           Attention Deficit Disorder (ADHD/ADD)

Attention Deficit Disorder (ADHD/ADD) also known as attention deficit hyperactivity disorder is a chronic disease occurs in certain age and years of a children, defined as a condition with characteristics of co-existence of attentional problems and hyperactivity. It effects about 5% of the all children in the world with boys have 3-4 times higher risk than girl. Over one third of children with ADHD will have continued symptoms existed into their adult life.

                 The Herbal medicine - Skullcap

Herbal medicine believe ADHD is a result of coordination failure in the reticular activating system of the brain. Therefore, herbs used to treat ADHD are those improves concentration, attention, motivation and performance, promote the stay on task and normal energy levels and particular enhance the flow of oxygen and nutrients to the brain and supports brain functions in information processing and memory, etc.

a. Liver diseases
In the evaluation of the anti-proliferative effects on human hepatoma HepG2 and Hep3B cells of each fraction of Pheophorbide a, a major antitumor component purified from Scutellaria barbata, found that pheophorbide a induced apoptosis in Hep3B cells, a viral-induced hepatoma cell line. However, it was found to be non-toxic in normal human liver cells WRL-68. DNA fragmentation, sub-G1 cell cycle arrest, as well as suppression of the anti-apoptotic protein Bcl-2, release of cytochrome c to the cytosol, and activation of pro-caspase 3 and pro-caspase 9 were observed when Hep3B cells were treated with 40 microg/mL (i. e., 67.5 microM) pheophorbide a for 48 hours, according to "Pheophorbide a, a major antitumor component purified from Scutellaria barbata, induces apoptosis in human hepatocellular carcinoma cells" by Chan JY, Tang PM, Hon PM, Au SW, Tsui SK, Waye MM, Kong SK, Mak TC, Fung KP.(74)

b. Antioxidants
In the Comparisons of Scutellaria baicalensis, Scutellaria lateriflora and Scutellariaracemosa: genome size, antioxidant potential, found that Antioxidant potential studies showed that there were no significant differences in the 3 Scutellariaspecies. Phytochemical analyses detected and quantified the flavonoids baicalin, baicalein, scutellarin, and wogonin as well as the human neurohormones melatonin and serotonin in leaf and stem tissues from S. baicalensis, S. lateriflora, and S. racemosa, according to "Comparisons of Scutellaria baicalensis, Scutellarialateriflora and Scutellaria racemosa: genome size, antioxidant potential and phytochemistry" by Cole IB, Cao J, Alan AR, Saxena PK, Murch SJ.(75)

c. Neurodegeneration diseases
In the study of flavones isolated from Scutellaria baicalensis root exhibit strong neuroprotective effects on the brain, found that Baicalein, the main flavone present in Scutellaria baicalensis root, strongly inhibited aggregation of neuronal amyloidogenic proteins in vitro and induces dissolution of amyloid deposits. It exerts strong antioxidative and anti-inflammatory activities and also exhibits anti-convulsive, anxiolytic, and mild sedative actions. Importantly, baicalein, and also another flavone: oroxylin A, markedly enhanced cognitive and mnestic functions in animal models of aging brains and neurodegeneration, according to "Flavones from root of Scutellaria baicalensis Georgi: drugs of the future in neurodegeneration?" by Gasiorowski K, Lamer-Zarawska E, Leszek J, Parvathaneni K, Yendluri BB, Błach-Olszewska Z, Aliev G.(76)

d. Anxiolytic activity
In the investigation of the phytochemistry and biological activity of Scutellarialateriflora L. (American skullcap) which has been traditionally used as a sedativeand to treat various nervous disorders, found that these compounds may play a role in anxiolytic activity since baicalin and baicalein are known to bind to the benzodiazepine site of the GABAA receptor and since GABA is the main inhibitory neurotransmitter, according to "Phytochemical and biological analysis of skullcap (Scutellaria lateriflora L.): a medicinal plant with anxiolytic properties" by Awad R, Arnason JT, Trudeau V, Bergeron C, Budzinski JW, Foster BC, Merali Z.(77)

                                 Dietary Minerals

Dietary Minerals are the group of minerals which is essential for our body to sustain normal functions and physical health.

                                      Calcium

Calcium, a trace mineral plays an important role in build and maintain strong bones and teeth, found abundantly in meat, poultry, fish, nut, seeds, bean, etc. As we age, calcium is absorbed less effectively.

