Andrographis paniculata in Reduced Risk of Early Onset of Cognitive Impairment

Andrographis paniculata is also known as Andrographis is a herbaceous plant, genus Andrographis, belonging to the family Acanthaceae, native to India and Sri Lanka, used in traditional Siddha and Ayurvedic in treating the common cold, flu, upper respiratory infections, infectious diseases, diarrhea, dysentery, cholera, pneumonia, ...

Cognitive impairment is considered an early stage of dementia-related to progression of aging.

Andrographis paniculata (Burm. F.) Wall. Ex Nees (Acanthaceae), the forgotten herbal medicine may have a potential effect in reducing the early brain cell damage in the expression of cognitive impairment, a renowned institute study suggested.

According to the joint study lead by the Indian Institute of Technology (Banaras Hindu University),
Andrographolide extracts isolated from the herb exhibited a significant property against diabetes-associated cognitive deficits, increased acetylcholinesterase activity and deteriorated antioxidative status which can increase risk of cognitive impairment.

In the study of Streptozotocin-induced diabetic Charles Foster albino rats treated orally with a hydro-methanolic A. paniculata leaf extract (50, 100 and 200 mg/kg/day), or with pure andrographolide (15, 30 and 60 mg/kg/day) for 10 consecutive days, group treated showed an increased levels of antioxidants such as superoxide dismutase (SOD), and catalase (CAT) and decreased lipid peroxidation (LPO) in brain tissues.

Dr. the lead author said, "AP or andrographolide treatments dose-dependently attenuated cognitive deficits, reduced acetylcholinesterase activity, oxidative stress, improved diabetic hyperglycemia, and insulin deficiency".

In the social species, Octodon degus (degu) developed Alzheimer's-like pathology with age, oral administration of (Andrographolide, ANDRO), to aged degus (56-month old) for 3 months in comparison to administrated saline of control groups (young degus: 12-month old and aged degus: 56-month old), showed a strong effect on recovery of spatial memory and learning performance and protection of brain tissues against plague accumulation in inducing brain neural damage associated with aging processes.

In support of the above analysis, the study of "Andrographolide reduces cognitive impairment in young and mature AβPPswe/PS-1 mice" suggested that the efficacy of the herbal medicine not only protects the long-term potentiation (LTP) against the damage induced by Aβ oligomers but also prevent the changes in neuropathology in tested subjects.

In fact, ANDRO also reduced the Aβ levels, changing the ontogeny of amyloid plaques, and tau phosphorylation in regulated brain cell survival around the Aβ oligomeric species.

There is no doubt that Andrographis paniculata may have a pharmaceutical value in treating cognitive impairment, further large sample size and multi centers studies are necessary.

Herbal Anise in Reduced Risk, Progression and Treatment of Gastric Ulcers

Anise may have a potential and substantial effect in reduced risk and progression and treatment of gastric ulcers, some scientists suggested.

Anise is a flowering plant ofthe species of Pimpinella anisum, genus Pimpinella, belongings to the family Apiaceae native to to Egypt and the Mediterranean region, used in traditional herbal medicine as fragrance in soaps, oils, and mouth fresheners and stomachic, anti-septic, anti-spasmodic, ....

Chemical comstituents
Anethole, D-pinene, I-phellandrene, Alpha-terpineol, Safrol, Methyl chavicol, Resin, Shikimic acid, etc.

In the evaluate the claim of Unani and Arabian traditional medicine practitioners on the gastroprotective potential effect by using 80% ethanol, 0.2 mol/L NaOH, 25% NaCl and indomethacin-induced gastric ulceration and secretion in rat model, researchers found that application of doses of 250 and 500 mg/kg body weight of Anise exert a significant protective activity against basal gastric acid secretion, acidity and completely inhibited the ruminal ulceration caused by necrotizing agents, by initiating production of depleted levels of gastric mucosal NP-SH and gastric wall mucus concentration.

Further analysis suggested that the anti gastric ulcers of Anise suspension was attributed the herbal medicine antioxidants in enhanced production of hormone prostaglandins in inhibited levels of gastric secretion.

