                                          Premenstrual syndrome(PMS)

Premenstrual syndrome is defined as faulty function of the ovaries related to the women's menstrual cycle, effected over 70% to 90% of women in the US and lesser for women in Southeast Asia because of difference in living style and social structure. The syndrome also interferes women's physical and emotional states, and daily activities as a result of hormone fluctuation and occurs one to two weeks before menstruation and then declines when the period starts.

Causes of Breast Tenderness

Breast tenderness effects more than 70% of women in the US alone. Sometimes, it is so painful that some women afraid to be hugged and are experience uncomfortable during sleep.

1. Caffeine
Caffeine is a stimulant, containing methylxanthines, a chemical exhibited the symptoms of PMS(4) including breast tenderness(5)(6), breast pain(1)(2) and induced fibrocystic breast disease(2)(3). Reducing intake of foods containing caffeine such as coffee, tea 3 to 4 days before period may help to reduce the symptoms of breast tenderness and pain(1)(4).

2. Stress
Prolactin produced abundant by during baby feeding(9), may also be released during prolong period of stress(7)(8) to cause breast tenderness(10)(11)(12) for some women with PMS.

2. Hormone imbalance
Some women may experience breast changes during menstrual cycle(13). The problem is linked to the cycle, caused by the fluctuation of levels of estrogen and progesterone as resulting of hormonal imbalance(14).

3. Sluggish Liver
Liver is important in carbohydrate(15), fat(16) and protein(17) metabolism. Sluggish liver causes abnormal function of liver in regulating the levels of Omega 3 and 6 fatty acids(18), and induced imbalance of levels of PGE2 induced breast tenderness and pain(19)(20).

4. Nutrients deficiency
a)Vitamins and minerals deficiency
For what ever reason, women with pre menstrual syndrome are found to have low levels of micronutrients(22)(23)(24), of that can lead to over production of levels of PGE2(24) which causes PMS including breast tenderness and pain(19)(20).

b) Omega 3 fatty acid deficiency
As we mention before, Omega 3 fatty is important to inhibit the levels of PG2(16). Deficiency of Omega 3 fatty acid causes high levels of PGE2(16) in the bloodstream induced breast tenderness and pain(19)(20).

5. Prostaglandins hormone
Balancing of prostaglandins hormone is vital in regulating the uterus muscles(21) during menstrual cycle. If the levels of PGE2(19)(20) of the prostaglandins is getting too high, it may cause breast tenderness and breast pain(19)(20) for women with PMS.

           Attention Deficit Disorder (ADHD/ADD)

Attention Deficit Disorder (ADHD/ADD) also known as attention deficit hyperactivity disorder is a chronic disease occurs in certain age and years of a children, defined as a condition with characteristics of co-existence of attentional problems and hyperactivity. It effects about 5% of the all children in the world with boys have 3-4 times higher risk than girl. Over one third of children with ADHD will have continued symptoms existed into their adult life.

Diseases with similar symptoms to ADHD

A number of other conditions and disorders have similar symptoms, therefore, it is imporant for the doctors to rule out before treating ADHD
1. Visual disturbances
Visual disturbances are defined as a condition of abnormalities of sight associated with neurological disorders often include double vision (diplopia), moving or blurred vision as a result of nystagmus, a rapid involuntary movements of the eyes. Some children with ADHD are experience the symptoms of visual disturbances as a result of neuropsychological dysfunction.

2. Thyroid disorders
Children who are experience the symptoms of dementia and depression, dysfunction of cerebellum and cranial nerves as a result of hypothyroidism (lacks sufficient thyroid hormone) may be risk of misdiagnosis as the symptoms are similar of those in ADHD (10)

3. Epilepsy
Epilepsy is defined as condition of chronic neurological disorders characterized by seizures and with recurrent symptoms as a result of mutations in several genes. ADHD occurs frequently in children with epilepsy. The factors associated with increased risk of ADHD include the onset age of epilepsy, the types of seizures or epileptic syndromes, the epileptiform EEG discharges, and the effects of antiepileptic drugs(11)

4. Sleeping problems, and attention-deficit/hyperactivity disorder.
Some children with ADHD may be experience the symptoms of childhood eczema, sleeping problems, but not all of them. Researchers at University Clinic of Wuerzburg indicated that ADHD and AE appear to be strongly and independently associated in children with sleeping problems, but not in children without sleeping problems. A substantial part of diagnoses met for ADHD might be engendered by the presence of AE and concomitant sleeping problems.(12)

