The Science Base of Herbs in Induced Weight Loss and Weight Maintenance

Overweight and obesity have reached epidemic proportions in many countries, causing economic burden in increased absentees in the workplaces and funding for the health care system in dealing with obesity-related disorders such as diabetes, hypertension, and cardiovascular diseases(1).

One of six Malaysian are either extremely overweight or obese, according to Datin Paduka Santha Kumari, chairman of the Selangor branch of the Malaysian Diabetes Association and the global health observatory, at least 2.8 million people die each year as a result of being overweight or obese, and an estimated 35.8 million (2.3%) of global burden of disease is caused by overweight or obesity(2).

Epidemiological studies, linking herbal medicine, foods, and vitamins in preventing and treating these diseases have been inconclusive(a)

Some researchers suggested that the intake of certain herbal medicine may enhance appetite suppression.

According to the Georgetown University Medical Center, the appetite of average daily food intake was decreased only with the herbal formulation, regardless number of single-ingredient in comparison to the effectiveness among herbal formulation and a commonly available(3).

Other researchers insisted that using herbs and supplements to induce weight loss should be taken with care, as a considerable number of reports have been published on hepatotoxicity associated to intake of herbal products(4)(5)(6)(7).

The College of Medicine, The Ohio State University insisted that various dietary, lifestyle and psychological factors are involved in the etiology of Prameha, in initiated disturbances of fat and carbohydrate metabolism in overweight and obese(8), without effective management, obtaining a workable weight loss plan may be extremely difficult.


1. Ginseng

According to the Tehran University of Medical Sciences, Slimax (extract of several plants including Zingiber officinale and Bofutsushosan) showed a significant decrease in body weight or inhibited weight gain(18).

Application of composition of a multi-ingredients supplement containing primarily raspberry ketone, caffeine, capsaicin, garlic, ginger, and Citrus aurantium, also improved body composition, waist, and hip girth, in an eight-week diet and exercise weight loss program(19).

In a comparison of the weight loss effect of Orlistat, a pharmacological agent promoting weight loss, injection of ginger (Zingiber officinale Roscoe, Zingiberacae) in male albino rats induced a great ability to reduce body weight without inhibiting pancreatic lipase level, or affected bilirubin concentration, and increased peroxisomal catalase level and HDL-cholesterol(20).

3. Cissus quadrangularis (CQ)
Cissus quadrangularis is an ancient herb used to treat a variety of indications, belongings to the grape family, probably native to India or Sri Lanka.

4. Sambucus nigra
Elder also known as Common Elder, is a shrub or small tree of the genus Sambucus of 30 specie, belongings to the family Adoxaceae, native to the temperate-to-subtropical regions of both the Northern Hemisphere and the Southern Hemisphere, used in traditional and herbal medicine to treat constipation, colic, diarrhea, colds, flu, bronchial and upper-respiratory, irritation of the skin, rheumatism, etc.

The composition of supplement with Sambucus nigra and Asparagus officinalis study expressed effectiveness in reduced weight, blood pressure, enhanced physical and emotional well-being and quality of life(24).

In 12 weeks supplementation of a botanical extract-based weight loss formula, including Asparagus, researchers at the Poznan University of Medical Science indicated that the formula exerts a significant change of the body composition improvement index (BCI), induces greater weight loss than placebo, probably through a concurrently performed exercise program recommended strategy for lifestyle modification(28).

6. Slimax
Slimax (extract of several plants including officinale and Bofutsushosan), in the reviews of the database of studies published, researchers showed that intake of Slimax indices a significant decrease in body weight(29).

In 41 animal studies, the formula was found to be effective in significant induced weight loss or inhibited weight gain(29).

Taken altogether, the above herbs may be effective in enhanced weight loss through suppression of appetite and improvement of syndrome induced by overweight and obesity. However, the intake of herbal supplements and composition should only be prescribed by herbal doctors.

FDA banned Ephedra (Ma Huang) weight loss products in 2004
[Ma Huang is also known as ephedra. The acrid, slightly bitter and warm herb has been used in TCM to promote urination and sweating and rid of cold, soothe wheezing asthma, ...., by enhancing the functions of lung and bladder channels(9).

According to Dongguk University, certain chemicals found in Ephera showed an association with changes of BW and BMI through alteration of gut microbiota varied by indigenous microbiota of tested subject(10).

The combined study of a low-calorie diet and Ephera, in 125 otherwise healthy obese women, showed effectively in reducing BMI. RMR changes not compensated for by the herbal medicines tried.

Resting metabolic rate (RMR) change seemed to be affected by constitution and body composition rather than by medicine(11). The Dongeui University study of composition GGEx18( Laminaria japonica, Rheum palmatum, and Ephedra sinica) in high fat diet-induced obese mice suggested that GGEx18 improves obesity through skeletal muscle AMPK and AMPK-stimulated expression of PPARα and its target enzymes for fatty acid oxidation, through inhibited lipid accumulation, and similar activation of genes(12).

Please note that due to its adverse reactions of palpitations, stress, headache and insomnia and stroke, heart attacks, cardiac arrhythmias, seizures, and psychotic effect, the FDA banned the product on 2004, the Dutch Inspectorate for Health Care and the Ministry of Health in Canada have recently requested a market recall of some ephedrine-containing herbal products in response to above adverse reactions. In the Netherlands, the status of Ephedra-containing products is currently reconsidered(13)]

Ginger, May Be A Natural Alternative for Testosterone Hormone Therapy with No Side Effects, According to Studies

Scientists may have a pungent spice which processes a potential to increased levels of testosterone deficiency, particularly in the elderly with no side effects according to studies.

Testosterone is a steroid hormone found in humans made mostly from the testicles in men and a small amount in women's ovaries.