The hormone has been study intensively for protection and treatment of gastric ulcer by some scientists due to its effect in promoted rapid restitution of the superficial epithelium of the gastric mucosa and preservation of the deeper layers through the ability of the hormones in inhibited gastric secretion, stimulated bicarbonate secretion, and increased gastric blood volume.

According to Dr. Wilson DE, "Orally administered prostaglandin analogues have been helpful in some patients who failed to heal with standard ulcer treatment and in smokers, who usually do not heal as well as nonsmokers".

Some researchers suggested that the prostaglandin analogues may be the ideal solution for patients who are at high risk for gastric lesions due to chronic consumers of nonsteroidal antiinflammatory drugs or alcohol.

Moreover, in the investigation of the effect of essential oil isolated from Croton zehntneri (EOCZ), a plant native to northeastern Brazil and anethole at doses of 30–300 mg/kg, researchers found that the composition demonstrated a strongly and linearly gastroprotective effect against ethanol- and indomethacin-induced gastric damage without changing to cold-restraint stress-induced ulcers in rats.

According to the measurement by Alcian blue binding assay, EOCZ and anethole (both at 30 and 300 mg/kg) similarly and significantly increased the mucus production by the gastric mucosa.

Additionally, the composition also consisted a significant effect on the volume of gastric juice, pH, or total acidity through its ability of EOCZ and anethole in enhanced production of gastric wall mucus.

The above of differentiation were supported by the study to test the major bioactive compound anethole effect in the ethanol-induced ulcer model in mice conducted by the joint investagation lead by Coordenação do Curso de Farmácia,

According to the results, after taking into account of other co-founders, application of anethole (300 mg/kg) increased the level of gastric mucus, while derivatives (300 mg/kg) showed gastroprotective activity without affecting mucus secretion, through anethole antioxidant and anti-inflammation activities.

By examining the antioxidant aspects in the experiment mice, researchers surprisingly found that anethole exhibited protective gastric effect through reduced oxidative stress caused by induced suspension in the activated the gastric mucosal nitric oxide (NO) in maintaining gastric mucosal integrity, and improving production of the natural antioxidants in the host body, including glutathione (GSH), lipid peroxide (MDA, measured as malondialdehyde) glutathione peroxidase (GPx), superoxide dismutase (SOD), and catalase.
Importantly, anethole also reduced oxidative stress in initiated levels of pro-inflammatory substances of plasma tumor necrosis factor-α with function involved in systemic inflammation in the acute phase reaction and interleukin-1β and IL-6 in regulated immune response in protecting the host against bacterial and viral in induction of infection.

Taken together, herbal Anise and its majors chemical constituent anethole may be considered as a secondary therapy accompanied with conventional medicine for treatment of gastric ulcer. However, intake of Anise supplement should be taken extreme care in prevention of acute liver toxicity.

Herbal Cinnamon - The new generation for Treatment and Management of Diabetes

Cinnamon, a kitchen spicy in many cultures, may have gained many believers, including medical professions because of its anti-diabetic effect, through regulating the blood glucose levels in healthy and diabetic subjects. According to herbalists, intake of 1/4 tsp. in the morning and evening may reduce the blood sugar significantly with little or no adverse side effects.

The herb has been used in herbal and traditional medicine as anti-fungal and bacteria level to improve the reproductive organ, prevent flatulence and intestinal cramping, treat indigestion, diarrhea, bad breath, headache, migraine, etc.(1).

According to the 2011 statistic from National Diabetes InformationClearinghouse (NDIC), among U.S. residents ages 65 years and older, 10.9 million, or 26.9 percent, had diabetes in 2010, about 215,000 people younger than 20 years had diabetes—type 1 or type 2—in the United States in 2010 and approximately, 1.9 million people ages 20 years or older were newly diagnosed with diabetes in 2010 in the United States.

In a total of 66 patients with type 2 diabetes study conducted by Xuhui District Central Hospital, showed a reduction significant of blood triglyceride levels even in the low-dose group, after 3 months of oral administration(2)

Some researchers suggested that cinnamon extract improved fasting blood glucose levels and glucose tolerance without altering insulin secretion, through lower triglyceride and increased liver glycogen content and improved insulin action in liver tissues(3).