5. Some genetic disorders
Researchers at the Complejo Hospitalario Universitario de Albacete found that the X-chromosome-linked ichthyosis is caused by mutation or deletion of the STS gene as. Depending on its size, it can present as an isolated entity or combined with a syndrome caused by neighbouring genes, thus associating itself with other monogenic diseases as well as other mental disorders. The most relevant findings from the literature review are the importance of the Xp22.3-pter region and the higher incidence of neurological disorders among males such as attention deficit hyperactivity disorder, autism and X-linked mental retardation(13)

6. Eenuresis
Nocturnal enuresis (MNE) or bedwetting is defined as condition of involuntary urination while asleep inchildren at which bladder control usually occurs. Researchers at Duzce University School of Medicine said that monosymptomatic nocturnal enuresis (MNE) and attention deficit and hyperactivity disorder (ADHD) are multifactorial disorders and biological, social, and psychological factors may play significant roles in the development of both. Children with enuresis display a higher prevalence of ADHD compared to the normal population, in a study of A total of 64 children between the ages of 6 and 13 years (14)

7. Schizophrenic
Schizophrenia is defined as a serve mental disorder characterized by a breakdown of thought processes and by poor emotional responsiveness, including symtoms similar to those of ADHD.
and both associated with deficits in inhibition and working memory, but the hypothesis that working memory is a primary deficit in schizophrenia, but secondary to the inhibitory deficit in ADHD.(15)

8. Substance Use Disorders
In the replied to the article by Timothy E. Wilens, M.D. in The American Journal of Psychiatry, VOL. 164, No. 6, the writer wrote that the case presented does exhibit symptoms typical of marijuana users (4—6). According to Fletcher et al. (7), long-term cannabis use is associated with disruption of short-term memory, working memory, and attentional skills, all of which are exemplified in the case presented in Dr. Wilens’s article (16)

9. Autism and ADHD are characterized with the association of neurodeficits
Even though there are allots of similar symptom, but children with ASD also showed slower rates of adaptation compared with both control groups while children with ADHD did not show this excessive generalization to the proprioceptive target, but they did show excessive variability in the speed of movements with an increase in the exponential distribution of responses (τ) as compared with both TD children and children with ASD.(17)

10. Tourette's Syndrome
Tourette syndrome (TS) is defined as a neurological disorder characterized by repetitive, stereotyped, involuntary movements and vocalizations called tics. Some time it is misdiagnosis to children with ADHD or vice versa, but there are the evident that ADHD are more common in patient with chronic tics and Tourette's syndrome(18)

k. Oppositional Defiant Disorder (ODD)
ODD is a psychiatric disorder that is characterized as a set of symptoms, including aggressiveness and a purposefully bother and irritate others. ODD may sometime associate with ADHD and other neuropsychiatric disorders including, depression, Tourette's syndrome, etc..and sometime they can be treated with the same medication and at different improvement rate(19)

11. Anxiety Disorders
Anxiety disorder is defined as a condition of several different forms of a type of mental illness of abnormal and pathological fear.. In the study of children (8-17 years) with anxiety disorders (n = 24) and children (8-16 years) with ADHD (n = 23) on neurocognitive tests of both general and emotion-based attention processes, showed that children with ADHD demonstrated poorer selective and sustainedattention, whereas youth with anxiety disorders demonstrated greater attentional bias toward threatening faces on a visual probe task.(20)

12. Depressive Disorders
Depressive disorders is defined as a metal illness of which includes mood, and thoughts of the patient, affecting their way of eating and sleeping, feeling, etc. with the prevalence rate for MDD is 6.7%, while the ADHD prevalence rate in US adults is 4.4%.(21)

Regular physical activity has long been found to associate to the prevention of cancer, cardiovascular disease, and other chronic diseases, the joint study led by Universidade Federal de Minas Gerais, showed that regular physical activity (PA) induces desirable changes in plasma levels of high- and low-density lipoproteins (HDL and LDL, respectively) and triglycerides (TG), important risk factors for cardiometabolic diseases(1). But regular walking contribution of reduced risk of atherosclerosis  in the younger and older elderly have been lacking, probably due to the effectiveness of medication in reduced cholesterol build up on the inner wall of arteries.

