Turmeric Bioactive compound Curcumin, the Potential Treatment for Reducing Radiation Dermatitis Severity in Cancer Patients

Radiation dermatitis is skin damage caused by exposure to radiation. The condition is often found in patients with radiotherapy for the treatment of cancer.

According to statistics, approximately 85% of patients with radiotherapy are affected by the symptoms of moderate-to-severe skin reactions, particularly in the first 2 weeks of the treatment.

Depending on the period of exposure and levels of skin damage, some patients with radiation of dermatitis may experience symptoms of mild skin redness, itchiness, peeling, moistness,... and blistering

In severe cases, patients may also induce painful broken skin, that prone to infection.

If you are treated with radiotherapy, you make sure that you know all the side effects for preventive measures.

Believe it or not, patients with certain medical conditions, inclduing skin disease, obesity
malnutrition, infectious diseases, and diabetes are more prone to radiation dermatitis, compared to those who do not.

Most cases of radiation dermatitis are treated successfully by topical steroid cream often prescribed by radiologists. However, if the topical cream failed, antibiotics, silver leaf nylon dressing may be prescribed to reduce symptoms and prevent infection.

Dr. Benoîte Méry, the lead scientist in the examined the risk of skin damage in breast cancer patients who were treated by radiation wrote, "54 patients were enrolled in 2013. Eight patients (14.8%) had grade ≥2 toxicity. The average weight and chest sizes were 65.5 kg and 93.6 cm, respectively. Bra cup size is significantly associated with a risk of grade 2 dermatitis [odds ratio (OR) 3.46, 95% confidence interval (CI) (1.29–11.92), p = 0.02]. Anthropometric breast fat mass measurements, such as thickness of left [OR 2.72, 95% CI (1.08–8.26), p = 0.04] and right [OR 2.45, 95% CI (0.99–7.27), p = 0.05] axillary fat, is correlated with an increased risk".

And, "Breast size and its different anthropometric measurements (thickness of left and right axillary fat, nipple-to-pectoral muscle distance) are correlated with the risk of skin toxicity".

The results suggested that other parameters, such as the area treated with radiation must also be taken into account of radiation dermatitis.

Turmeric is a perennial plant in the genus Curcuma, belonging to the family Zingiberaceae, native to tropical South Asia.

The herb has been used in traditional medicine as anti-oxidant, hypoglycemic, colorant, antiseptic, wound healing agent, and for the treatment of flatulence, bloating, and appetite loss, ulcers, eczema, inflammations, etc.
With an aim to find a skincare product with no side effects, researchers reviewed the chemistry of turmeric on radiation dermatitis occurred in 95% of patients with radiation therapy (RT) for cancer.

The multisite, randomized, double-blinded, placebo-controlled trial included 686 breast cancer patients received four 500-mg capsules of placebo or curcumin three times daily throughout their prescribed course of RT until 1-week post-RT.

According to the severity (RDS) at the end of RT, using the RDS scale, compared to placebo, oral curcumin did not reduce radiation dermatitis severity at the end of RT.

However, the curcumin group showed an improvement in fewer patients with RDS > 3.0.

In other words, oral curcumin exerted a prominent effect in inducing a trend toward reduced severity compared to the non-curcumin treated group.

According to the reports from participants, curcumin also showed a change in pain, symptoms, and quality of life, although they were not statistically significant between arms.

Taken altogether, turmeric processed a high amount of bioactive compound curcumin may be considered supplements for reducing radiation dermatitis severity, pending to the confirmation of the larger sample size and multicenter human study.

Intake of turmeric in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.

Herbal Andrographis, the Potential Anti Gastric Ulcer Remedy

Scientists may have found a natural plant for the prevention and treatment of gastric ulcers with no side effects, according to studies.

Gastric ulcer is a condition of localized tissue erosion in the lining of the stomach.

Most cases of gastric ulcer are associated with the infection with the bacterium Helicobacter pylori (H. pylori) and long-term use of nonsteroidal anti-inflammatory drugs (NSAIDs).

If you are experiencing the persistent symptoms of dull pain in the stomach, nausea or vomiting bloating, acid reflux heartburn, a burning sensation in the chest pain that may improve when you eat, drink and black color of stool due to bleeding, you may have a gastric ulcer. Please check with your doctor to rule out the possibility.