Dr. Kim SH and Dr. Choung SY. told PubMed that "the effect of cinnamon in significantly increases insulin sensitivity, reduces serum, and hepatic lipids, and improves hyperglycemia and hyperlipidemia possibly by regulating the PPAR-medicated glucose and lipid metabolism"(4)

In support of the above, chromium(Cr) and polyphenols found in cinnamon(Cinnamomum cassia) indicated an improvement of glucose, insulin, cholesterol, and HbA1c in patients with type 2 diabetes following Cr supplementation, in a double-blind placebo-controlled study(7).

Unfortunately, according to the Western University of Health Sciences, consumption of cinnamon is associated with a statistically significant decrease in levels of fasting plasma glucose but insisted that high degree of heterogeneity may limit the ability to apply these results to patient care, due to the unclear preferred dose and duration of therapy(5)

A randomized clinical trial in which 70 patients with type II diabetes assigned randomly two groups (35 in cinnamon and 35 in placebo group for the testing of the effect of cinnamon on the glucose level in blood, indicating an insignificant effect of using cinnamon on glucose level of diabetic patients and the herb should not be recommended to patients with type II diabetes depending on further studies(6).

Taken altogether, cinnamon in a low dose may be effective in controlling the levels of blood glucose in a patient with diabetes without altering the insulin level. according to DR. Georgakopoulou EA(7), Oral intake of Cinnamon may cause Stomatitis and burning sensation in mouth with large amounts. The traditional Chinese medicine also insisted that Overdoses of cinnamon can be toxic and damaged to the liver(1).

Cranberry Processes the Potent Anticancer Activity in Vitro

Cancers are a class of chronic and medical conditions characterized by cells growth irregularly and disorderly in the tissue of a specific organ.

Most cases of cancer start in the cell on the surface of the inner lining of organ tissue caused by the alternation of the cell DNA.

At the early stage, due to the very small size of the tumor, patients may not experience any symptoms. However, as the cancer progression, cancer cells may penetrate into deeper layers, before colonizing entire organs. At this stage, the primary cancer cells can travel a distance away from the original site to infect other healthy tissue and organ via fluid and blood circulation.

According to the National Cancer Institute, approximately 38.4% of men and women will be diagnosed with cancer at some point during their lifetimes, based on the 2013–2015 data.

In 2017, about15,270 children and adolescents ages 0 to 19 were diagnosed with cancer. Cancer in the same year also causes the death 1,790 of children and adolescents Americans.

As of today, researchers do not know the exact cause of cancer neither they know why people with the same health condition and family history, some are susceptible to DNA alternation in facilitating the onset of cancer, while others do not.

There are many prevalent factors associated with cancer onset, including an increase of age, excessive alcohol consumption, exposure to chemical toxins and sunlight, and infectious pathogens.

Most common factors that cause the rise of cancer, including colorectal cancer in the Western world according to Dr. Omran S, the led author in the study of "Dietary and Lifestyle Risk Factors for Colorectal Cancer in Apparently Healthy Adults in Jordanian Hospitals" are dietary patterns, sedentary lifestyle, and other practices.

In CRC risk, the doctor continued, aging with the mean age of 57.0 ± 8.56 years, male gender with less than secondary school formal education.

And, the commonest risk factors for CRC among the participants were overweight or obesity (76.1 %), lack of exercise (71.6 %), limited consumption of vegetables (70.8 %), smoking (60.6 %), over-consumption of red meat (56.3 %), and diabetes mellitus (24.1 %)".

However, most people with some of the aforementioned risk factors have never developed cancer.

Cranberry is an evergreen dwarf shrub, genus Vaccinium, belongings to the family Ericaceae, native to Northern America and Southern Asia. Because of its health benefits, cranberry has been cultivated in some parts of the world for commercial profit and used in traditional and herbal medicine to treat wounds, urinary disorders, diarrhea, diabetes, stomach ailments, and liver problems.

On finding a potential compound that processes chemopreventive activity, researchers assessed the anti-cancer activities of Chlorhexidine (CHX) and Cranberry against oral cancer cell lines.

According to the results of AW13516 (poorly to moderately differentiated squamous cell carcinoma of the tongue) and KB (Nasopharyngeal carcinoma) using Sulforhodamine B (SRB) assays, the CHX efficacy of 80.15% & 95.7% of growth inhibition against AW13516 & KB cell line respectively.

