Osteoarthritis is an inflammation of joints caused by the depletion of the cartilage on the ends of the bones due to the wear and tear over time.
The disease is one most common forms of arthritis that affect millions of people worldwide.
According to the statistics, in the US, approximately 10% of men and 13% of women aged 60 years or older have osteoarthritis (OA).
Most common risk factors found in patients with OA are older adults, female gender, overweight and obesity, knee injury, repetitive use of joints, low bone density.
Besides pain on the joints, the other most common symptoms of OA include tenderness, stiff, low flexibility, grating sensation.
Conventionally, treatment of OA is focused on managing symptoms, by reducing pain, stiffness and swelling through the use of pain killers, improving joint mobility and flexibility and maintaining a healthy weight by exercise.
Some researchers suggested that the high prevalence of OA in the US has become a burden to the health care cost and economic burden.
Dr.Bitton R, the lead scientist at the Health Plan of Nevada wrote, "approximately one-third of direct OA expenditures are allocated for medications, much of which goes toward pain-related agents. Hospitalization costs comprise nearly half of direct costs, although these expenditures are consumed by only 5% of OA patients who undergo knee or hip replacement surgery".
And, "Indirect costs for OA are also high, largely a result of work-related losses and home-care costs".
Bromelain, a proteolytic enzyme found in pineapples (Ananas comosus) has been used in traditional medicine as an inflammatory agent and to treat pains, strains, and muscle aches and pains and ease back pain and chronic joint pain, skin diseases, etc.
With an aim to find a natural compound for diseases associated with inflammation, researchers investigated the effects of bromelain in mild-to-moderate knee OA patients.
The randomized, single-blind, active-controlled pilot study included 40 knee OA patients randomized to receive oral bromelain (500 mg/day) or diclofenac (100 mg/day).
At week 4, both bromelain and diclofenac treated groups showed an improvement of total Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and pain subscales compared to the baseline.
According to the tested assays, bromelain and diclofenac groups exerted similar effects after 4 weeks.
At week 16 of bromelain treatment, patients had improved total WOMAC scores (12.2 versus 25.5), pain subscales (2.4 versus 5.6), stiffness subscales (0.8 versus 2.0), and function subscales (9.1 versus 17.9), and physical component of SF-36 (73.3 versus 65.4) as compared with baseline values.
In other words, bromelain reduced symptoms and improved quality of life in OA patients by inhibiting the levels of oxidative stress and overexpression of pro-inflammatory cytokines at week 16.
Based on the results, researchers after taking other factors into account wrote, "Our results show the inhibitory effect of enzyme therapy on collagen-induced arthritis in rats, as well as the efficacy of cyclosporin".
Taken altogether, bromelain may be considered supplements for the prevention and treatment of osteoarthritis (OA), pending to the confirmation of the larger sample size and multicenter human study.
Intake of turmeric in the form of supplements should be taken with extreme care to prevent overdose acute liver toxicity.

Bilberry May Process Anti Breast Cancer Potency with No Side Effects, According to Medical Literature
Bilberry may be the next potential and therapeutic treatment of breast cancer with no side effects, according to studies.

Dr. Manuel Picon‐Ruiz, the lead scientist in the study of "Obesity and adverse breast cancer risk and outcome: Mechanistic insights and strategies for intervention", wrote "Obesity is associated both with a higher risk of developing breast cancer, particularly in postmenopausal women, and with worse disease outcome for women of all ages".
And, " the effects of obesity to increase local and circulating proinflammatory cytokines, promote tumor angiogenesis and stimulate the most malignant cancer stem cell population to drive cancer growth, invasion, and metastasis".
The results clearly suggested that if you are obese, your risk of breast cancer is increased substantially, compared to non-obese individuals.
Bilberry is a species of low-growing shrubs in the genus Vaccinium, belonging to the family Ericaceae, native to Northern Europe.
The herbal plant has been used as foods and herbs in traditional medicine for the treatment of acute and chronic diarrhea, gastritis, gastric ulcer, and duodenal ulcer, enterocolitis, ulcerative colitis, anemia, cystitis, kidney disease, and psoriasis, diabetes, etc.
Researchers on finding a natural compound for the treatment of breast cancer examined the bilberry effect on the MCF7 human breast cancer cells.
Bilberry extract inhibited cell proliferation in a concentration-dependent fashion with a 50% inhibitory concentration of 0.3-0.4 mg/mL.
At this concentration, the natural compound also concerts the induction of apoptotic cell death of the cancer cells with no apparent effect on the microtubule that provides structure and shape to the cytoplasm of eukaryotic cells or actin cytoskeletons.
In other words, bilberry extract induced cancer cell apoptosis was not through cell cycle arrest at this concentration.
However, at the higher extract concentrations (0.5-0.9 mg/mL), the extract did cause an increase in the fraction of cells at the G(2)/M phase of the cell cycle.
Further differentiation of the cell death, bilberry extract at 0.3-0.4 mg/mL did not appreciably inhibit microtubule polymerization in vitro, but significant inhibition of polymerization (approximately 30%) did occur at higher extract concentrations (0.5-1 mg/mL).
Moreover, researchers found that the efficacy of bilberry against breast cancer cells was attributed not by the expression of anthocyanins but all phytochemicals in the extracts.
Dr. Nguyen V and colleagues wrote in the final report, " (Bilberry extract) inhibits proliferation of and induces apoptosis in breast cancer cells at its lowest effective concentrations via a mechanism that does not involve action on microtubules or on mitosis.
And, "At somewhat higher concentrations the extract modifies microtubule organization in cells and causes accumulation of cells at mitosis by a direct action on microtubules".
Taken altogether, bilberry may be considered a functional remedy for the prevention and treatment of breast cancer, pending to the confirmation of larger sample size and multicenter human study.