In serious cases, patients may also develop anemia and vomit blood.

According to the statistic, Peptic ulcer disease (PUD) is a very common disorder in the US, affecting over 4 million people each year.

In the study compared the bacterial infection in patients with duodenal ulcers and gastric ulcer, researchers showed prevalence of H. pylori in antral mucosa was higher in patients with duodenal ulcers (81.5%) than in patients with gastric ulcer and non-ulcer dyspepsia (56% and 52.8%) (P<0.05) and the detection of gastric ulcer is lower compared to duodenal ulcers.

Furthermore, Among patients with antibody to H. pylori, H. pylori were detected in 85.7% of patients with duodenal ulcers, 62.5% of patients with gastric ulcers and 22.2% of patients with non-ulcer dyspepsia(p<0.05).

These results clearly suggested that bacterial H. pylori are the main culprit to ulcers in the digestive system, an indication of adaptive immune defenses against infection and inflammation.

Andrographis paniculata is also known as Andrographis is a herbaceous plant, genus Andrographis, belonging to the family Acanthaceae, native to India and Sri Lanka.

The herbal plant has been used in traditional Siddha and Ayurvedic for the treatment of common cold, flu, upper respiratory infections, infectious diseases, diarrhea, dysentery, cholera, pneumonia, swollen lymph nodes, leprosy, bronchitis, sore throats, tuberculosis, chickenpox, etc.

Besides the main ingredient andrographolides, giving the plant a bitter taste, herbal Andrographis also processes ingredients of diterpene lactones, such as neoandrographolide, andrographiside, 14-doexy-11, 12-didehydro andrographolide, etc., flavonoinds viz., oroxylin, wogonin, andrographidines A, B, C, D, E, F.

In order to find an effective natural compound for the treatment of gastric cancer with no side effects, researchers evaluated the gastroprotective property of andrographolide, a chief component of the leaves of Andrographis paniculata in terms of the ulcer preventive effect in rats.

The dose-responsive study was conducted in rats pretreated with andrographolide (1, 3 and 5 mg/kg) for a period of 30 days, prior to ulcer induction by administering ethanol, aspirin or by pyloric ligation.

During the experiment, researchers found the LD(50) value was found to be 48 mg/kg b. wt and the effective dose was found to be 3 mg/kg.

The application of andrographolide showed an increasing level of the pH and decrease in titrable acidity observed in the gastric fluid of rats pretreated with the test drug.

Andrographolide reduced the expression of the gastric tissue H(+)-K(+) ATPase, an enzyme that functions to acidify the stomach. and myeloperoxidase activities that were elevated in ulcer-induced animals.

Furthermore, the antioxidants and mucin levels were significantly maintained in the gastric tissue of drug-pretreated animals. No toxicity was found in the tested rats.

After taking into account of co and confounders, researchers wrote in the final report, "the ulcer preventive efficacy of andrographolide may probably due to its antioxidant, cytoprotective and antiacid secretory effects"

Additonally, in a study of pretreated with HAEAP (100,200,500mg/kg b. wt for 30 days) and then gastric ulcers were induced by ethanol, aspirin, pylorus ligation, and cold restraint stress models firmly supported the use of a hydroalcoholic extract of Andrographispaniculata (HAEAP) for treatment of gastric ulcers.

Also, the ulcer score was found to be low in HAEAP-pretreated rats, particularly at the dose of 200 mg/kg b.wt. and the tested drug significantly reduced the acidity, pepsin concentration, myeloperoxidase, and H(+)K(+)ATPase activities in ethanol-administered rats as of that in the previous study.

More importantly, the tested drug also normalized the elevated TBARS, formed as a byproduct of lipid peroxidation, overexpression of free radicals and decreased glutathione (GSH), the antioxidant enzymes produced by the host and mucin levels.

Taken altogether, herbal Andrographis may be considered a functional remedy for the prevention and treatment of gastric ulcer, pending to the confirmation of large sample size and multicenter human study.