Atherosclerosis is a condition of hardening and narrowing of the arteries as a result of plaques of fatty cholesterol on inner wall, leading to reduce oxygen-rich blood flow to the organs and other parts of the body.
According to the joint study led by the University of Shizuoka, off-site walking program of a prospective, 12-week, randomized and controlled study of Fifty-two women aged 32 to 57 years suggested, the program may decrease fibrinogen levels which was found to be a trigger cardiac events in patients with atherosclerosis(3).

In fact, regular leg exercise not only reduced the complications of patients with atherosclerosis(3) but also induced expansive arterial remodelling in corresponding improvements in risk factors for atherosclerosis in healthy men of of different ages(4). Dr. Rauramaa R and colleagues in a randomized clinical trial in overweight men, insisted that regular exercise (walking to slow jogging) significantly inhibited secondary platelet aggregation from 27% to 36% of that induced protective effects in the initiation of atherosclerosis(5).

Dr. Kovacić L said, "The criteria for physical active person were walking three times a week for 30 minutes at least .... in prevention of many health problems, like obesity, arterial hypertension, diabetes, hearth diseases, atherosclerosis, osteoporosis and others"(2).

In support of regular exercise in lowering the risk of Atherosclerosis, Dr. Ali Al-Mamari said, "physical activity has been recognized as a preventive measure for atherosclerosis" and " moderately vigorous exercise resulted in a substantial reduction in mortality from all causes by 23% and from CHD by 41% compared with sedentary classmates. Maintaining or increasing physical activity level in late middle age was associated with a reduction in mortality rates, and light activities appeared to be sufficient to produce this benefit in older men"(6), probably due to its contribution in the reduced abnormalities in lipid and lipoprotein metabolism(7) and serum TG concentration(8).

                                      Sarsaparilla

Sarsaparilla is a perennial, trailing vine, in the genus Smilax, belonging to the family Smilacaceae, native to the Central America. The herb has been used in traditional medicine to treat skin inflammation, psoriasis, arthritis, herpes, and leprosy, reduce symptoms of eczema, etc.

Health Benefits
1. Anti-mycobacterial effects
In the identifyication ofantimycobacterial constituents from Aralia nudicaulis rhizomes, a wild sarsaparilla, found that Falcarinol and panaxydol were identified as the principal constituents responsible for the antimycobacterial activity of Aralia nudicaulis rhizomes validating an ethnopharmacological use of this plant by the Canadian First Nations, according to "Anti-mycobacterial diynes from the Canadian medicinal plant Aralia nudicaulis" by Li H, O'Neill T, Webster D, Johnson JA, Gray CA.(1)

2. Anti-cancer effects
In the investigation of Smilax glabra Roxb. (SGR)inhibited growth of human breast cancer cell line MCF7, colon carcinoma cell line HT-29, and gastric cancer cell line BGC-823 in a dose-dependent manner, found that the changes in expression profiles of genes related to apoptosis, proliferation and cell cycle control in the cells treated with SGR. Our results demonstrated the mitochondrial regulation of apoptosis by which SGR exerts the anti-cancer effect, according to "Mitochondrial apoptosis contributes to the anti-cancer effect of Smilax glabra Roxb" by Gao Y, Su Y, Qu L, Xu S, Meng L, Cai SQ, Shou C.(2)

3. Hyperuricemia and nephropathy
In the study of astilbin, a flavonoid compound isolated from the rhizome of Smilaxchina L. and its effects on hyperuricemia and nephropathy rats found that astilbin significantly decreased the serum uric acid (Sur) level by increasing the urinary uric acid (Uur) level and fractional excretion of urate (FEUA) but not inhibiting the xanthine oxidase (XOD) activity. In addition, kidney function parameters such as serum creatinine (Scr) and blood urea nitrogen (BUN) were recovered in astilbin-treated hyperuricemic rats. Further investigation indicated that astilbin prevented the renal damage against the expression of transforming growth factor- β1 (TGF-β1) and connective tissue growth factor (CTGF) and also exerted a renal protective role by inhibiting formation of monosodium urate (MSU) and production of prostaglandin E₂ (PGE₂) and interleukin-1 (IL-1), according to "Astilbin attenuates hyperuricemia and ameliorates nephropathy in fructose-induced hyperuricemic rats" by Chen L, Lan Z, Zhou Y, Li F, Zhang X, Zhang C, Yang Z, Li P.(3)

