                                          Premenstrual syndrome(PMS)

Premenstrual syndrome is defined as faulty function of the ovaries related to the women's menstrual cycle, effected over 70% to 90% of women in the US and lesser for women in Southeast Asia because of difference in living style and social structure. The syndrome also interferes women's physical and emotional states, and daily activities as a result of hormone fluctuation and occurs one to two weeks before menstruation and then declines when the period starts.

       The Effects of Phytoestrogens

Phytoestrogens also known as dietary estrogen, are non steroidal plant compounds, having a structural similarity of estradiol with ability to promote estrogenic(1)(2) and anti estrogenic(3) effects.

The effects
1. Hormone balancing
As we mentioned in the definition, phytoestrogens in the plant have the ability to lower levels of estrogen if it is too high and higher the levels of estrogen if it is too low(3)(4). Since most women with pre-mentsrual syndrome always have some kinds of hormone imbalance(6), intake of photyestrogens not only restores the balance, but also reduces the symptoms of PMS, such as headache and breast tenderness(5).

2. Hormone stimulating
Phytoestrogens also have the ability to regulating the levels of sex hormone binding globulin SHBG(7) circulating in the bloodstream. SHBG is produced by liver in fat and protein metabolism(7) and the right levels of SHBG help to reduce symptoms of PMS(8).

3. Menstrual cycle
Intake of phytoestrogens induced reduction of over production of estrogen(1)(2)(3) causes of mammary cancers(10)(11) through increased the length of menstrual cycle(9).

4. Liver booster
Phytoestrogen also increases the liver function(13) in regulating the levels of prostaglandins hormone by fat and protein metabolism, thus lessening the menstrual pain and cramps(12)(14) for some women with PMS.

                    Overcome Infertility

Infertility is defined as inability of a couple to conceive after 12 months of unprotected sexual intercourse. It effects over 5 millions couple alone in the U. S. and many times more in the world. Because of unawareness of treatments, only 10% seeks help from professional specialist.

           Psychological Causes of Infertility

1. Slimness effect
The obsession with slimness at all cost may interferes the production of the gonadotropin-releasing hormone which not only causes delay or abnormal sperm and eggs development but also interferes with primitive process of natural selection in the brain leading to absence of period as blood redirected to other organs need causing nutrients deficiency.
2. Failure to conceive
Many couple after several tries to conceive but fail or just having a miscarriage, feel dramatic pressure and stress which cause hormone imbalance and interfere with the production of sperm in men and normal cycle in women resulting in increasing the risk of inability to conceive.
3. Stress
Stress interferes with hormone production ( increasing the secretion of adrenaline hormone that suppress the production of progesterone which is necessary for the soften of uterine lining for fertilized egg implantation) increasing the risk of absence of ovulation leading to infertility in women. In men, it causes low sperm count and reduce quality of sperm.
4. Depression
Depression increases the production of certain hormone in the nervous system and the glands leading to hormone imbalance and interferes the normal procedure of menstrual cycles in women and lowering sperm count in men, leading to infertility.
5. Medical problem
Some medication may have side effects leading to nervous tension and the risk of malfunction of some organs in the the body such as hypertension medication has an effect to kidney function, causing psychological problem such as stress and anxiety.
e) Cervical Causes of Infertility
I. Definition
Cervix is a lower neck of uterus, it joins on top with the uterus and the bottom with the vagina. It normally is hostile to the sperm with thick and sticky mucus and only produces sperm friendly mucus a few day before ovulation to invite sperm invasion for conception.

Visceral or subcutaneous fat accumulation is fat accumulated in the abdominal area, leading to higher risk of insulin resistance, and its concomitant metabolic disorders (the metabolic syndrome) including atherogenic dyslipidemia(1), in comparison to those with less visceral fat accumulation.

Regular physical activity has long been found to associate to the prevention of cancer, cardiovascular disease, and other chronic diseases, but regular walking contribution of reduced risk of visceral or subcutaneous fat accumulation in the younger and older elderly have been lacking, probably due to its association to complication of obesity(2).

The study of total of 173 sedentary, overweight (body mass index > or =24.0 and >33% body fat), postmenopausal women aged 50 to 75 years who were living in the Seattle, Wash, area, suggested, Walking reduced body weight and body fat among overweight and obese postmenopausal women.as it was the most frequently reported activity in the group(3).

Dr. Lynch NA and colleagues at the University of Maryland School of Medicine insisted that walking not only is associated with greater reductions in visceral adiposity in obese postmenopausal women but also improved in maximal O(2) uptake (VO(2 max)(4) of that is said to significantly improve the metabolic profile(5).

In fact, regular walking is found to reduce risk of visceral or subcutaneous fat accumulation as it prevents the early state of weigh gain.

The Okayama Southern Institute of Health in the study of Thirty one obese Japanese males (body mass index (BMI) > or = 25) ranging in age from 32 to 59, participated in a 1-year follow up study said, " intra-abdominal visceral adipose tissue is critically involved in insulin resistance and daily walking rather than improvement of exercise capacity correlated with the reduction of visceral adipose tissue in obese Japanese males."(6). As walking significantly lowers serum leptin concentrations(8), and thus it may improve the leptin resistance observed in overweight adults(7).

