451.17, 425.73, 253.60, 542.71, and 360.17 ppm), respectively, according to “Evaluation of medicinal plant extracts against ticks and fluke” by Elango G, Rahuman AA.(2).

In the elucidation of the biological basis for hypolipidemic and antioxidant activities of E. prostrata, indicated that the results clearly demonstrate the effects of E. prostrata on serum lipid and oxidative metabolism in rats. After recording the past and present history and completing a physical exam, including searching the nodule in the surrounding tissue and abnormal lymph nodes nearby. The tests which your doctor orders may include
1. Blood test
The aim of the test is to the level of free T4 and thyroid stimulating hormone (TSH) levels. Circulating analytes used in the exploration of thyroid function are TSH and free thyroid hormones (FT4 and FT3). TSH is used as first line analysis in diagnosis because a normal value excludes almost always a thyroid dysfunction. However in the follow up of a treatment for hypo- or hyperthyroidism, free hormones are to be determined since TSH reaction is delayed and does not reflect precisely the restoration of euthyroidism. The measurement of anti-thyroperoxidase antibodies (anti-TPO) will show the presence of an autoimmune thyroiditis and that of thyroid stimulating antibodies (TSI) will establish the diagnosis of Graves’ disease. The measurement of circulating thyroglobulin has no place in the diagnostic of thyroid dysfunction nor in the evaluation of a goitre but it is now the golden standard in the follow up of patients with a differentiated thyroid cancer after surgery and radioiodine ablation in patients without antithyroglobulin antibodies, according to the Services de Diagnostic et de Traitement par Isotopes(25).

2. Echography and Thyroid scintigraphy
Echography is the first line examination to evaluate thyroid morphology. It shows tiny thyroid nodules and gives precious informations on their structure, as well as paremchymatous diffuse abnormalities associated with thyroiditis. Thyroid scintigraphy with 99mTc allows establishing the functional characteristics of thyroid nodules (warm or cold) and to precise the origin of a thyrotoxicosis (autonomous toxic nodule vs Graves-Basedow, sub acute or silent thyroiditis). Whole body scintigraphy is mandatory after radioiodine treatment with 131I in order to visualize possible metastasis and establish their avidity for iodine(25).

3. Ultrasonography
All ultrasound examinations for thyroid nodule should include a malignancy risk assessment based on the markedly hypoechoic nature of the nodule, presence of microcalcifications, ill-defined margins, nodule with shape taller than wide and intra-nodular hypervascularity at color Doppler. In patients with multinodular thyroid gland, precise nodule mapping is necessary to allow accurate follow-up of each nodule, correctly identify which nodule(s) is hyper functioning on iodine scan (if done) and guide fine needle aspiration (FNA) of suspicious nodules(26).

4. Fine-needle aspiration biopsy (FNA) and Core needle biopsies (CNBs)
The most commonly used diagnostic method for the preoperative diagnosis of thyroid nodules is ultrasound-guided fine-needle aspiration biopsy (FNA), which often yields non-diagnostic or non-definitive results and seldom produces definite malignant diagnoses. Core needle biopsies (CNBs) may be beneficial for the diagnosis of papillary thyroid carcinoma and other non-follicular thyroid lesions. CNB may be considered as an additional diagnostic procedure in cases with FNA suspicious for malignancy(27).

Interestingly, Pa-EE markedly inhibited NF-κB, activator protein (AP)-1, and cAMP response element binding protein (CREB) activation; further, according to direct kinase assays and immunoblot analyses, Pa-EE blocked the activation of the upstream signaling molecules spleen tyrosine kinase (Syk), p38, and transforming growth factor β-activated kinase 1 (TAK1). Finally, orally administered Pa-EE clearly ameliorated EtOH/HCl-induced gastritis in mice(37).

2. Cinnamomum cassia
Cinnamomum cassia Blume (Aceraceae) has been traditionally used to treat various inflammatory diseases such as gastritis. According to the study by the Sungkyunkwan University, 95% ethanol extract (Cc-EE) of Cinnamomum cassia exerts strong anti-inflammatory activity by suppressing Src/Syk-mediated NF-κB activation, which contributes to its major ethno-pharmacological role as an anti-gastritis remedy. Future work will be focused on determining whether the extract can be further developed as an anti-inflammatory drug(38).

In the study to examine the association between consumption of sugar-sweetened beverages and weight change and risk of type 2 diabetes in women, found that those with stable consumption patterns had no difference in weight gain, butweight gain over a 4-year period was highest among women who increased their sugar-sweetened soft drink consumption from 1 or fewer drinks per week to 1 or more drinks per day (multivariate-adjusted means, 4.69 kg for 1991 to 1995 and 4.20 kg for 1995 to 1999) and was smallest among women who decreased their intake (1.34 and 0.15 kg for the 2 periods, respectively) after adjusting for lifestyle and dietary confounders. Increased consumption of fruit punch was also associated with greater weight gain compared with decreased consumption. After adjustment for potential confounders, women consuming 1 or more sugar-sweetened soft drinks per day had a relative risk [RR] of type 2 diabetes of 1.83 (95% confidence interval [CI], 1.42-2.36; P<.001 for trend) compared with those who consumed less than 1 of these beverages per month. Similarly, consumption of fruit punch was associated with increased diabetes risk (RR for > or =1 drink per day compared with <1 drink per month, 2.00; 95% CI, 1.33-3.03; P =.001).(1).

                       Misdiagnosis

1. Hyalinizing trabecular tumors (HTT)
Hyalinizing trabecular tumors (HTT) are very rare thyroid tumors whose diagnosis is difficult, particularly in cytology. There is a report of two HTT cases which were initially diagnosed as papillary carcinoma, on the preoperative cytology and for which the patients underwent total thyroidectomy with lymph node dissection(18).

2. Totally cervical thymoma
Totally cervical thymoma is extremely rare and usually arises from ectopic thymic tissue. We report a case of a B1 thymoma localized in the neck, misdiagnosed for a decade as a thyroid nodule(19).
3. Papillary thyroid carcinoma (PTC).

There is a report of two cases diagnosed as papillary thyroid carcinoma (PTC) by FNA cytology, but histopathology reports indicated colloid goiters with infarcted nodules. Review of histopathologic material showed features of PTC in the viable tissue at the periphery of nodules. Immunohistochemical study for galectin-3 and CD44 in 1 of the cases supported the diagnosis of PTC(20).

4. Pharyngoesophageal diverticulum
A pharyngoesophageal diverticulum can be mistaken for a posteriorly placed “thyroid nodule” on ultrasound scan if the subtle differentiating signs are missed. An awareness of this condition is important to avoid unnecessary needle biopsies, according to the study by the Alexandra Hospital, Singapore(21).

5. Primary thyroid paraganglioma
There is a report of thyroid PG case to highlight importance of careful evaluation of clinical and pathological findings to correctly identify paragangliomas which anatomically mimic MTCs(22).

6. Anaplastic thyroid cancer
There is a report of a case of Riedel’s thyroiditis that had presented rapidly growing, hard, fixed, thyroid mass mimicking anaplastic thyroid cancer in a 41-year-old female patient with longstanding benign thyroid nodule for 6 years(23).

7. Hodgkin’s lymphoma
Hodgkin’s lymphoma of the thyroid is rare and can mimic a primary thyroid epithelial tumor or thyroiditis clinically(24).