4. Anticonvulsant activity and neurotoxicity
In the determination of the anticonvulsant activity and neurotoxicity of ethanolic extract and ethyl acetate fraction of the rhizome of Smilax china (EESC and EAF, respectively) in mice, found that MES test was reduced significantly (P < 0.001) by EESC at a dose level of 400 mg/kg and EAF at both higher dose levels (200 and 400 mg/kg). In PTZ model, the seizure latency was prolonged by all the test groups, according to " Anticonvulsant and neurotoxicity profile of the rhizome of Smilax china Linn. in mice" by Vijayalakshmi A, Ravichandiran V, Anbu J, Velraj M, Jayakumari S.(4)

5. Lung cancer
In the study of furostanol saponins and theirs cytotoxicity effect, found that The isolated furostanol saponins were evaluated for cytotoxic activity against human normal amniotic and human lung carcinoma cell lines using neutral red and MTT assays. In vitro experiments showed significant cytotoxicity in a dose dependent manner with IC(50) values in the range of 32.98-94.53 µM, according to 'New furostanol saponins from Smilax aspera L. and their in vitro cytotoxicity" byIvanova A, Mikhova B, Batsalova T, Dzhambazov B, Kostova I.(5)

6. Breast cancer
In the investigation of the breast tumor cell toxic components in S. china L. continuously and systematically, found that among these six polyphenols, five (1, 3-6) were reported for the 1st time with in vitro activities on anti-breast tumor cell. It is likely that these polyphenols are the active components of S. china L. responsible for the anti-breast tumor cell activities, according to 'Cytotoxic polyphenols against breast tumor cell in Smilax china L" by Wu LS, Wang XJ, Wang H, Yang HW, Jia AQ, Ding Q.(6)

7. Diabetes
In the investigation of the efficacy of astilbin on experimental diabetic nephropathy (DN) in vivo and in vitro and its possible mechanisms, found that astilbin inhibition of CTGF may be a potential target in diabetic nephropathy (DN) therapy. This work provides the first evidence for astilbin as a new candidate of DN therapeutic medicine, according to "Effect of astilbin on experimental diabetic nephropathy in vivo and in vitro" by Li GS, Jiang WL, Yue XD, Qu GW, Tian JW, Wu J, Fu FH.(7)

8. Anti-inflammatory activities
In the identification of Smilax china Linn. used in traditional Chinese medicine (TCM) as well as in Pakistan for its effect on anti inflammatory disorders, found that Sieboldogenin seems to be a potential new anti-inflammatory compound responsible for anti-inflammatory activities of Smilax china Linn. Its in vitro and in vivo inflammatory activities are in good agreement with the folk medicinal use ofSmilax china Linn. in inflammatory disorders, according to "Anti-inflammatory activities of Sieboldogenin from Smilax china Linn.: experimental and computational studies" by Khan I, Nisar M, Ebad F, Nadeem S, Saeed M, Khan H, Samiullah, Khuda F, Karim N, Ahmad Z.(8)

9. Anti Herpes simplex virus type 1 (HSV-1)
In the evaluation of the glycoproteins possessing antiviral and anti-proliferative activities were isolated from the Chinese medicinal herb Smilax glabra, found that the glycoprotein potencies for antiviral activity appeared to depend on the molecules' binding affinity for fetuin, that is, the fetuin-binding protein was more potent than the non-fetuin binding proteins, according to "Antiviral and anti-proliferative glycoproteins from the rhizome of Smilax glabra Roxb (Liliaceae)" by Ooi LS, Wong EY, Chiu LC, Sun SS, Ooi VE.(9)

10. Liver cancer and disease
In the determination of the raw SGR plant extracted with Accelerate Solvent Extractor anti-proliferative effect on the human hepatoma cell lines, HepG2 and Hep3B, found that SGRE inhibited HepG2 and Hep3B cell growth by causing cell-cycle arrest at either S phase or S/G2 transition and induced apoptosis, as evidenced by a DNA fragmentation assay. SGRE-induced apoptosis by alternation of mitochondrial transmembrane depolarization, release of mitochondrial cytochrome c, activation of caspase-3, and cleavage of poly(ADP-ribose) polymerase, according to "Anti-proliferative and pro-apoptotic effect of Smilaxglabra Roxb. extract on hepatoma cell lines" by Sa F, Gao JL, Fung KP, Zheng Y, Lee SM, Wang YT.(10)

11. Dermatitis
In the assessment of astilbin for the treatment of human immune diseases, found that 3'-O-methylated astilbin, a new metabolite of astilbin and isolated it from the culture solution nhibited picryl chloride-induced ear swelling in mice and suppressed the expression of tumor necrosis factor-alpha and interferon-gamma, similarly to astilbin, according to "Identification of a new metabolite of astilbin, 3'-O-methylastilbin, and its immunosuppressive activity against contact dermatitis" by Guo J, Qian F, Li J, Xu Q, Chen T.(11)

12. Adjunctive arthritis(AA)

                                  Overcome Infertility

Infertility is defined as inability of a couple to conceive after 12 months of unprotected sexual intercourse. It effects over 5 millions couple alone in the U. S. and many times more in the world. Because of unawareness of treatments, only 10% seeks help from professional specialist.