Calcium and aluminum in Neurodegenerative disorders

Both calcium and aluminum have been implicated in the cell damage and death that occurs in several neurodegenerative disorders including Alzheimer's disease (AD). In the study to examine the effects of experimentally elevated intraneuronal levels of aluminum ([Al]i) and/or calcium ([Ca2+]i) on neuronal degeneration and antigenic alterations in the microtubule-associated protein tau in cell cultures of rat hippocampus and human cerebral cortex, showed that Exposure of cultures to Al3+ alone (200 microM) for up to 6 d did not result in neuronal degeneration. Neurons exposed to the divalent cation ionophore A23187 degenerated within 4 h when Ca2+ was present in the culture medium whether or not Al3+ was present. Measurements of [Ca2+]i using the calcium indicator dye fura-2 demonstrated a direct relationship between increased [Ca2+]i and neuronal degeneration. In contrast, neurons did not degenerate when exposed to A23187 in the presence of Al3+ and the absence of Ca2+, despite a 10-fold elevation in [Al]i as measured by laser microprobe mass spectrometry. Calcium influx, but not aluminum influx, elicited antigenic changes in tau similar to those seen in AD neurofibrillary tangles. Neurons exposed to glutamate in the presence of Al3+ but in the absence of Ca2+ were not vulnerable to injury. Finally, increased [Al]i occurred in neurons that degenerated as the result of exposure to glutamate indicating that aluminum associates with degenerating neurons(10).

                   Cirrhosis

Cirrhosis is defined as a condition of irreversible scarring liver as a result of liver tissue by fibrosis due to final phase of chronic liver diseases of that can lead to poor function of the liver and liver failure. According to the statistics, Number of discharges with chronic liver disease or cirrhosis as the first-listed diagnosis: 101,000 in 2009 and Deaths per 100,000 population: 10.3 in 2010(a). Hepatitis B infection cause of the disease is very prevalent in South-East Asia.

               The Treatments

A. Treatments of cirrhosis depend not on the underline causes and stage of the diseases
Treatment of the complications of cirrhosis - Variceal bleeding
Variceal bleeding as a complication of liver cirrhosis is a result of veins in the esophagus, stomach, and rectum enlarge due to blood flow through the liver was blocked. Patients with variceal bleeding, are at 70% greater risk to have a further bleed within 2 yr after, and patients with acute variceal bleeding has a significant mortality of 5% to 50%.
The amin purpose of the treatment to reduce and stop blood loss
A.2.3.1. In conventional medicine
Portal hypertension can lead to life-threatening hemorrhage, ascites, and encephalopathy. In the study to review the pathophysiology and multidisciplinary management of portal hypertension and its complications, including the indications for and techniques of the various surgical shunts. Variceal bleeding is the most dreaded complication of portal hypertension, showed that treatment of acutevariceal bleeding includes resuscitation followed by upper endoscopy for sclerosis or band ligation of varices, which can control bleeding in up to 85% of patients. Medical therapies such as vasopressin and somatostatin can also be useful adjuncts. Shunt therapy, preferably the placement of a TIPS, is indicated for refractory acute variceal bleeding. Recurrent variceal bleeding is common and is associated with a high mortality. Therapies to prevent recurrent variceal bleedinginclude chronic endoscopic therapy, nonselective beta-blockade, operative or nonoperative (TIPS) shunts, devascularization operations, and liver transplantation. Recommendations and a treatment algorithm are provided, taking into account both the etiology and the manifestations of portal hypertension(90).

1. Anticoagulation
In the study to determine the safety and efficacy of anticoagulation treatment for portal vein thrombosis in cirrhosis patients with acute variceal bleeding, with patient eligibility determined by contrast ultrasonography findings, indicated that early anticoagulation treatment in cirrhosis patients with portal vein thrombosis and acute variceal bleeding may be safe, tolerated, and effective in cases with positive intra-thrombus enhancement on contrast ultrasonography(91).

2. Prophylactic antibiotics
According to the study by Jasmohan S Bajaj, MD, Arun J Sanyal, MD, in the article of Treatment of active variceal hemorrhag, Multiple trials evaluating the effectiveness of prophylactic antibiotics in cirrhotic patients hospitalized for bleeding suggest an overall reduction in infectious complications and possibly decreased mortality. Antibiotics may also reduce the risk of recurrent bleeding in hospitalized patients who bled from esophageal varices. Thus, patients with cirrhosis who present with upper GI bleeding(92).

3. Variceal banding ligation and Injection sclerotherapy
Injection sclerotherapy of bleeding oesophageal varices is undoubtedly beneficial but it is associated with a substantial complication rate, and variceal rebleeding is common during the treatment period before variceal obliteration is achieved. According to the study to compare whether endoscopic variceal banding ligation is safer and more effective by King's College Hospital, showed that there was no difference in outcome between the groups, but 14 sclerotherapy patients were withdrawn from the trial (7 for orthotopic liver transplantation) compared with only 5 (1 for liver transplantation) in the banding ligation group (p < 0.05). Complication rates were similar in the two groups. Variceal banding ligation is a safe and effective technique, which obliterates varices more quickly and with a lower rebleeding rate than injection sclerotherapy(93).