Health Benefits
1. Neuroprotective and antioxidant effects
In the investigation of the effects of the ethyl acetate (EA) fraction on various types of neuronal cell damage induced in primary cultured rat cortical cells, found that the EA fraction potently inhibited the neuronal damage induced by arachidonic acid. We also found that it significantly attenuated the neuronal damage induced by spermine NONOate, a stable NO generator. In addition, it inhibited the A(beta(25-35))-induced neurotoxicity and glutamate- or N-methyl-D-aspartic acid-induced excitotoxicity. It was found that the oxidative neuronal damage induced by H2O2, xanthine/xanthine oxidase, or Fe(2+)/ascorbic acid was also inhibited by the EA fraction. Furthermore, it was shown to inhibit lipid peroxidation initiated by Fe(2+)/ascorbic acid in rat brain homogenates, and scavenge DPPH radicals(1).

2. Carcinogenic activity
In the evaluation of the carcinogenicity of young, pre-blooming flowers of coltsfoot, Tussilago farfara L., in inbred strain ACI rats, divided into 4 groups: Group I received 32% coltsfoot diet for 4 days, and subsequently 16% diet until the termination of experiment. Groups II and III respectively received 8% and 4% coltsfoot diet for 600 days. Group IV was fed a normal diet as a control group, found that after 600days, all the rats in Group I survived beyond 380 days after the start of feeding and 8 out of 12 rats developed hemangioendothelial sarcoma in the liver, whereas only one out of 10 rats developed the tumor in Group II, and no tumors were observed in Group III. Chemical studies on the dried, young flowers used in this experiment suggested that the carcinogenicity of coltsfoot is most probably due to senkirkine, a hepatotoxic pyrrolizidine alkaloid(2).

3. Anti inflammatory effects
In the demonstration of that TSL, Tussilagone (TSL), isolated from the flower of buds of Tussilago farfara (Compositae),exerts anti-inflammatory activities in murine macrophages by inducing heme oxygenase-1 (HO-1) expression, found that TSL may be an effective HO-1 inducer that has anti-inflammatory effects, and a valuable compound for modulating inflammatory conditions(3).

4. Antimicrobial activity
In the investigation of the antimicrobial activity of crude ethanolic extracts of 16 Siberian medicinal plants, showed that 12 showed antimicrobial activity against one or more species of microorganisms. The most active antimicrobial plants were Bergenia crassifolia, Chelidonium majus, Rhaponticum carthamoides, Sanguisorba officinalis, and Tussilago farfara(4).

                           Amenorrhea

All women are experience irregular period sometimes during their child bearing years, before menopause. While Western medication has never viewed that irregular menstruation is a problem of women reproductive system, traditional Chinese medicine looks at this problem seriously, if untreated, it will disharmonize the women ecosystem, leading to nervous tension and other health problems, including infertility. Imbalance of liver is defined is a health condition of which liver is no longer function normally in moving blood in the blood vessels and govern smooth flow of the emotion and stress due to the affect of excessive anger or unexpressed anger, leading to liver being damaged that affects the regular menstruation and fertility.
Fire effulgence is defined as a health condition of yin vacuity as a result of yin deficiency over a prolonged period of time and the depletion of yin essence, leading to yin/yang imbalance and yin can not control yang, due to aging, frequency sexual activity, repeat masturbation, etc resulting in amenrrhea in women.

2. Unhealthy diet
Women who love to diet with high level of saturated fat and trans fat as well as drinking cold drink comes directly from the refrigerator are most likely to suffer kidney yin deficiency caused by excessive yang produced in the stomach's function in food absorption.

3. Spleen qi deficiency
Since spleen in charge of transformation qi and fluid in the body and to the other organ, damage of the spleen or stomach inability to provide enough yin for the productive organ to perform its normal menstrual cycle, cause prolonged yin phrase.

4. Aging
Since women are classified as yin in nature, aging causes depletion of kidney essence -the true yin in the body, leading to scanty period or amenorrhea and early menopause.

Obesity is a medical condition of excess body fat accumulated overtime, while overweight is a condition of excess body weight relatively to the height. According to the Body Mass Index(BMI), a BMI between 25 to 29.9 is considered over weight, while a BMI of over 30 is an indication of obesity. According to the statistic, 68% of American population are either overweight or obese.

You can calculate your BMI index BMI= weight (kg)/ height (m2)

The Studies of  Obesity and reproductive performance in women

Obesity and overweight are not only problem of developed nations but also increasing problem in developing countries with health consequence. According to the study by the Association for Reproduction and Family Health, posted inPubMed, found that obesity contribute majorly to two most common medical risks in pregnancy: diabetes and hypertension. Is is associated with increasesd risk of large-for-gestational age and hypertensive disorders, including pre-eclampsia, which may be associated with low birth weight. Combination of factors viz individual lifestyle, environmental and genetic play role in obesity. Weight loss has demonstrated to improve fertility in obese women through recovery of spontaneous ovulation, and improved response to ovarian stimulation in infertility treatment.