                              Uterine Causes of Infertility

The uterus is female hormone-responsive reproductive sex organ. It is where the fertilized egg implanted and develop to a fetus then to a baby. One end, it is connected to the cervix, the other is connected on both sides to the Fallopian tubes.
1. Bicornate uterus
Bicornate uterus also known as heart-shaped uterus caused by abnormal formation of mullerrian duct, thereby increasing the risk of premature born baby or miscarry due to small than normal uterus.

2. Fibroid
Fibroid is benign tumour which grows in, on or outside of the wall of the uterus. If it grows big enough, it may interfere with the implantation of embryo in the uterus leading to premature delivery, miscarriage and increasing the risk of infertility if they grow near the opening of both Fallopian tubes and distort the shape of uterine captivity.

3. Sepate uterus
Sepate uterus is a condition in which the uterus is divided by septum into separate half. Since the uterus is divided, each half has little space for the fetus to grow. It may not interferes with implantation of fertilized eggs but the pregnancy may end with premature birth of miscarriage.

4. Unicornuate uterus
Unicornuate uterus is a malforming uterus which is formed from one only of the paired mullerian ducts while the other mullerian duct does not develop. Women with unicornuate uterus may never be diagnosis and may be possible to get pregnant but the reproductive failure is high.

5. Uterine factors
Uterine problem such as endometriosis may increase the risk of infertility because it interferes with the implantation of fertilized egg into the uterus.

Juicy melon, fresh lime juice and tropical coconut oil make this a sweet, refreshing --and nutritious - smoothie for warm summer days.
Tips;
Coconut oil is solid at room temperature. It has a melting temperature of 76 degree F (24 degree C), so it is easy to liquefy. To melt it, place in a shallow glass bowl over a pot od simmering water.
To boost the protein content of your smoothie, try adding a scoop of your favorite protein powder -- I prefer hemp or sprouted brown rice protein. Both are available in natural foods store and well-stocked supermarket. Not all are raw, though, so be sure to read the labels.
1 cup Nut milk 250mL
1/2 cup chopped honeydew 125mL
3tbsp. freshly squeezed lime juice 45mL
2tbsp. melted coconut oil 30mL
1/4tsp. raw vanilla extract
In a blender combine nut milk, melon, lime juice, coconut oil and vanilla. Blend at high speed until smooth. Serve immediately. Make 2 cups (500mL)
Variation: If you prefer a sweeter smoothie. add 1 to 2 tbsp. (15 - 30mL) raw agave nectar to taste.
Substitute an equal amount of cantaloupe for the honeydew melon and 1tbsp. (15mL)coconut butter for coconut oil.

Obesity is a medical condition of excess body fat accumulated overtime, while overweight is a condition of excess body weight relatively to the height. According to the Body Mass Index(BMI), a BMI between 25 to 29.9 is considered over weight, while a BMI of over 30 is an indication of obesity. According to the statistic, 68% of American population are either overweight or obese.

You can calculate your BMI index BMI= weight (kg)/ height (m2)

The Studies of Ovulation in obese and lean women

Obesity is known to interfere with reproductive outcomes in polycystic ovary syndrome. According to the University of Queensland, In searched MEDLINE and several other databases from 2000 to September 2011 for studies reporting on OAT and reproductive outcomes. Data were synthesized to determine the risk ratio of reproductive outcomes (ovulation and pregnancy) in lean (BMI <25 kg/m(2)) as opposed to overweight or obese women. with 15 datasets (14 articles) obtained for analysis, included 905 subjects in the obese group and 879 subjects in the lean group, lean women had increased ovulation rates (RR 1.43, 95% CI 1.22-1.66) as compared to obese women. Pregnancy rates also showed a similar trend (RR 1.73, 95% CI 1.39-2.17). Reproductive outcomes were generally better in younger women, more recent studies and randomized controlled trials, posted in PubMed.