* Administration of CE (80µg/ml) showed no anti-cancer property against AW13516 cell line.

*However, CE inhibited 70.6% growth against KB cell line.

Furthermore, both CHX and CE were less potential than positive control drug Adriamycin, at the values of GI50, TGI, and LC50.

Based on the findings, researchers said, "CHX exhibited better anti-cancer properties than CE for both the oral cancer cell lines".

In other words, CE exerted anti-cancer activity depending on the types of primary cancer cells.

Taken altogether, cranberry may be considered a remedy for the prevention and treatment of cancer, depending on the types of primary cancer cells, pending to the confirmation of the larger sample size and multicenter human study.

Fennel In Attenuated Risk, Progression and Treatment of Irritable Bowel Syndrome

Herbal fennel may have a strong implication in reduced risk and treatment of irritable bowel syndrome, some scientists postulated.

Fennel (Foeniculum vulgare) is a plant species of genus, belongings to Apiaceae (Umbelliferae), native to the Mediterranean, used in traditional and herbal medicine as warming, carminative, antispasmodic, antidepressant agent and to stimulate the appetite, ease indigestion, soothe coughing, reduce intestinal spasms, to regulate the menstrual cycle and relieve PMS,...

Irritable bowel syndrome is a common digestive disorder, affecting the function of the large intestine.

According to the statistic, approximately 10 -15% of the world population is infected by the disease, depending on geographic variation.

According to the study of 121 patients with mild-to-moderate symptoms of IBS defined by an Irritable Bowel Syndrome- symptom severity score (IBS-SSS) 100-300 and abdominal pain score 30-70 on a 100 mm Visual Analogue Scale (VAS), randomly assigned to Curcumin-Fennel (CU-FEO) or placebo (2 capsules b.d. for 30 days), treatment group demonstrated a significant effect in decreased mean scores observed by the relative IBS-SSS and the pain score in compared to placebo group.

In the overall performance, intake of CU-FEO group showed an incredible improvement of all symptoms with the percentage of symptom-free patients significantly higher of 26 % in compared to 7% the placebo group, particularly, in all domains of IBS-quality of life(QoL), without inflicting any side effect.

Furthermore, in the period of the experiment, curcumin + fennel treatment displayed a progressive decrease in intensity and a greater proportion of patients in reporting ≥50% reduction in abdominal pain.

In the length of the experiment, the treated group expressed a substantial improvement of health-related QoL observed through the reduced use of health resources and increased work productivity, progressively in compared to the baseline taken before the treatment.

Dr. Portincasa P, the lead author after taking into account of other co-founders, said, "CU-FEO was safe, well-tolerated and induced symptom relief in patients with IBS" and " CU-FEO significantly improved symptoms and quality of life in IBS patients over 30 days".

Some researchers suggested that the efficacy of fennel in reduced symptoms of irritable bowel syndrome was possibly associated to the major chemical component anethole of fennel oil seeds in exerting relaxant effect on intestinal smooth muscle, similar to those medicine used as neurotransmitter dopamine in inhibited the spasmodic activity in the facilitation of abdominal pain.

Moreover, the application of fennel also expressed a strong impact on bowel symptoms through orchestrated intestinal inflammation and immune activation against the onset and progression of the disease.

Injection of anethole in vitro and Vivo parameters of chicken immunity during experimental avian coccidiosis exerted an important modulation of the immune mediators interleukin-6 (IL6) in response to infection and tissue injuries and contribution to host defense; interleukin-8 (IL8) in stimulated production of neutrophils, basophils, and T-cells during the inflammatory process; interleukin-10 (IL10), an anti-inflammatory factor which plays a critical role in reduced overexpression of proinflammatory cytokines in precipitated cells damage and cells death and tumor necrosis factor ligand superfamily member 15 (TNFSF15) in response to acute and chronic inflammation by induced the development of the new blood vessel to promote the site of infection.

These result addressed an impression of fennel anethole effect in improved symptoms and quality of life of patients with IBS through inhibited the increased number of inflammatory cells in the colonic and ileal mucosa activated by low-grade intestinal inflammation

The low-grade inflammation in the pathophysiology of irritable bowel syndrome accompanied by microscopic and molecular abnormalities in the large subset in patients today has been focused by some scientists in searching for new conventional treatment for the disease through designed clinical trials for drugs that target this pathophysiologic mechanism.