Herbal Anise in Ameliorated Menopausal Hot Flash Frequency and Severity

In the investigation of the effect of Pimpinella anisum and symptoms of hot flash in postmenopausal women, researchers at the Shahid Beheshti Universit conducted a review of medical records at health centers of sample of 72 women with hot flashes randomly selected according to the predetermined criteria and divided into experimental group received a capsule containing 330 mg Pimpinella anisum 3 times a day and control group received 3 capsules, each containing 330 mg potato starch, over 4 weeks with pre-treatment of frequency and severity of hot flashes taken in 2 weeks before administration indicated that women in anise treatment group exert a significantly reduced frequency and severity of hot flashes from the pre-treatment of 4.21% and 56.21% to 1.06% and 14.44% at the end of the fourth week respectively in compared to control group.
Further analysis found that women who are in the anise testing group also continue benefits from the ameliorated of the frequency and severity of hot flashes in 4 weeks follow up examination.
These results suggested that long term intake of herbal Anise may have a continued efficacy in relief and recurrence of menopausal hot flashes.
Additional illustration of the prevalence of hot flash in menopause women by reviewing the literature published on MEDLINE (1966 to January 2015), Scopus (1996 to January 2015), and Cochrane Central Register of Controlled Trials (The Cochrane Library, issue 1, 2015), researchers at joint study lead by the Mashhad University of Medical Science showed that overall studies approved the herbal effects of Anise in contribution of reduced hot flash occurrence and frequency as well as alleviating the side effects of hot flashes.
Moreover, in the double-blind clinical trial conducted to determine the effects of Anise abstract on hot flashes in women referring to rural and urban health centers of Qazvin province during 2006 by randomly assigned 72 postmenopausal women at different verities selected according to pre-determined criteria and divided into group of experimental group took a capsule containing 100 mg Anise 3 times a day and control group received 3 placebo capsules, each containing 330 mg starch over 4 weeks experimental periods, searchers after compared to the frequency of hot flashes and the types of food they took 1 week before administrations found that there is significant difference in hot flash expression in these 2 groups of women
The Anise group showed a substantial decrease in the frequency and severity of hot flashes from 4.21% and 56.21% before treatment and 1.06% and 14.44% at the end of the fourth week, respectively.
And women in control expressed an insignificant change of hot flash frequency and severity in compared to measurement before treatment.
Interestingly, after taking account of other confounders, Dr. Fatemeh Nahidi, the lead author said, " women can be taught to choose different alleviating methods, preferably".
In other words, by providing enough information, women who suffered from menopause hot flash should be able to choose different alleviating methods in relief and recurrence of the symptoms.
Women who do not tolerate the side effect of conventional medicine with symptoms of the hot flash may want to add a portion of Anise into their diet, please consult with your herbalist before taking the herbs for preventive measure.
Due to the short term of 4 weeks of study, some researchers suggested that further studies with longer duration of treatment Anise for assessing hot flashes are necessary.

Injection by the thiol antioxidant N-acetylcysteine (NAC) in the experiment showed to attenuate all cellular effects of CuB completely.
Interestingly, pretreatment with glutathione synthesis inhibitor butithione-sulfoxime (BSO), significantly exacerbated CuB's cytotoxic effects against the tested cancer cells in vitro and Vivo.
Further analysis of the anti-NSCLC efficacy of CuB, researchers found that CuB induces cell death via rapidly and significantly decrease the levels of protein thiols and GSH/GSSG ratio by disrupting cellular redox balance induced cytotoxicity.
Dr. Kausar H, the lead scientist said, "CuB interacts with and modulates cellular thiols to mediate its anti-cancer effects. Collectively, our data shed new light on the working mechanisms of CuB and demonstrate its therapeutic potential against NSCLC".
Furthermore, CuB also exerted anti-NSCLC in vitro and Vivo CuB by exhibiting a strong anti-migratory and anti-invasive ability against metastatic NSCLC at nanomolar concentrations.
In other words, CuB showed significant tumor angiogenesis-inhibitory effects by the inhibition of migratory, invasive and tube-forming capacities of human umbilical vein endothelial cells.
Moreover, in experimental EMT model CuB also suppressed the overexpression of protein associated with the production of proinflammatory cytokines in the initiation of cellular change and Wnt/β-catenin signaling axis in stimulating the production of formation of new blood vessels to facilitate cancer proliferation.

In other words, TSL-mediated HO induction may be regulated at the translational level involved in the cellular response to stressors, growth cues, and differentiation through its anti-inflammatory properties.
Pro-inflammatory proteins such as nitric oxide (NO), tumor necrosis factor (TNF)-alpha, and prostaglandin E2 (PGE2) were also inhibited by the administration of TSL in lipopolysaccharide (LPS)-stimulated RAW264.7 cells.
Based on the findings, researchers said, "TSL may be an effective HO-1 inducer that has anti-inflammatory effects and a valuable compound for modulating inflammatory conditions.".