By searching the PubMed, Scopus, and Web of Science databases for English language articles (full texts or abstracts) from November 1991 through August 2018 using the keywords "ginger" and "Zingiber officinale" versus "testosterone" linking the effects of ginger on testosterone researchers found that ginger supplementation, particularly in oxidative stress conditions, enhances testosterone production in males.

The mechanisms by which this occurs mainly by enhancing luteinizing hormone (LH) production in stimulating the Leydig cell production of testosterone, increasing the level of cholesterol in synthesizing male sex steroid, testosterone (T), by reducing oxidative stress and lipid peroxidation in the testes.

Furthermore, application of ginger also inhibited the expression of free radical by promoting the activity of the antioxidant enzymes.

Ginger also normalized the blood glucose by increasing blood flow to the testes.

Moreover, ginger improved production of testosterone by increasing testicular weight and recycling testosterone receptors.

The finding strongly supports the use of ginger in the enhanced testosterone production for treatment of diseases associated with the testosterone deficiency.

In order to reveal more information about ginger in the production of testosterone by examing the

ginger effects on multiple animal models, including 60adult male Sprague Dawley rats divided into * Group A served as normal control,
* Group B received 0.3% lead acetate in drinking water,

*Group C and group D received supplementary 0.5 and 1 gm/Kg body weight of ginger respectively along with lead acetate in drinking water.


During the experiments, co-administration with different doses of ginger showed a significant rise in the testosterone levels in of teste, compared to the lead-treated group.

Interestingly, ginger application at different doses showed no significant change of LH levels, compared to the rise in the previous ginger treatment,

These results strongly suggested there must other compounds from ginger that exhibits the increased levels of LH that is not mentioned in both studies.

Taken altogether, ginger may be considered a functional remedy for the prevention and combined with primary medicine for the treatment for diseases associated with testosterone deficiency.

Fennel In Ameliorated Risk, Progression and Treatment of Breast Cancer

Fennel may have a potential and therapeutic effect in reduced risk and treatment of breast cancer, some scientists suggested.



Breast cancer is a medical condition characterized by cell growth disorderly and uncontrollably in the breast tissue. At the later stage, cancerous cells may travel a distance away to infect other healthy tissue and organs.

In the evaluation of the essential oil extracted from fennel activity in the human breast cancer cell line (MDA-Mb), researchers found that the application of fennel extract exerted a significant effect in inhibition of the expression of cancer cell lines at a dose of lower than 10 μg/mL with IC50.

According to the University of Malaya in investigation of the DNA protecting activity and inhibition of nicotine-induced cancer cell migration of 9 spices, fennel exerted a potential effect in protection of the DNA integrity and inhibition of nicotine-induced cancer cell migration against Murine fibroblasts (3T3-L1) and human breast cancer (MCF-7) cells pre-treated with spice extracts and then exposed to H₂O₂ and nicotine.


Additional analysis suggested that the result of inhibition was attributed to fennel antioxidants of phenols and flavonoids in ameliorated H2O2 in initiated chain activity in the induction of cytotoxicity in altered DNA of the experiment cell line.



Some researchers suggested that overproduction of hydrogen peroxide may serve as a prodrug in the presence of transition metal ions, in induced production of highly reactive hydroxyl radicals to initiated DNA damage in precipitated apoptosis.


In other words, the application of highly oxidative hydrogen peroxide in the breast cancer cell line may cause a significant cytotoxic effect in cancer cell death.

These results demonstrated the correlation of increase of apoptosis with the rise in the generation of reactive oxygen species in cancer cells.

Due to this potential activity, some researchers suggested that hydrogen peroxide overproduced in tumor cells can serve as a prodrug for the treatment of cancer.

On the other hand, the application of H2O2 in the healthy cells may induce DNA damage in the initiation of cancer cell development.



Furthermore, fennel scored a statistically significant DNA protecting activity compared to other spices, such as at ginger with 50 μg/ml in reduced 68% of DNA damage in 3T3-L1 cells.

However, Fennel exerted a lesser effect in inhibited cancer MCF-7 cells with nicotine-induced cell migration compared to other spices such as pepper, long pepper, and ginger.


The above differentiation was supported by the joint study lead by the Avicenna Tajik State Medical University, in the evaluation of the antioxidant effect of essential oil of fennel (Foeniculum vulgare) against MCF-7 (human breast adenocarcinoma).
According to the result from Gas chromatographic-mass spectrometric analysis, fennel displayed a moderated antioxidant in precipitated cytotoxic activity against MCF-7 (human breast adenocarcinoma) with IC50 values between 30-210 mg L-1 in compared to cytotoxic reference compounds.
In fact, the efficacy of fennel essential oil in inhibited breast cancer proliferation and migration was attributed to the presence of lipophilic secondary metabolites and anise aldehyde, which have potential in elevated the interference with molecular targets in cells by fennel antioxidant compounds such as anethole.


Taken together, fennel and its bioactive compound may be considered as functional food in reduced risk, progression, and treatment of breast cancer. However, intake of fennel supplements should be taken with extreme care in prevented liver acute toxicity.

Studies suggested that garlic may have the potential in killing bowel cancer cells before they can spread to form colonization.

In vitro, intake of garlic and its lipid-based extracts, induced cell cycle arrest, through a fourfold increase in caspase activity (apoptosis) in PC-3 cells when treated at a dose of 0.5 or 1 μg/m and may be considered a potential as promising anticancer agents, including colon cancer.

Moreover, lipid-peroxidation associated with liver stress was inhibited by WS administration in hypercholesteremic animals, compared to other normal counterparts.

Based on the findings, researchers said, " it appeared that WS root powder is also effective in normal subjects for decreasing lipid profiles".