Alfalfa, the Natural Anti Microbial Whole Food Medicine

Alfalfa is a flowering plant in the genus Medicago, belonging to the family Fabaceae, cultivated all over the world as hay for cattle feeding, and used in traditional medicine over thousands of year to treat high cholesterol, asthma, osteoarthritis, rheumatoid arthritis, diabetes, enhance digestive system, bleeding disorder, kidney and urinary tract infection, etc. North American Aboriginals have used Alfalfa seed as food, such as making bread and mush.

Alfalfa may be a potential antimicrobial agent used for the treatment of infection or infectious related diseases.

According to the Via Orabona 4, Università, chemical compound saponins found in the alfalfa family exhibited antimicrobial activities in a selection of medically important yeasts, Gram-positive and negative bacteria.

Dr. Avato P, the lead author said, "(The antimicrobial) Activity (of alfalfa)was especially high against Gram-positive bacteria (Bacillus cereus, B. subtilis, Staphylococcus aureus, and Enterococcus faecalis) with M. arabica being the species showing a broader spectrum of action. Discrete antifungal activity was also observed, mainly against Saccharomyces cerevisiae".

Furthermore, in the study of 40 steers fed either a sainfoin (hay or silage) or alfalfa (hay or silage) diet over a 9-week period, in vitro, alfalfa fed group showed a decline in fecal E. coli numbers marginally.

The study also suggested the result of the decline of anti-E. coli number is due to the presence of phytochemical phenolics in alfalfa.

In fact, The Karunya University in the investigation of Antimicrobial of Alfalfa(Medicago sativa) in various bacterial strains showed a positive effect of alfalfa seed extract in inhibition of
five bacterial strains namely Bacillus licheniformis, Pseudomonas aeruginosa, Lactococcus lactis, Klebsiella pneumonia. The efficacy also depends on the concentration of extract with the maximum inhibition was seen in 300µg/ml concentration of extract.

There is no doubt that the intake of alfalfa regularly may be beneficiary in reduced risk and treatment of microbial infection and diseases. But further human trials are necessary.

Herbal Artichoke, May Process Anti Non-alcoholic Fatty Liver Disease (NAFLD) Potency, According to Studies

Artichoke may be used as a herbal remedy for the treatment of nonalcoholic fatty liver disease with no side effects, according to studies.

Nonalcoholic fatty liver disease is a fatty liver disease caused by fat accumulated in the liver other than those of excessive alcohol drinking.

The disease, in most cases, is asymptomatic, however, it has been seen to slowly progress to end-stage liver disease.

According to the statistics, the non-alcoholic fatty liver disease affects over 25% of the US population.

Epidemiological studies suggested that most cases of nonalcoholic fatty liver disease are found in obese and extreme overweight adults. However, people with diabetes, high cholesterol or high triglycerides are associated with the increased risk of the diseases. Truly, 75% of obese have steatosis and 20% to 35% have nonalcoholic steatohepatitis (NASH).

As of today, there is no effective treatment in patients with NAFLD, change of diet and lifestyle to induce weight loss are the most common nonmedical treatment conventionally for management of the disease.

Some researchers suggested that the cause of NAFLD may be associated with the promotion of the Western diet over the past few decades.

Dr. Michael D. Roberts, the lead scientist in the evaluation of the Western diet risk in NAFLD, wrote, "...sub-chronic WD feeding appears to increase hepatic steatosis development over a 6-week period but only induces select inflammation-related liver transcripts, mostly acute phase response genes".

And, "diet-induced liver fibrosis and inflammation may either: a) occur much later with more chronic WD rat feeding paradigms; and/or b) may be initiated by other factors that precede eventual perturbations in local and systemic pro-inflammatory signalers with chronic WD feeding".

The results strongly suggested the Western diet pattern have a strong impact on the development of the non-alcoholic fatty liver disease.

Artichoke is a perennial thistle of Cynara cardunculus species of the Cynara genus, belonging to the family Carduoideae, native to Southern Europe around the Mediterranean.

The herbal plant has been used in traditional medicine as a liver protective and detoxified agent, and for the treatment of digestive disorders, abdominal pain gas and bloating, etc.

Scientists on finding a natural ingredient for the treatment of nonalcoholic fatty liver disease examined the artichoke (JA) potential in attenuating lipid disturbances in an animal model.

The study included Wistar of rats fed a control diet, a 60 % fructose-enriched diet (FRU), or an FRU with 10 % JA (n 6-7) for 4 weeks.

After 21 days of the experiment, according to the liver samples collected, JA treatment group showed a significant improvement of a hepatic TAG accumulation, an indication of a fatty liver.

The expression of fat accumulation in the tested rats was observed by the expression of malic enzyme 1 (Me1), associated with fatty acid synthesis; decorin (Dcn), related to fibrosis altered by the FRU.

Additionally, the injection of the supplement also improved the gene associated with liver inflammation caused by FRU.

Furthermore, in order to reveal more information about artichoke anti NAFLD, researchers conducted a randomized double-blind placebo-controlled parallel-group trial in ultrasound-diagnosed NAFLD.

100 subjects were randomized to either ALE 600 mg daily or placebo for a 2-month period.

Injection of AlE showed increased hepatic vein flow (p < .001), reduced portal vein diameter (p < .001) and liver size and levels of liver oxidative stress markers.

ALE treated group improved the liver function by reducing the expression of the levels of bilirubin.

More importantly, ALE supplementation also reduced total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, non-high-density lipoprotein cholesterol, and triglyceride concentrations.

The results showed that artichoke anti-NALFD via several mechanisms, including the inhibition of levels of cholesterol and liver stress markers.

Taken altogether, artichoke may be considered a functional remedy for the prevention and treatment of non-alcoholic fatty liver disease, pending to the confirmation of larger sample size and multicenter human study.

Better than Aspirin Single Herb in Inhibited Platelet Aggregation to Prevent Onset of Cardiovascular Disease without Any Adverse Effects

Arnica may have a profound and positive effect in inhibited platelet aggregation, thus reducing the risk of cardiovascular disease, some scientists suggested.

Arnica is a herbaceous species, genus perennial, belongings to the family Asteraceae, native to Europe and Asia, used in herbal medicine for centuries to treat strains, sprains, and bruises.
Wound healing is a process of repairing after the damage of the skin, tissues, organs, etc.

The ingredients
Helenalin, chamissonolid, Sesquiterpene lactone, lignans of the furofuran, dibenzylbutyrolactone, dibenzylbutyrolactol types, pinoresinol, epipinoresinol, phillygenin, matairesinol, nortrachelogenin, and nortracheloside, six dibenzylbutyrolactol derivatives.

In the study of several plant extracts including Arnica to evaluate anti-platelet and cytotoxic effects on blood platelets, researchers found that flowers of arnica (A. Montana) extract exhibited a significant effect on platelet reactivity and cell viability, according to assays from the standard method.

According to the flow cytometric monitoring of platelet cytotoxicity, the application of a phenolic acid-rich extract of Arnica at doses of 7.5 and 15 µg/ml significantly reduced Adenine di-Phosphate (ADP)-induced aggregation in both whole blood and platelet-rich plasma (PRP).

Further analysis also suggested that the extract decreased the platelet reactivity index (PRI; VASP phosphorylation) used as markers for clinical outcomes in non-ST elevation acute coronary syndrome in whole blood, through its antioxidant capacity.

Interestingly, Arnica extract displayed huge suppressed platelet hyper-reactivity, without influenced cytotoxicity.

These results indicated that high polyphenol content of flower extract of Arnica consisted an excellent antioxidant capacity and exceptional anti-platelet properties in compared to the application of the extract of J. regia husks in reduced ADP-induced platelet aggregation in whole blood and PRI at doses of 7.5 µg/ml and 15 µg/ml.

According to a randomized clinical trial conducted by the Canadian Family Physician, 18 patients selected from the Centre Hostipatier of Laval University divided into 2 groups. group A received Arnica 5CH at first visit and 2 weeks later received placebo and group B received the reverse application as of group A.

In the analysis along the period of the experiment, one patient was excluded from the study due to bleeding over 9.5 minutes compared to only 5 minutes of the normal baseline.