4. Transjugular Intrahepatic Portosystemic Shunt (TIPS) and endoscopy
According to the study to compare early use of transjugular intrahepatic portosystemic shunt (TIPS) with endoscopic treatment (ET) for the prophylaxis of recurrent variceal bleedingby the First Affiliated Hospital of Xi'an Jiaotong University, early use of TIPS is more effective than endoscopic treatment in preventing variceal rebleeding and improving survival rate, and does not increase occurrence of hepatic encephalopathy(94).

5. Distal splenorenal shunt (DSRS)
In the study to examine the cost and cost effectiveness of distal splenorenal shunt(DSRS) and transjugular intrahepatic portosystemic shunt (TIPS) in the prevention of variceal rebleeding, showed that
TIPS is as effective as DSRS in preventing variceal rebleeding and may be more cost effective. TIPS, in all aspects, is equal to DSRS in the prevention of varicealrebleeding in patients who are medical failures(95).

6. Periesophagogastric devascularization and fundectomy
Cirrhotic patients with gastric fundal bleeding occasionally require operative intervention. According to the study by the Seoul National University, indicated that patients who were successfully stabilized by preoperative endoscopic intervention had significantly lower mortality ( p < 0.001). During follow-up there was no recurrent bleeding from gastric varices, and there was only one case (4.35%) of hemorrhage from esophageal varices. Hence, periesophagogastricdevascularization and fundectomy offers an alternative operative method for cirrhotic patients with variceal hemorrhaging from the gastric fundus(96).

7. Etc.

A.2.3.2. In traditional Chinese medicine
In the study to find a method for inducing Chinese drugs to adhere to the esophageal mucosa to control bleeding from ruptured esophageal varices, showed that The adhesion of Chinese hemostatie drugs remained in the lower segment of esophagus for more than 15 minutes in lying posture, longer than that in standing posture (P < 0.01). (2) The effectiveness of controlling bleeding esophageal varices had no significant difference between the balloon tamponade and adhesion of Chinese hemostatie drugs (P >0. 05)(96a).

Among elderly men, cancer of the prostate (451 per 100,000), the lung (449 per 100,000) and the colon (176 per 100,000) make up around half of all diagnosed cancers. Prostate cancer is around 22 times more frequent among elderly men than among younger men. The corresponding most frequent cancers among elderly women, making up 48% of all malignant cancers, are breast (248 per 100,000), colon (133 per 100,000), lung (118 per 100,000) and stomach cancer (75 per 100,000), According to the Danish Cancer Society, Institute of Cancer Epidemiology(PubMed)

Cancer is a class of diseases in which a group of cells growing and multiplying disordered and uncontrollable way in our body, have become progressively worse and damaged other healthy tissues, sometimes spreads to other organs in the body via lymph or blood and results may be in death.

Cancers are primarily an environmental disease with 90-95% of cases attributed to environmental factors and 5-10% due to genetics.
Causes of cancer can be divided into 2 groups

                 Hereditary genetic causes of Cancer

Due to abnormalities of certain class of gene in cells division and replication such as oncogenes and tumor suppressor genes.

C. Hormone
Some hormones factor can enhance the development and spread of sex-related cancer, such as cancer of the breast, endometrium, prostate, ovary, and testis, and also of thyroid cancer and bone cancer.

D. Inflammation
Although there is no evidence of inflammation caused caner, repeated injuries to the same tissues might promote excessive cell division and replication, thus increasing the risk of a cancerous mutation.

                    Overcome Infertility

Infertility is defined as inability of a couple to conceive after 12 months of unprotected sexual intercourse. It effects over 5 millions couple alone in the U. S. and many times more in the world. Because of unawareness of treatments, only 10% seeks help from professional specialist.

Computer-Assist Semen Analysis of  Male Infertility In Conventional medicine Perspective

I. Definition
The sample of sperm is scanned in to the computer pre store program to show how the sperm quality, quantity, shape and movement. Since any small change of the computer may produce a significant change in sperm calculation, therefore any abnormal sperm count should be confirmed by manual count.

II. Procedure
Since 25% of all infertility is caused by a sperm defect and 40-50% of infertility cases have a sperm defect as the main cause, or a contributing cause. The second test that your doctor order usually is the computer assist semen analysis if the normal semen analysis fail to given a conclusive result. A sample semen sperm is collected through masturbation in the clinic laboratory.

III. Diagnosis
It said that CASA provides more accurate reading than normal semen analysis, because 25%
of all samples shown as infertility in normal semen analysis even with the normal fertility male. The computer will study chemical substances and sperm quality, quantity as in normal semen analysis, including
1. Volume
The semen sample required for this test is than 2.0 ml or more
It measures the number of sperm per millilitre ( ml ) in the semen.
2. pH
It is requires in the range of 7.2-8.0 to be considered as normal.

3. Sperm concentrationFor a normal sperm concentration it requires 20,000,000 per ml or more.

4. Motility
The normal sperm motility is 50% or more with forward progression. It can be classified in 4 grade
a) Sperm with progressive motility:
b) Non-linear motility
c) Non-progressive motility:
d) Non motility:
