Herbal Comfrey(Symphytum officinale L.), the Potential and Natural Treatment of Myalgia

Myalgia is a group of diseases associated with muscle pain.

Most common types of myalgia are
* Systemic lupus erythematosus

The systemic lupus erythematosus (SLE) is an autoimmune disease characterized by the body's immune system abnormal function that attacks healthy tissue in many parts of the body, inducing symptoms of severe fatigue, joint pain, swelling, and headaches.

*Systemic sclerosis
Systemic sclerosis (SSc) is an autoimmune disease of the connective tissue that may be misdiagnosed as rheumatoid arthritis. According to Dr. Horimoto AM and Dr. Costa IP, arthritis, clinically observed, maybe a feature often found in a patient with Systemic sclerosis, often leading to early misdiagnosis with rheumatoid arthritis (RA).

The most common symptoms of systemic sclerosis are hair loss, calcium deposits, or white lumps under the skin, small, dilated blood vessels under the skin's surface and joint pain.
* Polymyositis

Polymyositis (PM) is a type of chronic inflammation of the muscles associated with dermatomyositis and inclusion body myositis.

The most common system of polymyositis is muscle pain and stiffness, muscle weakness, particularly in the belly (abdomen), shoulders, upper arms, and hips and joint pain and stiffness.

The most common causes of myalgia are overuse, injury, or strain of muscles. However, myalgia can also be caused by diseases, medications, and in some, it is a response to vaccination.

Conventionally, most cases of myalgia are treated by over-the-counter medications such as Tylenol (acetaminophen) or nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin, ibuprofen, and naproxen

Comfrey is a perennial plant in the genus of Symphytum, belonging to the family Boraginaceae, native to Europe. It has been used in herbal and traditional medicine to ease excessive bleeding or on open wounds, relieve pain and swollen, treat circulation issues, reduce cholesterol, etc.

On finding a potential compound for the treatment of diseases associated with chronic pain, researchers investigated the effectiveness and tolerability of the topical Symphytum product Traumaplant (Harras Pharma Curarina, München, Germany) (10% active ingredient of a 2.5:1 aqueous-ethanolic pressed concentrate of freshly harvested, cultivated comfrey herb [Symphytum uplandicum Nyman], corresponding to 25 g of fresh herb per 100 g of cream) in the treatment of patients with myalgia (n=104) were tested against a 1% reference product (corresponding to 2.5 g of fresh comfrey herb in 100 g of cream; n=111).

According to the results from the primary efficacy parameter in this double-blind, reference- controlled, randomized, multicenter study of 215 patients with pain in the lower and upper back and motion and the. secondary efficacy parameters included pain at rest, pain on palpation, and functional impairment, high concentrations of the treatment product showed a significant amelioration of pain on active motion (P<5 x 10 -9 ), pain at rest (P<.001), and pain on palpation compared to the reference product which was clinically highly relevant.

The high-concentration product exerted a significantly better global efficacy and faster onset of effects.

Tolerability of the highly concentrated study product was good to excellent in all patients.

Based on the results, researchers said, "Study results confirm the known anti-inflammatory and analgesic effects of topical (Symphytum cream. As a new finding, applicability in certain forms of back pain can be concluded".

Taken altogether, Comfrey combined with other herbal medicine for the treatment of myalgia, pending to the confirmation of the larger sample size and multicenter human study.