Group A exerted an increase of bleeding time of .59 minutes, compared to reduce of .26 minutes in group B all in compared to normal bleeding time at the baseline.
Application of Arnica showed an significant effect in Fibrinogen (factor I), a glycoprotein circulates in the blood with function to converted enzymatically by thrombin to fibrin and subsequently to a fibrin-based blood clot during injure without affecting the PT, PTT, platelet count and procoagulant activity.

Moreover, application of Arnica also exerted no significant modification of platelet aggregation in application of 4 dilution collagen, ADP and epinephrine in compared the increased levels of platelet aggregation of collagen, ADP and epinephrine after 3 weeks of treatment in the study of 39 patients.

The conflicting result of the study may be caused by several reasons, including
* The nature of patients condition
* Doses of Arnica with a strong implication of blood coagulation was not mentioned used in the experiment
* The types of test use was not clarified
And the result was not a statistic analysis.

Taken together, Arnica as a whole herb and its flower extract may have a potential effect in inhibited platelet aggregation in the induced risk of bleeding. However. due to conflict results, an additional large example size conducted by multi centers is necessary to reaffirm the viability.

Herbal Aloe Vera, the Promising Anti Skin Diseases Functional Remedy

Scientists may have found a herbal plant for the treatment of skin diseases with no side effects, according to studies.

Skin diseases are conditions of skin damage and classified according to each pathogenic cause.

Most cases of skin disease are involved in allergens, irritants. However certain medical conditions such as irritable bowel disease, autoimmune diseases, and genetic preposition can also cause r skin diseases.

Recent studies suggested that a poor diet is also associated with the onset of skin diseases such as the dermatological disease may be linked to systemic disease, and dietary changes may affect health outcomes, as in psoriasis.

Dr. Rajani Katta and colleagues in the dermatological disease team at the Baylor College of Medicine said, " In this review, the authors focus on conditions for which the role of diet has traditionally been an underappreciated aspect of therapy".

"In some cases, dietary interventions may influence the course of the skin disease, as in acne. In others, dietary change may serve as one aspect of prevention, such as in skin cancer and aging of the skin"

" In others, Lastly, systemic medications prescribed for dermatological diseases, such as steroids, are known to raise the risk of other diseases, and dietary change may reduce this risk".

These results clearly suggested that reducing intake of pro-inflammatory food such as fried and process foods and red meats and increasing certain anti-inflammatory foods such as tuna, salmon, garlic into your diet can be helpful in reducing the risk and treatment of skin diseases.

Aloe Vera is species of succulent plant in the genus Aloe, belonging to the Family Xanthorrhoeaceae, native to Sudan.

It has become very popular for commercial cultivation due to its health benefits. Aloe vera has been used in herbal medicine in treating many kinds of diseases, including wound, burn healing, minor skin infections, sebaceous cysts, diabetes, and elevated cholesterol, etc.

The herbal plant is also one of many popular herbs studied in scientific ways with some conflicting results.

The ten main areas of chemical constituents of Aloe vera include amino acids, anthraquinones, enzymes, minerals, vitamins, lignins, monosaccharide, polysaccharides, salicylic acid, saponins, and sterols.

With an aim to find a natural ingredient or remedy for the treatment of skin diseases, researchers at examined all dermatology-oriented in vitro and in vivo experiments and clinical trials on aloe vera preparations.

According to the 40 studies selected for the investigation, oral administration of aloe vera in mice is effective on wound healing in inducing the decrease of the number and size of papillomas.

Furthermore, injection of Aloe vera also exhibited anti-tumor and parasitic effects in reducing the incidence of tumors and leishmania parasitemia by >90% in the liver, spleen, and bone marrow.

The herbal medicine was found to exerted certain skin protective effects against psoriasis, seborrheic dermatitis, frostbite, burn, wound healing and inflammation.

Moreover, the topical gel from Aloe vera can be considered a biological vehicle and an anti-microbial and antifungal agent against skin infection and inflammation.

However, contrary to general belief, the topical application of aloe vera was not effective in the prevention of radiation-induced injuries and sunburn or suntan protection.

The results firmly suggested that Aloe vera showed some promising results for diverse dermatologic conditions.

Taken altogether, herbal Aloe vera may be used as a topical or oral administration agent for the prevention and combined with the primary medicine for the treatment of skin diseases, pending to the reconfirmation of large sample size and multicenter human study.