Taken together, fennel with major bioactive compound anethole may be considered as a functional food and secondary therapy in reduced risk and progression and used the combination with conventional medicine for the treatment of irritable bowel syndrome

Herbal Anise in Ameliorated Risk and Treatment of Functional Dyspepsia

Anise may have a potential and substantial effect in reduced risk and treatment of functional dyspepsia (FD), some scientists suggested.

Anise is a flowering plant of the species of Pimpinella anisum, genus Pimpinella, belonging to the family Apiaceae native to to Egypt and the Mediterranean region, used in traditional herbal medicine as a fragrance in soaps, oils, and mouth fresheners and stomachic, anti-septic, anti-spasmodic, ....

Chemical constituents
Anethole, D-pinene, I-phellandrene, Alpha-terpineol, Safrol, Methyl chavicol, Resin, Shikimic acid, etc.

Functional dyspepsia (FD) is a chronic disorder characterized by sensation and movement (peristalsis) in the upper digestive tract.

In the study of total of 180 patients attending the gastroenterology clinic, 107 ones with the diagnosis of postprandial distress syndrome according to Rome III criteria, randomly assigned to anise group of 47 patients received anise powders, 3 g after each meal (3 times/day) for 4 weeks and placebo group of 60 patients received placebo powders (cornstarch), 3 g after each meal (3 times/day) for 4 weeks researchers at the joint study lead by the Iran University of Medical Sciences indicated that

Anise treatment scored a significant improvement of all domains of SF-36(The Short Form (36) Health Survey) at week 12 in compared to placebo group, although both groups scored a similar at the baseline.

More specifically, at week 12, the mean of physical component score was 141 in placebo group and 251 in Anise treatment group compared to the baseline scores of 159 and 167, respectively and the mean mental component scores at weeks 12 was 135 in placebo group and 233 in anise group in compared to 172 and 165 respectively.

Domain of SF-36 completed by the patients includes vitality, physical functioning, bodily pain, general health perceptions, physical role functioning, emotional role functioning, social role functioning, and mental health, used widely in managed care organizations and Medicare for routine monitoring and assessment of care outcomes in adult patients. The lower the score is an indication of more disability in compared the higher the score of less disability.

The strong improvement of physical and mental scores in the Anise group in compared to the baseline and placebo may be associated to Anise in contributed to increase quality of life in patients of functional dyspepsia, through reduction of symptoms intensity.

Further differentiation, in a total of 107 patients attending the gastroenterology clinic, aged 18-65 years, diagnosed with PDS according to ROME III criteria, randomly assigned to group of 47 patients receive either received anise powders, 3 g after each meal (3 times/day) or placebo involved 60 patients and received placebo powders (corn starch), 3 g after each meal (3 times/day) for 4 weeks. researcher found that anise group expressed a significant effect in reduced all symptoms at weeks 4 and 12, but retching, nausea and vomiting in compared to control group and at the baseline.

The examine of the mean total scores of FD severity scale in anise group also expressed improvement at weeks 2, 4 and 12 in compared to insignificantly different of the two groups before the intervention.

Anise intervention also demonstrated a decreased significantly of mean severity scores of epigastric fullness, epigastric discomfort, epigastric burning/pain, early satiety, bloating, belching, and loss of appetite in compared to epigastric pain, epigastric fullness and belching increased significantly in the placebo group after intervention.

Moreover, Anise group also exerted a strong improvement of mean of pain score in compared to placebo, probably as a result of its anti spasmolytic feature.

However, after taking into account of other co-founders such as factors in causes of FD and anise effects on broad spectrum of pharmacological effects on GI, nervous, muscular and immune systems, researchers concluded that there are no surprise that Anise application showed a significant improvement of symptoms in patients with FD.

The information findings may be astonished, but the study also found to induce certain limitation, including limited the external validity of the results with a small sample size and shorted follow up periods.

Taking together, herbal Anise not only improve quality of life in patients with functional dyspepsia but may also be considered as adjunct therapy in combined with conventional standard medicine for reduced symptoms and treatment of functional dyspepsia.
